United States National Health Measurement Study, 2005-2006 (ICPSR 23263)

The National Health Measurement Study (NHMS) surveyed older United States adults with a suite of health-related quality of life (HRQoL) indices to allow comparison and cross-calibration of these instruments. The design oversampled African Americans and older individuals to allow subgroup analyses. Several preference-weighted indices measuring self-reported generic HRQoL are used widely in population surveys and clinical studies in the United States and around the world. These indices are used to evaluate individual and population health. Because they have been developed using econometric methods to elicit utility weights for their scoring systems, they are generally accepted for use in cost-effectiveness analyses of health interventions. Each index uses a multidimensional representation of health, but each index covers the dimensions of health (e.g., physical function, mental function, social function, pain, other symptoms, etc.) differently, and uses questionnaires with different psychometric properties. Each index is scored so that perfect health is represented as 1.0 and dead is represented as 0.0, but they are known to have different scaling properties. Rarely have two or more of these instruments been included in a population survey, so there have been few opportunities to directly compare how they describe and measure health using multi-instrument data. In this study, respondents indicated whether they had been diagnosed with coronary heart disease, stroke, diabetes, arthritis, eye disease, sleep disorder, chronic respiratory disease, clinical depression or anxiety disorder, gastrointestinal ulcer, thyroid disorder, and/or severe chronic back pain. Census tract is not identified, however race composition, education levels, economic factors, and urbanicity of each respondent's census tract of residence are included as contextual variables. Demographic, socioeconomic, and additional health data were elicited. Respondents are characterized by census region of residence, age, gender, marital status, race, ethnicity, education, household income and assets, health insurance, weight, height, smoking status, psychological well-being scales, and everyday and lifetime discrimination items. The data were de-identified, and extensive documentation was developed. The NHMS collected data on 3,844 adults in the continental United States (1,641 males and 2,203 females, 1,086 African Americans).

National Hospital Discharge Survey, 1979-2006: Multi-Year Public Use File (ICPSR 24281)

The National Hospital Discharge Survey (NHDS) collects medical and demographic information annually from a sample of hospital discharge records. Variables include patients' demographic characteristics (sex, age, race, marital status), dates of admission and discharge, source and type of admission, status at discharge, final diagnoses, surgical and nonsurgical procedures, dates of surgeries, and sources of payment. Information on hospital characteristics such as bed size, ownership, and region of the country is also included. This collection includes data for non-newborns for 1979-1989 (Dataset 1), non-newborns for 1990-2006 (Dataset 2) and newborns for 1979-2006 (Dataset 3). The medical information is coded using the INTERNATIONAL CLASSIFICATION OF DISEASES, 9TH REVISION, CLINICAL MODIFICATION (ICD-9-CM). In addition, there are several Excel files that contain information needed to calculate relative standard errors (RSEs) and to compute utilization rates based on Census population estimates (POPs).

Contraceptive Needs and Services in the United States, 1994-2016 (ICPSR 38891)

These data come from surveillance activities conducted by the Guttmacher Institute over several decades, collecting or compiling data for the period 1994 through 2016. These activities track the numbers of women who have a potential demand for contraceptive care (because they are of reproductive age, sexually active and not seeking to become pregnant), the subset of these women who likely need public support for care (because of their family income level or their age), the numbers of women who receive contraceptive services from publicly funded clinics, and the numbers of clinics providing publicly supported contraceptive services. These efforts have been conducted periodically, typically about every five years, but at times the intervals between efforts were shorter or longer than five years. The most recent data were collected or compiled for 2015 (women served) and 2016 (women with potential demand for services).

This release includes two separate datasets. Dataset 1, "Need for contraceptive services," provides county-level aggregate data for 6 different years (1995, 2000, 2002, 2006, 2010, and 2016). For each county, the data represent estimates of the number of women who have a potential demand for contraceptive services and the number who likely need public support for care, both in total, and according to key socio-demographic characteristics. Dataset 2, "Clinics providing contraceptive services and women served," provides county-level aggregate data for six different years (1994, 1997, 2001, 2006, 2010, and 2015). For each county, the data represent the number of publicly funded clinics according to clinic type and funding status and the number of female contraceptive patients served at those clinics.

