This data collection constitutes a portion of the historical data collected by the project "Early Indicators of Later Work Levels, Disease, and Death." With the goal of constructing datasets suitable for longitudinal analyses of factors affecting the aging process, the project is collecting military, medical, and socioeconomical data on a sample of white males mustered into the Union Army during the Civil War. The project seeks to examine the influence of environmental and host factors prior to recruitment on the health performance and survival of recruits during military service, to identify and show relationships between socioeconomic and biomedical conditions (including nutritional status) of veterans at early ages and mortality rates from diseases at middle and late ages, and to study the effects of health and pensions on labor force participation rates of veterans at ages 65 and over. This installment of the collection, Version M-5, supersedes any previous version of these data. Collected in this version are data from military service, pension, and medical records of veterans who were originally mustered into the Union Army in California, Connecticut, Delaware, District of Columbia, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New Mexico, New York, Ohio, Pennsylvania, Vermont, West Virginia, and Wisconsin regiments. Also included are data from a 20-company pilot sample and information on recruits whose pension records were stored at the Veterans Administration (VA) Archives in Washington, DC, but had not been collected previously. Data include date and place of birth, place of residence, marital status, number of children, occupation, wealth and income, muster place and date, length of service, battles fought, medical experiences (e.g., illness, wounds, and hospital stays), health status, pension information, and date, place, and cause of death. Additional variables provide the place and date of birth of the recruits' wives, children, and parents. The data are organized into three sections according to state of enlistment. Section 1 (Parts 1, 2, 3, and 4) contains data from New England, Kansas, Missouri, Minnesota, Iowa, New Jersey, Indiana, Wisconsin, California, New Mexico, and the 20-company pilot sample. Section 2 (Parts 5, 6, 7, and 8) contains data from New York, Michigan, Washington, DC, Delaware, Kentucky, Maryland, and West Virginia, along with pensions data from the VA Archives. Section 3 (Parts 9, 10, 11, and 12) contains data from Ohio, Pennsylvania, and Illinois. The variables in Part 13, Linkage Data, indicate which major document sources were located for each recruit. Also, provided is information regarding death dates (Part 14) for individuals whose death records came from the pension payout cards. Approximate date of death was determined by examining the last record of payment to the pensioner.

Electronic health records, or EHRs, have information about a patient's health such as test results, diagnoses, and treatments. EHRs also have clinical notes that doctors and patients can use to track goals and decisions.

Clinical notes may be useful for research or to help improve care. But it's hard to get information from these notes across large groups of patients. The notes may use different ways to describe the same thing. For example, high blood pressure may be called hypertension. Also, the notes may use abbreviations or have spelling mistakes.

In this project, the research team designed and built a search engine to make EHR notes easier to search and use for patient care and research.

The Healthy Steps for Young Children program began in 1995 as a new approach to primary health care for young children, birth to age three. The program is intended to enhance early pediatric care by incorporating preventive developmental and behavioral services as part of a comprehensive, whole-child, whole-family model of health care and to help provide mothers and fathers with the childrearing information and guidance they seek.

The evaluation of Healthy Steps consisted of three components: the National Evaluation, the Affiliate Evaluation, and the Embedded Observational Study. All data contained in these public release data sets come from the National Evaluation. These data were gathered to assess whether the Healthy Steps program was successful in reorienting pediatric practice to emphasize child development issues in increasing parents' knowledge about early nurturing of infants and parents' involvement in their children's development and in promoting parents' practices that improve the health, safety, and health care utilization of their children.

The data are organized as follows:

  • Parent Forms: Newborn

    The newborn form was used to gather data on the baby's characteristics, demographic characteristics of the mother, father, and family, prenatal utilization of services, health behaviors of the mother and father, and parents' decisions about a pediatric provider for their newborn.

  • Parent Forms: 6 Month

    The 6 month form was used to gather information on selected family demographic characteristics, child's health, parenting practices, and health behaviors of the mother and father. Questions included the frequency of injuries, emergency department visits, and hospitalization in the past 6 months, use of safety devices, activities that promote learning and development, sources of information on speech development, child care arrangements, smoking practices, and mother's receipt of postpartum care.

