Benefits of Stroke Treatment Delivered Using a Mobile Stroke Unit Compared to Standard Management by Emergency Medical Services (BEST-MSU Study), United States, 2014-2022 (ICPSR 39547)

The standard care of hospital or emergency department patients experiencing an acute ischemic stroke includes intravenously administered tissue plasminogen activator (tPA). Mobile stroke units (MSUs) are ambulances equipped with staff and a computed tomographic scanner that can allow for tPA to be administered more quickly. This comparative effectiveness trial examined clinical outcomes in stroke patients who received either an earlier diagnosis and treatment using an MSU or standard triage and transport by Emergency Medical Services (EMS). The sample included multicenter cohorts with randomized deployment weeks and blinded assessment of both trial entry and clinical outcomes.

The Study of Women's Health Across the Nation (SWAN), is a multisite longitudinal, epidemiologic study designed to examine the health of women during their middle years. The study examines the physical, biological, psychological, and social changes during this transitional period. The goal of SWAN's research is to help scientists, health care providers, and women learn how mid-life experiences affect health and quality of life during aging. The data include questions about doctor visits, medical conditions, medications, treatments, medical procedures, relationships, smoking, and menopause related information such as age at pre-, peri- and post-menopause, self-attitudes, feelings, and common physical problems associated with menopause. The study began in 1994. Between 2001 and 2003, 2,617 of the 3,302 women that joined SWAN were seen for their fifth follow-up visit. Midlife in the United States (MIDUS 2): Biomarker Project, 2004-2009 (ICPSR 29282)

The Biomarker study is Project 4 of the Midlife in the United States (MIDUS) longitudinal study, a national survey of more than 7,000 Americans (aged 25 to 74) begun in 1994. The purpose of the larger study was to investigate the role of behavioral, psychological, and social factors in understanding age-related differences in physical and mental health. With support from the National Institute on Aging (NIA), a longitudinal follow-up of the original MIDUS samples [core sample (N = 3,487), metropolitan over-samples (N = 757), twins (N = 957 pairs), and siblings (N = 950)] was conducted in 2004-2006. Guiding hypotheses, at the most general level, were that behavioral and psychosocial factors are consequential for health (physical and mental). A description of the study and findings from it are available on the MIDUS website.

The Biomarker Project (Project 4) of MIDUS 2 contains data from 1,255 respondents. These respondents include two distinct subsamples, all of whom completed the Project 1 Survey: (1) longitudinal survey sample (n = 1,054) and (2) Milwaukee sample (n = 201). The Milwaukee group contained individuals who participated in the baseline MIDUS Milwaukee study, initiated in 2005. The purpose of the Biomarker Project (Project 4) was to add comprehensive biological assessments on a subsample of MIDUS respondents, thus facilitating analyses that integrate behavioral and psychosocial factors with biology. The broad aim is to identify biopsychosocial pathways that contribute to diverse health outcomes. A further theme is to investigate protective roles that behavioral and psychosocial factors have in delaying morbidity and mortality, or in fostering resilience and recovery from health challenges once they occur. The research was not disease-specific, given that psychosocial factors have relevance across multiple health endpoints.

Biomarker data collection was carried out at three General Clinical Research Centers (at UCLA, University of Wisconsin, and Georgetown University). The biomarkers reflect functioning of the hypothalamic-pituitary-adrenal axis, the autonomic nervous system, the immune system, cardiovascular system, musculoskeletal system, antioxidants, and metabolic processes. Our specimens (fasting blood draw, 12-hour urine, saliva) allow for assessment of multiple indicators within these major systems. The protocol also included assessments by clinicians or trained staff, including vital signs, morphology, functional capacities, bone densitometry, medication usage, and a physical exam. Project staff obtained indicators of heart-rate variability, beat to beat blood pressure, respiration, and salivary cortisol assessments during an experimental protocol that included both a cognitive and orthostatic challenge. Finally, to augment the self-reported data collected in Project 1, participants completed a medical history, self-administered questionnaire, and self-reported sleep assessments. For respondents at one site (UW-Madison), objective sleep assessments were also obtained with an Actiwatch(R) activity monitor.

This collection includes qualitative data files from two parts of the same study that examined the barriers to autism care for Latino children in Oregon.

