The forms you need to apply for access to our restricted datasets.

NIJ and OJJDP Data

  • Sample NACJD Restricted Data Use Agreement for NIJ and OJJDP data (pdf) – A fully executed NACJD Restricted Data Use Agreement is required for all restricted data access applications. Although you are welcome to reference this form as a sample, please use the Data Use Agreement provided in the online application portal. You must also complete the appropriate appendix A through E according to the data access mechanism for the selected dataset.
  • NIJ Privacy Certificate for NACJD Applicants (pdf) – A fully executed NIJ Privacy Certificate is required for all applications that involve NIJ-sponsored restricted datasets. Because some NACJD data contain potentially disclosive information, responses of “N/A” are not acceptable. Detailed guidance and suggested responses for completing the privacy certificate are provided in an appendix near the end of the file.
  • Annual Report Template (docx) – Each year, researchers are required to submit an annual report listing any publications or presentations that used restricted data. A final report covering the same information must also be submitted upon termination of your project.
  • Request for Extension of RDUA (pdf) – If, at the end of the 24-month agreement, your project still requires access to Secure Download datasets, Principal Investigators must submit a request for an extension for 24 more months. If the IRB has expired, current documentation must be included. A new Restricted Data Use Agreement is required after six (6) cumulative years of data access under the initial agreement terms.
  • Affidavit of Data Destruction (pdf) – At the end of the 24-month agreement, or after the research project has terminated (whichever comes first), Principal Investigators must destroy all restricted data files and submit an Affidavit of Data Destruction via IDARS in accordance with your Data Use Agreement. VDE and PDE researchers are exempt from this requirement because data access is automatically terminated when agreements expire.