  • Parent Forms: 12 Month

    Like the 6 month form, the 12 month form was used to gather information on selected family demographic characteristics, child's health, parenting practices, and health behaviors of the mother and father. Questions included the frequency of injuries, emergency department visits, and hospitalization in the past 12 months, use of safety devices, activities that promote learning and development, sources of information on speech development, child care arrangements, smoking practices, and mother's receipt of postpartum care.

  • Medical Record Abstraction: Vaccinations

    This file contains demographic and vaccination data, including gender, race/ethnicity, and insurance provider.

  • Medical Record Abstraction: Medical Visits

    This file contains data abstracted from forms completed for every visit recorded in the medical record or other primary care files, including type visit and whether a physical assessment was conducted.

  • Medical Record Abstraction: Referrals/Consultations

    These data are limited to information on any referrals or consultations noted in the child's medical record or other primary care files, including type of and reason for the referral.

  • Medical Record Abstraction: Hospitalizations/ED Visits

    These data are limited to information on any hospitalization, emergency department visit, or urgent care visit recorded in the child's medical record or other primary care files, including the type of and reason for the visit.

  • Healthy Steps Specialist Contact Logs

    Data contained in this file represent every interaction between Healthy Steps Specialists and the family, including home visits, office visits, telephone calls to or from the family, parent groups, mailings, and other types of contacts, such as hospital visits. Information collected on each contact included the date of contact, type of contact, person contacted, status of the contact, reason for the contact, whether a handout was given out or a referral made during the contact, the issues/ problems discussed during the contact, and any action taken by the Healthy Steps Specialist.

  • Parent Interviews: 2-4 Month

    The 2-4 month interview included questions about the respondent's knowledge of child development, his/her sense of competence about childrearing, his/her perception of support for childrearing activities from both formal and informal sources, and his/her engagement in activities with the child that promote health, learning, and development. The interview also gathered information on the socio- demographic characteristics of the family, including the mother's and father's education, marital status, employment, income, and household composition.

  • Parent Interviews: 30-33 Month

    The 30-33 month interview is the source of data on parent and child outcomes and assessed the extent to which families received the Healthy Steps intervention. Specifically, parents were asked about utilization of health-related services and about the child's health and progress in reaching age-appropriate developmental milestones, concerns the parent had about the child's development or behavior, and whether the child was referred to services for a behavior or developmentally-related problem. Additional questions addressed parenting activities that promote development, family routines, engagement in safety activities, the mother's general health status, use of substances, use of mental health services, and use of the child's doctor or her obstetrician/gynecologist as a source for discussing problems with depression and use of preventive health care.

Improving the Forensic Documentation of Injuries through Alternate Light: A Researcher- Practitioner Partnership, Maryland, 2021-2023 (ICPSR 39024)

This study assessed and evaluated the programmatic implementation of alternate light source (ALS) use during the examination of injuries among adult victims of sexual violence and intimate partner violence (IPV) in a hospital setting. Researchers selected two forensic nursing departments, one with no experience using ALS in clinical practice (Site A) and one with 10+ years' experience (Site B), and conducted a thorough contextual analysis of each site to 1. develop an evidence-based ALS implementation program for forensic nursing departments and 2. evaluate the feasibility of this ALS implementation program.

Contextual analysis involved the collection of qualitative data through structured focus groups with nurses and quantitative data using 6-months of de-identified forensic medical records from patients who received a medical forensic exam for reported (or suspected) sexual assault or IPV. Nurses were recruited to the study upon completing an anonymous survey about their prior knowledge of ALS, qualifications or certifications related to ALS, workplace team dynamics, and organizational support for changes in practice using the Organizational Readiness to Change Assessment [ORCA]. This Nurse Survey Data and aforementioned Medical Record Data are currently available for secondary users, and qualitative interview transcripts will be made available in a future update.