  • Part 1: The research team sought to describe community, family, and health care system barriers to diagnosing autism spectrum disorders (ASD) in Latino children. Five focus groups and four individual interviews were conducted with 33 parents of Latino children previously diagnosed with an ASD. Participants described Latino community perceptions of autism and barriers they experienced during the diagnostic process.
  • Part 2: The research team sought to assess the understanding and conceptualization of ASD in the Latino community to understand potential community barriers to early diagnosis. Five focus groups and four individual interviews were conducted with 30 parents of typically developing Latino children. Participants were asked structured questions concerning video vignettes that follow a Latina mother from the time she begins to worry about her 3-year-old son's behaviors to the time he receives an ASD diagnosis.

Midlife in the United States (MIDUS Refresher 1): Biomarker Project, 2012-2016 (ICPSR 36901)

The MIDUS Refresher study Survey (2011-2014 ICPSR 36532) recruited a national probability sample of 3,577 adults, aged 25 to 74, designed to replenish the original MIDUS 1 baseline cohort and paralleling the five decadal age groups of the MIDUS 1 baseline survey (ICPSR 2760). The MIDUS Refresher survey employed the same comprehensive assessments as those assembled on the core longitudinal MIDUS sample, but with additional questions about impacts of the economic recession of 2008-09. The MIDUS Refresher Biomarker study (2012-2016) obtained data from 863 respondents (n=746 Main sample, n=117 African Americans from Milwaukee) who completed the MIDUS Refresher Survey.

The purpose of the Refresher Biomarker Project (Project 4) parallels that of the MIDUS 2 Biomarker project (ICPSR 29282), which collected comprehensive biological assessments on a subsample of MIDUS respondents, thus facilitating analyses that integrate behavioral and psychosocial factors with biological regulation/dysregulation, broadly defined. The aim was to use such data to explicate biopsychosocial pathways that contributed to diverse health outcomes. A further theme was to examine period effects on health (mental and physical) related to the economic recession by comparing the pre-recession MIDUS sample with the post-recession MIDUS Refresher sample. A further objective of the MIDUS Refresher sample was to strengthen cross-project analyses by increasing the sample sizes available for testing hypotheses regarding the interplay of key factors (e.g., socioeconomic status, gender, psychosocial factors, biological factors) in mid- and later-life health.

Investigating the Neurobiologic Basis for Loss of Cortical Laterality in Chronic Stroke Patients, Charleston, South Carolina, 2014-2016 (ICPSR 37313)

The primary goal of this project was to determine the neurobiologic basis for elevated activity in the contralesional primary motor cortex (PMC). In healthy individuals, unimanual movement (with either the left or right hand) is associated with activity in a network of predominantly contralateral brain regions, including the primary motor cortex. This laterality is often compromised following a middle cerebral artery (MCA) stroke. Neuroimaging studies of these patients have shown that unimanual movements with the effected hand are associated with elevated blood oxygen level dependent (BOLD) signal in both the lesioned and the nonlesioned primary motor cortices. Elevated activity in the contralesional PMC is well-established in chronic stroke patients and is associated with poor motor rehabilitation outcomes. Yet the neurobiologic basis for this aberrant neural activity is equivocal.

One factor that may contribute to elevated activity in the contralesional PMC is increased cortical excitatory tone within the contralesional hemisphere. While approximately 80% of the descending corticospinal neurons that control the right hand originate in the left PMC, 20% originate in the right PMC. Elevated activity in the right PMC of left-sided stroke patients may reflect compensatory activity of these descending fibers. Neural activity in the PMC reflects the balance of local excitatory (glutamatergic) and inhibitory (GABAergic) processing. It can be measured in two manners: electrophysiologically, using single hemisphere paired pulse transcranial magnetic stimulation (TMS), and neurochemically, using magnetic resonance spectroscopy (MRS).

Another factor that may contribute to elevated activity in the contralesional PMC is a loss of transcallosal inhibition between the hemispheres. During right hand movement, the left PMC of healthy individuals actively inhibits the right PMC via inhibitory projections through the corpus callosum. In left MCA stroke patients, elevated activity in the contralesional (right) PMC when moving the right hand may reflect a loss of typical inhibition from the left PMC. The integrity of inter-hemispheric information transfer can be measured in two manners: using bi-hemispheric paired-pulse TMS, and using a multimodal brain stimulation/brain imaging approach, interleaved TMS/MRI. Through interleaved TMS/MRI, researchers can selectively stimulate the ipsilesional PMC and quantify the amount of TMS-induced activity in the contralesional PMC.

