This data collection constitutes a portion of the historical data collected by the project "Early Indicators of Later Work Levels, Disease, and Death." With the goal of constructing datasets suitable for longitudinal analyses of factors affecting the aging process, the project is collecting military, medical, and socioeconomical data on a sample of white males mustered into the Union Army during the Civil War. The project seeks to examine the influence of environmental and host factors prior to recruitment on the health performance and survival of recruits during military service, to identify and show relationships between socioeconomic and biomedical conditions (including nutritional status) of veterans at early ages and mortality rates from diseases at middle and late ages, and to study the effects of health and pensions on labor force participation rates of veterans at ages 65 and over. This installment of the collection, Version M-5, supersedes any previous version of these data. Collected in this version are data from military service, pension, and medical records of veterans who were originally mustered into the Union Army in California, Connecticut, Delaware, District of Columbia, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New Mexico, New York, Ohio, Pennsylvania, Vermont, West Virginia, and Wisconsin regiments. Also included are data from a 20-company pilot sample and information on recruits whose pension records were stored at the Veterans Administration (VA) Archives in Washington, DC, but had not been collected previously. Data include date and place of birth, place of residence, marital status, number of children, occupation, wealth and income, muster place and date, length of service, battles fought, medical experiences (e.g., illness, wounds, and hospital stays), health status, pension information, and date, place, and cause of death. Additional variables provide the place and date of birth of the recruits' wives, children, and parents. The data are organized into three sections according to state of enlistment. Section 1 (Parts 1, 2, 3, and 4) contains data from New England, Kansas, Missouri, Minnesota, Iowa, New Jersey, Indiana, Wisconsin, California, New Mexico, and the 20-company pilot sample. Section 2 (Parts 5, 6, 7, and 8) contains data from New York, Michigan, Washington, DC, Delaware, Kentucky, Maryland, and West Virginia, along with pensions data from the VA Archives. Section 3 (Parts 9, 10, 11, and 12) contains data from Ohio, Pennsylvania, and Illinois. The variables in Part 13, Linkage Data, indicate which major document sources were located for each recruit. Also, provided is information regarding death dates (Part 14) for individuals whose death records came from the pension payout cards. Approximate date of death was determined by examining the last record of payment to the pensioner.

This data collection, Aging of Veterans of the Union Army: Surgeons' Certificates, United States, 1862-1940, constitutes a portion of the historical data collected by the project "Early Indicators of Later Work Levels, Disease, and Death." This data collection is an epidemiological study of chronic disease in the oldest old. It is based on information collected from Kaiser Permanente facilities in Northern California. Members of Kaiser Permanente Medical Care programs who were aged 65 or older at the time the data were being collected and who had taken a multiphasic health checkup examination within four to six years of the baseline date were included in the study. There are several components to the dataset. Part 1, Master Records, includes information from the morbidity review, in which over 35 chronic conditions or diagnoses were abstracted from the member charts, as well as detailed diagnostic criteria for the major conditions. A prevalence review was done, which included the four years prior to the baseline date for these same conditions. Recurrent disease is included for the following conditions: cancers, myocardial infarction, and various forms of strokes. A detailed account of outpatient health services use, and data from the multiphasic health checkup, which was administered to each participant during the nine yearly follow-ups, are also included in the Master Records file. Part 2, Hospitalization, contains records of causes and dates of hospitalizations and discharges and nursing home admissions. There is also a section on incomplete reviews and the reasons for them. Demographic information and some lifestyle information from the multiphasic health checkup (e.g., smoking, alcohol, and Body Mass Index) are also in this file.

The Mortality Detail File: External Cause Extract is a special subset of data prepared from the MORTALITY DETAIL FILES, 1968-1991 (ICPSR 7632). Due to changes in Cause of Death definitions incorporated in the INTERNATIONAL CLASSIFICATION OF DISEASES, NINTH REVISION (ICD-9), the 1968-1978 data files differ slightly from the 1979-1980 data files. The 1968-1978 data reflect cause of death codes of the INTERNATIONAL CLASSIFICATION OF DISEASES ADAPTED FOR USE IN THE UNITED STATES, EIGHTH REVISION (ICDA-8). The period immediately following (1979-80) utilizes ICD-9 cause of death codes. In addition to the differences in the Cause of Death codes and recodes, the 1979-1980 data include three variables not available in the 1968-1978 datasets. These are: mortality by marital status, state or country of birth, and place of death and status of decedent when death occurred in a hospital or medical center. With these exceptions, the data are similar in structure and content to the 1968-1978 data and provide detailed personal and geographic information such as month and day of death, decedent's race and gender, age of deceased at time of death, place of decedent's residence (specific to the city level), place of death (specific to the county level), and whether an autopsy was performed. The 1979-1980 files also contain new variables pertinent to criminal justice research: handgun versus other gun accidents, handgun versus other firearm suicides, handgun versus other firearm homicides, and drug poison versus other poison homicides.