The Drug Abuse Warning Network (DAWN) is a nationally representative public health surveillance system that has monitored drug related emergency department (ED) visits to hospitals since the early 1970s. To be eligible for selection into the DAWN sample, a hospital must be a non-federal, short-stay, general surgical and medical hospital located in the United States, with at least one 24-hour ED. DAWN cases are identified by the systematic review of ED medical records in participating hospitals. The unit of analysis is any ED visit involving recent drug use. DAWN captures both ED visits that are directly caused by drugs and those in which drugs are a contributing factor but not the direct cause of the ED visit. The reason a patient used a drug is not part of the criteria for considering a visit to be drug related. Therefore, all types of drug-related events are included: drug misuse or abuse, accidental drug ingestion, drug-related suicide attempts, malicious drug poisonings, and adverse reactions. DAWN does not report medications that are unrelated to the visit.

The DAWN public-use dataset provides information for all types of drugs, including illegal drugs, prescription drugs, over-the-counter medications, dietary supplements, anesthetic gases, substances that have psychoactive effects when inhaled, alcohol when used in combination with other drugs (all ages), and alcohol alone (only for patients aged 20 or younger). Public-use dataset variables describe and categorize up to 16 drugs contributing to the ED visit, including toxicology confirmation and route of administration. Administrative variables specify the type of case, case disposition, categorized episode time of day, and quarter of year. Metropolitan area is included for represented metropolitan areas. Created variables include the number of unique drugs reported and case-level indicators for alcohol, non-alcohol illicit substances, any pharmaceutical, non-medical use of pharmaceuticals, and all misuse and abuse of drugs. The DAWN public-use dataset provides information for all types of drugs, including illegal drugs, prescription drugs, over-the-counter medications, dietary supplements, anesthetic gases, substances that have psychoactive effects when inhaled, alcohol when used in combination with other drugs (all ages), and alcohol alone (only for patients aged 20 or younger). Public-use dataset variables describe and categorize up to 22 drugs contributing to the ED visit, including toxicology confirmation and route of administration. Administrative variables specify the type of case, case disposition, categorized episode time of day, and quarter of year. Metropolitan area is included for represented metropolitan areas. Created variables include the number of unique drugs reported and case-level indicators for alcohol, non-alcohol illicit substances, any pharmaceutical, non-medical use of pharmaceuticals, and all misuse and abuse of drugs. Birth Outcomes of Second Children after Community-Based Home Visiting (ICPSR 156962)

The purpose of the original research project was to evaluate birth outcomes of second children after community-based home visiting that targeted first-time parents. Outcomes include gestational age, birthweight, pregnancy spacing, cesarean sections, and timing of prenatal care initiation. Birth certificate data were used to create comparison groups for this observational study. Birth outcomes for the NFN second children were compared to: (a) their older siblings (the children whose birth led to enrollment in NFN), (b) a propensity-score-matched group of non-NFN children, and (c) children of families who were screened as eligible for NFN, but not offered the program due to home visitor availability and other logistical reasons. Due to the small size of the group and substantial differences in baseline characteristics, this last group (c: eligible but unenrolled) are not included in this dataset. The primary data sources include NFN program data and CT Birth Certificate data for 2005-2015. Access to the NFN dataset came in partnership with the University of Hartford Center for Social Research. The Connecticut Department of Public Health provided access to the CT Birth Certificate data. Data were collected through administrative and medical records. Birth certificates providing birth outcome information were linked to NFN data based on multiple identifying variables.The final dataset was created in Stata and provided here in Stata, SPSS, R, and .csv formats.

The main objective of this pragmatic randomized clinical trial (PCT) is to identify the optimal dose of aspirin for secondary prevention in atherosclerotic cardiovascular disease (ASCVD). A total of 15,076 high-risk patients with a history of a myocardial infarction (MI) or documented ASCVD were randomized in a 1:1 ratio to receive 81 mgs versus 325 mgs of aspirin every day. This trial addressed the following specific aims:

  1. To compare the effectiveness of two doses of aspirin (81 mg and 325 mg) in reducing a composite of all-cause mortality and hospitalization for nonfatal MI, or nonfatal stroke, and the primary safety endpoint of major bleeding. Secondary endpoints include the components of the primary endpoint and hospitalization for transient ischemic attack, unstable angina, or coronary revascularization procedures.
  2. To compare the effects of aspirin in selected subgroups of patients by sex, age, race, Internet users vs. non-users, and those with diabetes or advanced chronic kidney disease (CKD).
  3. To develop and refine the infrastructure for PCORnet to conduct multiple comparative effectiveness trials in the future.
  4. To explore biological mediators of heterogeneity of response to aspirin and of impact on clinical events.