National Survey of American Life Self-Administered Questionnaire (NSAL-SAQ), February 2001-June 2003 (ICPSR 27121)

The National Survey of American Life, 2001-2003 (NSAL) was followed up by a self-administered interview (NSAL SAQ) as a way to reduce respondent burden following the 2 1/2 hour NSAL survey. The SAQ includes additional questions about social, group, and individual characteristics: psychological resources (i.e., John Henryism), group and personal identity (racial awareness and identity), as well as ideology and racial relations (i.e., social dominance; stratification beliefs; egalitarianism; national pride; work ethic; authoritarian, interracial contact; and exposure to Black social contexts); political attitudes (i.e., Race-conscious Policy Index, Race-blind Policy Index, Non-Electoral Participation Index); care of elderly values; job and financial stressors; and wealth. Demographic variables include age, race, and sex.

Pathways to Adulthood: A Three-Generation Urban Study, 1960-1994: [Baltimore, Maryland] (ICPSR 2420)

This collection incorporates both prospective and retrospective data on three generations of families initially living in inner-city Baltimore, Maryland. The prospective data were selected from data collected as part of the Johns Hopkins Collaborative Perinatal Study (JHCPS), a survey of pregnant women seeking prenatal care and delivery at Johns Hopkins Hospital during 1960-1964. JHCPS studied these women (the first-generation mothers, abbreviated as G1) and the children born to them during 1960-1965 (the second-generation children, abbreviated as G2) until the children were 8 years old. The retrospective data come from a follow-up study, conducted in 1992-1994, of G1, G2, and the children born to G2 (the third-generation children, abbreviated as G3). Data from JHCPS on G1 include obstetrical and reproductive history at registration for prenatal care, sociological/family history variables at or around delivery of G2, observations of mother with child when G2 was 4 months old and 8 months old, and family history, demographic, and sociological variables when G2 was age 7. For G2, the data from JHCPS include delivery room observations at birth, pediatric examination data at age 4 months, developmental evaluation data at age 8 months, pediatric-neurological examination data at age 12 months, language, hearing, and speech evaluation summary data at age 36 months, psychological, behavior profile, physical growth, and other tests at age 48 months, psychological, motor, behavior, neurological, vision, physical, and other tests at age 7-1/2 years, and language, hearing, and speech evaluations, physical growth, interval medical history, and other tests at age 8 years. Retrospective data from the follow-up study on G1 include variables on education, employment, family composition, health and health care usage, housing conditions, income and income sources, marital status, partnerships and changes, neighborhood characteristics at registration to JHCPS and current, and reproductive history. For G2, data from the follow-up include information on aspirations, education, schooling, employment, family composition, health and health care usage, housing conditions, income and income sources, legal problems, living arrangements, marriage, partnership and changes, neighborhood characteristics at birth, at ages 11/12 and 16/17, and current, reproductive history, social relationships, smoking, and substance abuse. Data for the assessed third-generation children, i.e., G3s who were 7-8 years old during the follow-up period, include information on cognitive development, academic achievement and behavior, prenatal care, health, day care, and parental aspirations.

Annual Health Survey (AHS), India, 2007-2012 (ICPSR 38097)

The Annual Health Survey (AHS), conducted by the Government of India between July 2010 and May 2013, investigates maternal and child health in nine states: Assam, Bihar, Chhattisgarh, Jharkhand, Madhya Pradesh, Orissa, Rajasthan, Uttarakhand, and Uttar Pradesh. These states constitute about 70 percent of neonatal deaths in India and about one-in-five neonatal deaths globally. The AHS consists of a three-round panel that interviewed over 4 million households in each round, as well as a one-time Clinical, Anthropometric, and Bio-Chemical Survey (CAB). The data were originally released to the public in 2015 as a set of 45 .csv files. The .csv files are included in a restricted-use zipped package as part of the ICPSR release (see dataset 21).

The survey focused on topics such as household composition, caste, fertility, family planning, pre- and post-natal care, breastfeeding, infant mortality, illness, disease, disability, and health care practices. Demographic information includes sex, age, education, occupation, marital status, household size, and religion. The CAB files contain biometric data including but not limited to height, weight, blood pressure, hemoglobin, pulse, and blood glucose.

Potential data users should note that the public-use and restricted-use versions of the datasets are the same except for the masking of day component variables for certain dates in the public-use versions of the files (please see the Description of Variables section for full details). Therefore, only researchers with a limited set of research questions that require full birth, marriage, and death dates will need to apply for the restricted-use versions of the data files.

Additionally, because the final data files are very large and potentially very time consuming to analyze on personal computers, researchers have the option to download ten-percent samples of each file (see datasets 3, 4, 7, 8, 11, 12, 15, 16, 19, and 20). These samples contain the same variables as the original files but only ten percent of the records. The samples were determined by taking a randomly selected ten percent of households in each district. P.I. codebooks were not produced for these samples. Please note that the ten-percent samples for each dataset were selected independently, so it is not advised to merge across datasets within the AHS using these samples, as the match rates will be very low.