These two explanations were tested through a cross-sectional investigation of neural function in left MCA stroke patients with mild-moderate right upper extremity impairment and controls matched for age and cardiovascular risk factors. To assess the clinical relevance of these factors on motor dysfunction, the researchers performed a detailed kinematic assessment of movement efficiency, smoothness and compensation.

This study sought to: (1) document the spectrum of injuries and injury characteristics observed among physically-abused older adults reported to Adult Protective Services (APS) and compare those findings to injuries found among non-abused older adults, (2) identify observable injury characteristics and abuse circumstances that healthcare providers, law enforcement and prosecutors consider to be key forensic markers of physical abuse, (3) document information and evidence integral for achieving successful criminal prosecution, and (4) describe approaches that community-based frontline workers can employ to better document evidence of physical abuse.

The data included in this collection were collected under a National Institute of Justice-funded project that sought to document the spectrum and characteristics of injuries observed among physically abused, community-dwelling APS clients.

The collection includes two SAS datasets: injury.sas7bdat (with 47 variables and 403 cases) and subjectleveldata.sas7bdat (with 122 variables and 165 cases); and three SAS System Program Files: analysis-code-v1.sas, cleaned-injury-datasets-v1.sas, and formats.sas. Demographic variables in the collection are in the subjectleveldata.sas7bdat dataset, and include age, year of birth, gender, race, language, and level of education.

This study sought to evaluate the functional independence in children at a Centers for Pediatric Rehabilitation Teleton (CRIT) facility in Guanajuato, Mexico through the use of the WeeFIM Instrument (0-3 Module). The dataset in this collection was generated in May 2013 from electronic health records for secondary analysis of de-identified data. The goal of CRIT, that this research sought to evaluate, was to improve social integration for children with disabilities in Mexico through comprehensive rehabilitation services, including physical therapy, occupational therapy, neurotherapy, speech therapy, physical and rehabilitation medicine, psychology, social integration, and school for parents.

Addiction Health Evaluation and Disease (AHEAD) Management Study in Boston, Massachusetts, 2006-2010 (ICPSR 33581)

Substance dependence (SD) is a chronic disease that requires specialty drug and alcohol treatment, primary care (PC), and management of related problems. Although patients with SD may be linked with specialty care and PC, their health care often remains episodic and fragmented, rather than longitudinal, comprehensive, integrated, and coordinated. As a result, adults with SD often enter addiction treatment later and require acute medical care, rather than entering the system earlier when interventions of lower intensity but longer duration might prevent catastrophes. Chronic disease management (CDM) is a collaborative, longitudinal approach to treatment of certain chronic medical illnesses proven to be more effective than routine care. CDM addresses individual patient and health systems barriers to receipt of needed treatment. However, the effectiveness of CDM for SD has not been tested. The objective of this Addiction Health Evaluation and Disease management (AHEAD) study, was to test the effectiveness of CDM for SD in PC.

Subject identification and recruitment occurred primarily at a local detoxification center, as well as by self and physician referral from the Boston Medical Center primary and ambulatory care clinics, emergency department, urgent care center, inpatient settings, and the community. The study enrolled 320 adults with drug dependence and 320 adults with alcohol dependence who were not in SD treatment, and randomized them to a SD CDM program (the AHEAD Clinic) integrated into a real-world PC clinic or to referral to standard PC. All subjects were assessed regarding SD diagnosis, substance use and problems, readiness to change, health-related quality of life, and medical and drug treatment utilization. Subjects were evaluated 3, 6, and 12 months later, and health services utilization data were collected for 2 years from a statewide database. Additionally, in order to better understand and explain the implementation and fidelity of the AHEAD Clinic, the primary care providers (PCPs) of AHEAD Clinic patients were surveyed. Each PCP was presented with a letter from the Principal Investigator explaining the purpose of the survey, the reason why s/he was being asked to complete the survey, compensation for completing the survey, and details about confidentiality and anonymity. The survey itself consisted of questions asking providers about their satisfaction and their attitudes towards caring for patients with alcohol and drug problems, their knowledge of services that the AHEAD Clinic provides, and their experience working with the AHEAD Clinic.

Primary outcomes were illicit drug use, alcohol use, substance-related problems, emergency department visits, and hospitalizations. The proposal's hypothesis was that compared with standard care, a health services delivery intervention (CDM for SD integrated in PC) would decrease alcohol and illicit drug use and related problems, and improve health care utilization patterns. Improved outcomes using the AHEAD approach would support the adoption of a health services delivery strategy, CDM, to better care for patients with SD.