This data collection is a portion of the historical data collected by the project, "Early Indicators of Later Work Levels, Disease, and Death," which is collecting military, medical, and socioeconomic data on a sample of white males mustered into the Union Army during the Civil War. During 1850, 1860, and 1870, mortality information was gathered at the county level as an addendum to the population census. These data examine the impact of environmental factors on life outcomes and look at the influence of infectious disease rates on economic and health patterns at late ages. Part 1, Disease Data, looks at cause of death from 66 disease classifications. Part 2, General Disease Data, also examines cause of death but through 18 broad disease categories. Variables included in both parts are state, county, year of death, and frequency of death by disease.

The Miami-Dade County Medical Examiner Department (MDME) successfully conducted preliminary studies that evaluated the effectiveness of fast scanning liquid chromatography-tandem mass spectrometry (LC/MS/MS) which created a detailed merged-product ion scan spectra with library search capabilities to identify substances more accurately. The project intent was to establish its effectiveness in replacing more traditional immunoassay testing procedures that are costly, of limited scope, non-specific, and can only provide presumptive results. The goal was to develop a superior screening protocol that combines fast scanning LC/MS/MS technology with the currently utilized gas chromatography mass spectrometry (GC/MS) screening technique. By merging these two analytical tools, GC/MS and LC/MS/MS, a broader range of drugs can be identified more efficiently and at appropriate drug concentrations for postmortem analysis.

In 2011-2014, the MIDUS recruited a national probability sample of 3,577 adults, aged 25 to 74, designed to replenish the original MIDUS 1 (M1) baseline cohort and paralleling the five decadal age groups of the M1 baseline survey (ICPSR 2760).

The MIDUS Refresher (MR1) survey included the same comprehensive assessments as those assembled on the original MIDUS Core sample (obtained via interview and self-administered questionnaire), but with additional questions about the effect of the economic recession of 2008-09.

In 2012-2013, the National MIDUS Refresher sample was augmented with a sample of 508 Milwaukee African American adults, aged 25 to 64. This sample was designed to increase the number of racial minorities included in the MIDUS study. The MIDUS Milwaukee Refresher survey also included the same assessments as those in the Milwaukee Core sample (ICPSR 22840) and employed a personal interview. This dataset includes all known decedents from the National (ICPSR 36532) and Milwaukee (ICPSR 36722) Refresher samples, and is current for NDI records through December, 2023.

Many adults have multiple chronic conditions and medical care is often fragmented despite the recognized need to develop integrated guidelines. We describe a cohort of 44,645 patients (mean age 72) with an average of 6 chronic conditions determined from retrospective medical records review. Agglomerative hierarchical clustering methods are used to identify thirteen homogenous groups of patients with regards to chronic conditions.Identifying groups of patients with similar profiles of chronic conditions can provide a framework in which to develop efficient, comprehensive clinical management guidelines.

In 1958, the Medical Follow-up Agency (MFUA) of the Institute of Medicine began a project to identify twins who had jointly entered military service during World War II. In the end, MFUA identified nearly 16,000 White male twin pairs born 1917-1927 in which both members had served in the military. These twins comprise the National Academy of Sciences-National Research Council World War II Twin Registry (NAS-NRC Twin Registry). This collection represents data from service records, a mailed questionnaire assessing zygosity, and repeating health surveys, including information on education, employment history, and earnings.