The National Health and Nutrition Examination Survey II: Mortality Study, 1992 (NH2MS) followed a subset of the participants in the second National Health and Nutrition Examination Survey (NHANES II). The NH2MS cohort is comprised of adult respondents to the NHANES II who were 30-75 years of age at the time of the NHANES II, and who were interviewed and examined for NHANES II. Some participants who were 74 years of age during their interview turned 75 years old between their interview and the examination. The NHANES series of data collections included standardized physical examinations, laboratory tests, and interviews that covered various health-related topics. The NH2MS was designed to investigate the association between factors measured at baseline and overall mortality or death from specific causes. The NH2MS involved searching national databases containing information about mortality and causes of death. The study was entirely passive: participants were not recontacted, nor were all death certificates obtained. Instead, mortality status was ascertained solely by computerized matching to national databases and evaluation of the resulting matches. Furthermore, no recontact is planned in the future. Rather, matching to the National Death Index (NDI) and to other national databases will continue on a periodic basis. The Mortality Status file (Part 1) contains mortality status and demographic data for all NH2MS participants who were 30-75 years of age at their NHANES II examination. The Cause of Death file (Part 2) contains one record for each known decedent for whom multiple causes of death were obtained (2,103 out of the 2,145 participants identified as deceased). Cause of death is missing for some decedents either because there was no matching record on the NCHS Multiple Cause of Death files or the death certificate was not found through the state vital statistics office.

This data file supplies date and cause of death data for sample persons included in the Supplement on Aging (SOA) to the NATIONAL HEALTH INTERVIEW SURVEY, 1984 (ICPSR 8659). It can also be used for the cohort included in the NATIONAL HEALTH INTERVIEW SURVEY: LONGITUDINAL STUDY OF AGING, 70 YEARS AND OVER, 1984-1989 (ICPSR 8719), a follow-up to the SOA. The file contains the date of death for 4,230 persons who died by the end of 1991. It also contains the cause of death for 2,515 persons who died by the end of 1989. Records of participants in the SOA were matched with the National Death Index only if the participant provided all the information after being told that it would be used for matching for 1984-1989. Of the 16,148 participants, 15,974 provided the information and their records were matched to the National Death Index for the years 1984-1989. Of those 15,974, a total of 3,113 matched either on all items (2,640) or on the critical items (473). Records of the 3,113 decedents were then matched with multiple cause of death files only for the years 1984-1989. This file contains the identification number for adding this information to the Supplement on Aging and the Longitudinal Study of Aging files. Also supplied are the underlying cause of death (up to eight multiple causes of death), whether an autopsy was performed, the place of death, and codes for the occupation and industry of the deceased, as reported on the death certificate.

This data collection supplies date and cause of death data for sample persons included in the National Health Interview Surveys (NHIS) for the years 1986 through 1994. Beginning with survey year 1986, linkage information was collected on NHIS respondents aged 18 and older to allow for matching with other data systems such as the National Death Index (NDI). The Multiple Cause of Death (MCD) data files contain information on those persons with scores high enough to be considered deceased or scores high enough that they may be included in an analysis as deceased. The Ineligible Cases data files contain person IDs of those NHIS participants under the age of 18 as well as those with insufficient information to permit linkage with the NDI. These cases should be excluded from the NHIS survey data files prior to analysis. Linkage of the NHIS respondents with the NDI provides a longitudinal component to the NHIS that allows for the ascertainment of vital status. The addition of vital status permits the use of NHIS data to estimate survival, mortality, and life expectancy while using the richness of the NHIS questionnaires, both core and supplements, as covariates. These data files must be used in conjunction with the basic NHIS data files (1986 [ICPSR 8976], 1987 [ICPSR 9195], 1988 [ICPSR 9412], 1989 [ICPSR 9583], 1990 [ICPSR 9839], 1991 [ICPSR 6049], 1992 [ICPSR 6343], 1993 [ICPSR 6534], 1994 [ICPSR 6724]). The National Longitudinal Mortality Study (NLMS) consists of a database developed for the purpose of studying the effects of demographic and socio-economic characteristics on differentials in United States mortality rates. The NLMS database is based on a random sample of the non-institutionalized population of the United States. It consists of United States Census Bureau data from Current Population Surveys, annual Social and Economic Supplements and the 1980 Census combined with death certificate information to identify mortality status and cause of death.