National Comorbidity Survey: Adolescent Supplement (NCS-A), [United States], 2001-2004 (ICPSR 28581)

The National Comorbidity Survey Replication Adolescent Supplement (NCS-A) was designed to estimate the lifetime-to-date and current prevalence, age-of-onset distributions, course, and comorbidity of DSM-IV disorders in the child and adolescent years of life among adolescents in the United States; to identify risk and protective factors for the onset and persistence of these disorders; to describe patterns and correlates of service use for these disorders; and to lay the groundwork for subsequent follow-up studies that can be used to identify early expressions of adult mental disorders.

The core NCS-A interview schedule was an adaptation of the World Health Organization Composite International Diagnostic Interview (CIDI). NCS-A also administered the non-verbal subtest (Matrices subtest) of the Kaufman Brief Intelligence Test (K-BIT).

In addition to interviewing adolescents, information was collected from a parent or a parent surrogate to obtain an additional perspective on the adolescent's mental health and its correlates. Information from parents focused on the five adolescent disorders for which previous methodological research has most consistently shown that parental reports are important for making diagnoses: attention-deficit/hyperactivity disorder, conduct disorder, oppositional defiant disorder, major depressive episode, and dysthymic disorder.

Demographic information collected by NCS-A includes age, citizenship status, country of birth, criminal history, ethnicity, grandparents' country of birth, language(s) spoken in the home, parents' country of birth, race, religion, and sex.

The data collection contains six data files: (1) data for the adolescent household and school respondents; (2) data for the parents who responded to the long self-administered questionnaire; (3) data for the parents who responded to both the long self-administered questionnaire and short telephone interview; (4) diagnostic variables derived from the data collected from the adolescents and parents; (5) K-BIT scores normed to the NCS-A adolescent sample; and (6) raw K-BIT data.

NICHD Study of Early Child Care and Youth Development: Phase III, 2000-2004 [United States] (ICPSR 21942)

The overall purpose of this study was to examine the influence of variations in early childcare histories on the psychological development of infants and toddlers from a variety of family backgrounds. This general objective was addressed through a prospective, longitudinal study of the experiences of 1,364 children and their families, which took into account the complex interactions among child characteristics and those of the human and physical environments in which the children were reared.

Research Goals

The specific research aims were as follows:

  • Examining the relationship between infants' childcare arrangements (defined in terms of hours, type, quality, and stability of care and the age at which the child entered care) and children's concurrent and long-term development. Specifically, the study investigated the association between children's experiences in childcare and their social, emotional, language, and cognitive development. The social-emotional assessments included measures of attachment, independence, compliance, behavior problems, prosocial and antisocial behavior, and general competence in interacting with peers. Cognitive variables include general developmental level and problem solving skills. Language assessments incorporated measures of children's expressive and receptive communicative competence.

  • Examining whether the social ecology of the home moderates the effects of childcare, i.e., whether children from different home environments are differentially affected by similar childcare experiences. The study examined the moderating effects of parents' values and attitudes, psychological adjustment and mental health, stress and social support, child-rearing practices, time use, interactions with the child, the marital relationship, and family demographics.

  • Examining whether individual differences among children moderate the effects of infant care on child development. The study examined the moderating effects of such child characteristics as age, sex, health, birth order, and temperament.

  • Identify demographic and family characteristics associated with families' childcare decisions. The study examined whether specific childcare arrangements are related to the parents' social class, marital status, psychological adjustment and personality, child-rearing values and attitudes, parenting practices, stress, social support, marital relationship, and the availability of childcare in the community.

  • Provide a natural history of infant care in the 1990s, and help establish a baseline of data pertaining to the kinds of care being used by families. Whereas other national databases, such as those provided by the United States Census Bureau, provide static estimates of the number of children in different types of childcare, this network study supplements that knowledge with longitudinal data on successive enrollments into day care at various ages, patterns of arrangements used concurrently and over time, and the stability of arrangements during the first three years of life. One of the most valuable aspects of the collaborative study is the opportunity it provides to obtain a more complete and accurate picture of patterns of infant care used by families today. Census surveys use only gross categories of care (e.g., center vs. in-home). In this study, more fine-grained information regarding the types of centers and home-care facilities was gathered.