There are nine datasets associated with this restricted-use collection:

1) The Administrative dataset includes demographic, zygosity, service history, mortality, and questionnaire participation data;

2) The Service and Other Records dataset contains information collected from service records, physical exam data, cognitive test data, and dental records;

3) The Questionnaire 2 dataset consists of data collected in the first mailed questionnaire sent in 1965 about pain, illnesses, smoking habits, alcohol consumption, and employment;

4) The Questionnaire 3 dataset includes data from the baseline epidemiological questionnaire sent in 1974 about number and sex of children, religious attendance, education, income, and occupation;

5) Questionnaire 7, mailed in 1985, contains similar topics as in Questionnaire 2, and includes data about health conditions such as diabetes, as well as feelings about work and retirement;

The National Home and Hospice Care Survey (NHHCS) was reintroduced into the field in 2007 after a 7-year break. During that time, the survey was redesigned and expanded to include a computer-assisted personal interviewing (CAPI) system, many new data items, and larger sample sizes of current home health patients and hospice discharges. All agencies that participated in the survey were either certified by Medicare and/or Medicaid or were licensed by a state to provide home health and/or hospice services and currently or recently served home health and/or hospice patients. Agencies that provided only homemaker services or housekeeping services, assistance with instrumental activities of daily living (IADLs), or durable medical equipment and supplies were excluded from the survey. The 2007 NHHCS included a supplemental survey of home health aides employed by home health and/or hospice agencies, called the National Home Health Aide Survey (NHHAS). The 2007 NHHCS data were collected through in-person interviews with agency directors and their designated staffs; no interviews were conducted directly with patients or their families and/or friends. Agency data collected, available in agency administrative records, included information on the year an agency was established, the types of services an agency provided, referral sources, specialty programs, and staffing characteristics. Data collected on home health patients and hospice discharges, available in medical records, included age, sex, race and ethnicity, services received, length of time since admission, diagnoses, medications taken, advance directives, and many other items.

The National Home Health Aide Survey (NHHAS), the first national probability survey of home health aides, was designed to provide national estimates of home health aides employed by agencies that provide home health and/or hospice care. The NHHAS survey instrument included sections on recruitment, training, job history, family life, management and supervision, client relations, organizational commitment and job satisfaction, workplace environment, work-related injuries, and demographics.

This study examines AI adoption in US hospitals using three distinct datasets: (i) Survey data from the American Hospital Association on AI for operations-related uses (27% adopt), (ii) Employment data from Revelio Labs on workers at hospitals with AI skills (14% adopt), and (iii) Publication data from Dimensions on hospital-affiliated researcher publications (8% adopt). Consistent with adoption patterns for the business internet and for electronic medical records, AI adoption is higher in metro areas and in larger hospitals. In contrast to the business internet, metro area and firm size do not appear to be substitute correlates with adoption.

The Life-course Experiences And Pregnancy (LEAP) study is a retrospective cohort study of reproductive and perinatal health among 977 women participating since adolescence in an ongoing longitudinal study of eating, activity, and weight-related health (Project EAT: Eating and Activity over Time). For Project EAT, male and female adolescents aged 11-18 were recruited during the 1998-1999 school year from 31 public middle and high schools in the greater Minneapolis-St. Paul area of Minnesota. Participants were subsequently surveyed every 5 years, with the most recent Project EAT survey completed in 2014-2015 when participants were aged 25-36. Project EAT surveys have collected data on a wide range of measures including depressive symptoms, unhealthy weight control behaviors, binge eating, home food availability, physical activity, and body weight. In 2019, participants in Project EAT who identified as women were recruited to complete the LEAP survey if they had responded to at least two of the three most recent Project EAT surveys and resided in the United States. The LEAP survey asked participants about their reproductive histories, including whether they had ever attempted to get pregnant, whether they had been pregnant, and how many of their pregnancies resulted in a live birth. Women who reported having at least one live birth were asked, for each birth reported: their age when they gave birth; the gestation length of the pregnancy; any diagnosed pregnancy complications; and their best estimates of what they weighed just before the pregnancy and how much weight they gained during the pregnancy. In addition, for their first live birth only, women were asked about a range of psychosocial and behavioral characteristics during the pregnancy, including their perceived ability to live on their income, depressive symptoms, binge eating, and unhealthy weight control behaviors, among others. Women who had never been pregnant were asked if they had ever attempted to get pregnant and, if so, if they had received a diagnosis of infertility and/or sought fertility treatment. Self-reports of key pregnancy characteristics for each woman’s live births were validated against medical record data in a validation substudy (n=250). All participants who reported at least one live birth on the LEAP survey (n=656) were invited to sign HIPAA Authorization and medical records release forms authorizing the study team to request their prenatal, delivery, and postpartum medical records for each of their pregnancies. Data on pre-pregnancy weight, measured weights at each prenatal visit, delivery weight, gestational age at delivery, and diagnoses of pregnancy complications were abstracted from the data.