This survey was designed primarily to obtain information on the smoking habits of decedents by examining death certificates and questionnaires mailed to death record informants. Smoking variables in this data collection include number of cigarettes smoked when the decedent smoked most, number smoked the year before death, number smoked three years before death, and cigar and pipe smoking occurrence three years before death. Demographic variables include marital status, family type, number of children, living arrangements, size of family, birth and death of the decedent, family income and family debt, and cause of death.

The 1986 National Mortality Followback Survey (NMFS) is the first National Center for Health Statistics (NCHS) mortality followback study since the 1966-1968 survey of the same name (ICPSR 8370). The 1986 NMFS supplements characteristics of mortality found in the routine vital statistics system by collecting information from death certificate informants or other knowledgeable relatives, and from health care facilities that were used by decedents in the last year of life. The death records provide demographic data on the decedent and information on the circumstances of death (location, time, underlying causes, and other health conditions at time of death) and use of medical facilities in the preceding year. Additional issues addressed in the informant questionnaire were health care sought and provided in the last year of life, risk factors associated with premature death, socioeconomic status and mortality, and reliability of selected items reported on the death certificate. Health care facilities provided information on diagnosis, diagnostic and surgical procedures performed on the decedent, and length of stay.

The National Mortality Followback Survey (NMFS) Program, begun in the 1960s by the National Center for Health Statistics (NCHS), uses a sample of United States residents who die in a given year, supplementing information derived from the death certificate with information from the next of kin or another person familiar with the decedent's life history. This information, sometimes enhanced by administrative records, is collected in order to study the etiology of disease, demographic trends in mortality, and other health issues. The 1993 National Mortality Followback Survey (NMFS) sampled individuals aged 15 years and over who died in 1993. Forty-nine of the 50 state vital registration areas, as well as the independent vital registration areas of the District of Columbia and New York City, granted approval to sample their death certificates. (South Dakota declined to participate due to a state law restricting the use of death certificate information.) A sample of 22,957 death certificates from 1993 was then drawn. To obtain reliable numbers for important population subgroups, such as persons under age 35, women, and the Black population, death certificates from those subgroups were oversampled. The 1993 NMFS survey focused on five subject areas: (1) socioeconomic differentials in mortality, (2) associations between risk factors and cause of death (use of tobacco, alcohol, drugs, firearms, motor vehicles), (3) disability (medical condition and cognitive functioning during the last year of life), (4) access and utilization of health care facilities during the last year of life (number of doctor visits, days bedridden, nursing home experiences, use of assistive medical devices, availability of health insurance), and (5) reliability of certain items reported on the death certificate. Demographic variables include age, gender, race, marital status, birthplace, education, occupation and industry, and income and assets. The 1993 NMFS survey differed from the previous mortality followback surveys in several ways: First, it emphasized deaths due to homicide, suicide, and unintentional injury. Second, the subject areas were considerably broader (many previously-surveyed subject areas, however, are included for trend analysis). This survey was also the first to acquire national-level information from medical examiners and coroners. Finally, the complexity of the questionnaire necessitated telephone or in-person interviews. The 1993 NMFS was designed in collaboration with other agencies of the Public Health Service, Department of Health and Human Services, and the National Highway Traffic Safety Administration.

National Nursing Home Survey Follow-up: Mortality Data, 1984-1990 (ICPSR 6435)

The NATIONAL NURSING HOME SURVEY FOLLOW-UP (NNHSF) (WAVE I, 1987 [ICPSR 9813], WAVE II, 1988 [ICPSR 9838], and WAVE III, 1990 [ICPSR 6142]) is a longitudinal study that followed the cohort of current and discharged residents sampled in the NATIONAL NURSING HOME SURVEY, 1985 (NNHS) (ICPSR 8914). To generate the mortality data for this collection, 12,348 NNHSF records were matched to the National Death Index (NDI) to determine whether the NNHSF subjects who were alive at the last contact had died or if dates of death obtained during the follow-up waves were accurate. Of the 12,348 records sent to NDI for matching, 6,507 matches were found to be true or highly probable matches based on a scoring algorithm developed to determine the quality of the NDI match. These 6,507 records were then matched to the National Center for Health Statistics' Multiple Cause of Death files. Mortality data variables include the residence, marital status, education, race, and age of the decedent and the place and date of death, as well as the underlying cause of death and multiple conditions of death.

This study provides mortality data for respondents to the TECUMSEH COMMUNITY HEALTH STUDY, 1959-1969 (ICPSR 8969). The seven variables in the data collection include an identifier to link the mortality data with the core data file, the contact month and year (for respondents who were alive during the 1978 ascertainment period), respondents' mortality status (alive, dead, or unknown), the month and year of death, and the cause of death (International Classification of Diseases, 8th Revision codes). These mortality follow-up data were recorded for the 8,637 white respondents to the original data collection.