  • Examine the consequences for families of maternal employment and childcare choices. Family relationships, parental mental health, family stress, and so on, are not just inputs to child development or moderators of childcare effects, they are also outcomes. High-quality childcare may alleviate family stress and enhance parental adjustment. Low-quality childcare may add to the stress parents experience. Although the main focus in the study was on the effect of childcare on the child, the study also examined the effect of childcare on the family.

  • Identify demographic characteristics of childcare associated with childcare quality. Of interest to policy makers is another aspect of the study, the investigation of those regulatory characteristics that predict care of higher quality. These characteristics included the level and type of caregiver training, the size of the childcare group, the auspices of the childcare program (public/private, profit/nonprofit, independent/chain, employer-sponsored/church-based), whether the facility was licensed or unlicensed, the level of payment and fees, and whether the caregiver was a relative of the family.

Data File Organization

504 data files were compiled for this study and are organized into 4 main groups:

  1. Analytical Data Sets (ADS) -- The raw data were examined and composites defined by small groups of individual principal investigators according to the demographic, family, childcare, and child outcome content of the data. The psychometric and distributional qualities of the variables along with site differences were examined. A set of variables that was psychometrically and distributionally acceptable to be used in analytic analyses was designed to test the study hypotheses. These data files comprise Parts 1-49 of the study data material.

  2. Supplemental Data Sets -- New and revised analysis variables as well as across-time mean scores and primary composites were produced as a supplement to the original Analytical Data Sets. These data files comprise Parts 50-55 of the study data material.

  3. Raw Census-Related Data Sets -- Files were produced using geocoded addresses for survey respondents to match block group level data from the 1990 and 2000 Censuses for investigators to create additional measures of interest from the geocoded addresses. These data files comprise Parts 56-58 of the study data material.

  4. Raw Data Sets -- The raw data were made available and comprise Parts 59-505 of the study data material.

Training Workshop

A three-day summer training workshop on the SECCYD was put on by NICHD at the Inter-University Consortium for Political and Social Research in Ann Arbor, Michigan in 2010. The binder from that workshop, which includes the Powerpoint slides used during presentations, are freely available to the public as part of the study documentation.

These programs produced one output data file per subject, which were combined to produce some of the raw data files for those studies. The original program output is included as expanded documentation in this phase of the study.

Violence Against Athabascan Native Women in the Copper River Basin [Alaska], 2003 (ICPSR 25923)

A participatory evaluation was used to examine factors associated with the prevalence and incidence of violence against Ahtna (Alaska Native) women in the Copper River basin of Alaska. Eligibility for participation in the study was limited to adult women over the age of 17 who were Ahtna shareholders or descendents of Ahtna shareholders and who lived in one of eight Ahtna villages (Mentasta Lake, Chitina, Cantwell, Copper Center, Gulkana, Gakona, Tazlina, and Chistochina) in the Copper River Basin of Alaska. The Ahtna Corporation provided research staff with a list of 185 women who met the eligibiltiy criteria. The list from the Ahtna Corporation did not include individuals born after 1972 who had not yet inherited shares in the Ahtna Corporation. With the assistance of subjects and village officials, researchers utilized snowball sampling to identify female Ahtna descendents over the age of 17 within the region. These subjects were recruited through face-to-face contact with project staff. Each of the 185 women on the list of eligible participants that researchers received from the Ahtna Corporation was sent a personal letter in 2003 inviting her to participate in the study. Included in the letter was the interview consent form. A few weeks after mailing, research staff contacted those women who had responded to the mailing to review methods for completing the survey and begin scheduling interviews. Study participants completed the Main Victimization Survey (Part 1) (n = 109), and if the respondent reported a violent incident, a Detailed Physical Assault Incident Report (Part 2) (n = 186) was completed for each offender that had assaulted the survey respondent. All respondents were paid 25 dollars for their participation in the survey and all of the interviewers were female. The Main Victimization Survey (Part 1) includes variables about physical violence the respondent experienced as an adult, how many times the violence occured, and the relationship between the respondent and the offender. The survey also included questions about cultural identity, involvement in the community, and the respondent's living conditions. Demographic variables include marital status, employment, income, and alcohol use. Questions were also included to gather respondents' opinions on health and social services delivery to Ahtna women in the Copper River region. The Detailed Physical Assault Incident Report (Part 2) includes variables about the victim/offender relationship, the time and place of the victimization, the amount of physical harm done in the victimization, whether alcohol or other drugs were involved in the victimization, whether formal assistance (i.e., police, medical treatment) was sought, the victim's perceptions of and satisfaction with the formal system response, the reasons for reporting or not reporting the offense, and if the victim attempted to obtain shelter from further victimization.