The impact of criminal victimization on the health status of women is the focus of this data collection. The researchers examined the extent to which victimized women differed from nonvictimized women in terms of their physical and psychological well-being and differences in their use of medical services. The sample was drawn from female members of a health maintenance plan at a worksite in Cleveland, Ohio. Questions used to measure criminal victimization were taken from the National Crime Survey and focused on purse snatching, home burglary, attempted robbery, robbery with force, threatened assault, and assault. In addition, specific questions concerning rape and attempted rape were developed for the study. Health status was assessed by using a number of instruments, including the Cornell Medical Index, the Mental Health Index, and the RAND Corporation test battery for their Health Insurance Experiment. Medical service usage was assessed by reference to medical records. Demographic information includes age, race, income, and education.

Participants in this study were recruited between July 2013 and April 2014. Patients in the waiting rooms of the primary care clinic at the Center for Outpatient Health (COH) were approached by trained data collectors and asked to complete a survey in English.  Participants were asked to complete a self-administered written questionnaire and a verbally administered survey component.  The latter component assessed health literacy and was administered by a trained data collector who recorded responses. All participants completed a verbal consent process and signed a written consent form before completing the survey. As part of the consent process, participants could opt-in to have information abstracted from their medical records and merged with survey data.  The Human Research Protection Office at Washington University School of Medicine approved this study. Data collection Participants in this study were recruited between July 2013 and April 2014. Patients in the waiting rooms of the COH were approached by trained data collectors and asked to complete a survey in English. Surveys were administered on different days of the week and at different times of day; data collectors approached all patients in the waiting room during their shifts. Inclusion criteria were that patients be at least 18 years old, a patient at the COH, and speak English.  Participants were asked to complete a self-administered written questionnaire and a verbally administered survey component.  The latter component assessed health literacy and was administered by a trained data collector who recorded responses. All participants completed a verbal consent process and signed a written consent form before completing the survey. As part of the consent process, participants could opt-in to have information abstracted from their medical records and merged with survey data.  The Human Research Protection Office at Washington University School of Medicine approved this study. Approximately 26% (n=1,111) of those approached were ineligible to participate because they were not patients or had previously taken the survey. Among eligible participants, 44% (n=1380) agreed to participate in the study and were consented by trained data collectors. Of the 1380 consented patients, 1010 (73%) completed the written survey. Among those with complete written surveys, 602 (60%) completed the verbally administered component, and 781 (77%) opted in to have data pulled from their electronic medical record. The majority of patients are female (67%), between 35-64 years of age (59%), and live in St. Louis City (46%) or St. Louis County (31%).We propose and validate (against the gold standard written measure) a novel racial segregation measure, the Pictorial Racial Composition Measure (PRCM), which is a 10-item questionnaire of pictures representing social environments across adolescence and adulthood: neighborhoods and blocks (adolescent and current), schools and classrooms (junior high and high school), workplace, and place of worship.This data deposit contains the analytic sample for this analysis and does not contain the full sample that was collected. This data collection, Aging of Veterans of the Union Army: Surgeons' Certificates, United States, 1862-1940, constitutes a portion of the historical data collected by the project "Early Indicators of Later Work Levels, Disease, and Death." With the goal of constructing datasets suitable for longitudinal analyses of factors affecting the aging process, the project collects military, medical, and socioeconomic data on a sample of white males mustered into the Union Army during the Civil War. The surgeons' certificates contain information from examining physicians to determine eligibility for pension benefits. Also included are questions regarding the age, occupation, residence, and military experience of the veterans. Replication data for: Medical Care Spending and Labor Market Outcomes: Evidence from Workers' Compensation Reforms (ICPSR 113074)

Medical care represents an important component of workers' compensation benefits with the potential to improve health and post- injury labor outcomes, but little is known about the relationship between medical care spending and the labor outcomes of injured workers. We exploit the 2003–2004 California workers' compensation reforms which reduced medical spending disproportionately for workers incurring low back injuries. We link administrative claims data to earnings records for injured workers and their uninjured coworkers. We find that workers with low back injuries experienced a 7.6% post-reform decline in medical care, and an 8.1% drop in post-injury earnings relative to other injured workers.

Cuyahoga County, Ohio, Heroin and Crime Initiative: Informing the Investigation and Prosecution of Heroin-Related Overdose, 2012-2021 (ICPSR 38295)

In 2013, the Cuyahoga County (Ohio) Medical Examiner's Office (CCMEO) and the Regional Forensic Science Laboratory developed the Heroin Involved Death Investigation (HIDI) alert system and protocol in response to a substantial increase in opioid-related overdose fatalities. The HIDI protocol is designed to support a safe, coordinated, and rapid response to an active, suspected opioid-overdose death scene, or suspected opioid-overdose deaths occurring at hospitals that are not considered active scenes, by alerting investigators to potential dangers and facilitating the timely protection of scene integrity and evidence collection in order to successfully investigate and prosecute drug traffickers.

The primary goals of the project were to:

  1. Complete extended coding of local medical examiner decedent data--investigative reports and toxicology to identify demographic or geographic trends or patterns of overdose deaths, as well as paraphernalia and evidence present at death scenes that may be useful to prosecutions;
  2. Examine the efficiency of how cases flow through the investigative and prosecutorial stages and how these could be improved;
  3. Identify key variables that may contribute to the successful indictment of traffickers connected to fatal and non-fatal overdose cases; and
  4. Evaluate the implementation and perceived effectiveness of the Cuyahoga County HIDI protocol.

This multi-method project involved three phases of data collection and analysis. First, a forensic epidemiologist coded and analyzed existing CCMEO records for decedent toxicology and death scene characteristics, focusing on drug-related fatalities. Second, county and federal cases prosecuted for drug trafficking, especially those linked to deaths, were systematically reviewed to determine what evidence was deemed important for successful indictment. Third, interviews and focus groups were conducted with key stakeholders from local and federal law enforcement, intelligence analysts, public health officials, and local and federal prosecutors to learn about the HIDI protocol.

Data and documentation for interviews and focus groups will be made available in a future update.

The 2009 survey contains information from 32,281 patient visits to 1,293 physicians' offices. Data are available on the patient's smoking habits, reason for the visit, expected source of payment, the physician's diagnosis, and the kinds of diagnostic and therapeutic services rendered. Other variables include information on the following: drugs/medications ordered, administered, or provided during office visits; information on medication code, generic name and code, brand name, entry status, prescription status, federal controlled substance status, composition status, and related ingredient codes. Evaluating Medicaid Access for Halfway House Residents: A Research Partnership with the Connecticut Department of Correction, 2013-2017 (ICPSR 37580)

The goals of this study were to examine how providing Medicaid coverage for halfway house residents may affect care seeking, improve health care usage, and decrease criminal recidivism relative to providing health care through prison or jail medical facilities. To achieve these goals, we developed a researcher-practitioner partnership with the Connecticut Department of Correction (DOC) to implement a mixed-methods research design.

Qualitative data collection included focus groups with halfway house residents; interviews with halfway house staff, correctional health providers, correctional officers, and state-level DOC officials; and site observations of DOC medical facilities. Quantitative data collection included collection of administrative and recidivism data from DOC, coding of study participants' DOC medical charts to measure their baseline health status and health care usage in prison or jail, and collection of Medicaid enrollment and claims data from Connecticut's Department of Social Services (DSS) to measure Medicaid enrollment and health care usage in the community.