The major focuses of this Euro-Barometer survey are the environment and personal health. Opinions were sought on environmental issues such as nuclear accidents and radioactivity, pollution, and conservation of natural resources, as well as on the activities surrounding the European Year of the Environment. Health-related issues focused on cancer: smoking, knowledge and views regarding the causes of cancer, the extent of its occurrence in society, and medical recommendations for its early detection and prevention. Respondents were also asked if they had undergone medical examinations to screen for cancer. Women were questioned about specific kinds of cancer detection examinations as well. Other health-related items concerned dietary regimens and sports activities. In addition, respondents were asked about political party preferences, life satisfaction, views on national goals and national achievements, and attitudes toward the Economic Community and its policies, especially the Economic Community's Common Agricultural Policy, the European Parliament, the creation of the single common market in 1992, and use of daylight savings time. The data include demographic, socioeconomic, and geographic information on respondents.

The National Health and Nutrition Examination Surveys (NHANES) is a program of studies designed to assess the health and nutritional status of adults and children in the United States. The NHANES combines personal interviews and physical examinations, which focus on different population groups or health topics. These surveys have been conducted by the National Center for Health Statistics (NCHS) on a periodic basis from 1971 to 1994. In 1999 the NHANES became a continuous program with a changing focus on a variety of health and nutrition measurements which were designed to meet current and emerging concerns. The surveys examine a nationally representative sample of approximately 5,000 persons each year. These persons are located in counties across the United States, 15 of which are visited each year.

For NHANES 2003-2004, there were 12,761 persons selected for the sample, 10,122 of those were interviewed (79.3 percent) and 9,643 (75.6 percent) were examined in the mobile examination centers (MEC). Many of the NHANES 2003-2004 questions were also asked in NHANES II 1976-1980, Hispanic HANES 1982-1984, NHANES III 1988-1994, and NHANES 1999-2002. New questions were added to the survey based on recommendations from survey collaborators, NCHS staff, and other interagency work groups. As in past health examination surveys, data were collected on the prevalence of chronic conditions in the population. Estimates for previously undiagnosed conditions, as well as those known to and reported by survey respondents, are produced through the survey. Risk factors, those aspects of a person's lifestyle, constitution, heredity, or environment that may increase the chances of developing a certain disease or condition, were examined. Data on smoking, alcohol consumption, sexual practices, drug use, physical fitness and activity, weight, and dietary intake were collected. Information on certain aspects of reproductive health, such as use of oral contraceptives and breastfeeding practices, were also collected. The diseases, medical conditions, and health indicators that were studied include: anemia, cardiovascular disease, diabetes and lower extremity disease, environmental exposures, equilibrium, hearing loss, infectious diseases and immunization, kidney disease, mental health and cognitive functioning, nutrition, obesity, oral health, osteoporosis, physical fitness and physical functioning, reproductive history and sexual behavior, respiratory disease (asthma, chronic bronchitis, emphysema), sexually transmitted diseases, skin diseases, and vision. The sample for the survey was selected to represent the United States population of all ages. Special emphasis in the 2003-2004 NHANES was on adolescent health and the health of older Americans. To produce reliable statistics for these groups, adolescents aged 15-19 years and persons aged 60 years and older were over-sampled for the survey. African Americans and Mexican Americans were also over-sampled to enable accurate estimates for these groups. Several important areas in adolescent health, including nutrition and fitness and other aspects of growth and development, were addressed. Since the United States has experienced dramatic growth in the number of older people during the twentieth century, the aging population has major implications for health care needs, public policy, and research priorities. NCHS is working with public health agencies to increase the knowledge of the health status of older Americans. NHANES has a primary role in this endeavor. In the examination, all participants visit the physician who takes their pulse or blood pressure. Dietary interviews and body measurements are included for everyone. All but the very young have a blood sample taken and see the dentist. Depending upon the age of the participant, the rest of the examination includes tests and procedures to assess the various aspects of health listed above. Usually, the older the individual, the more extensive the examination. Some persons who are unable or unwilling to come to the examination center may be given a less extensive examination in their homes.

Demographic data file variables are grouped into three broad categories: (1) Status Variables: provide core information on the survey participant. Examples of the core variables include interview status, examination status, and sequence number. (Sequence number is a unique ID assigned to each sample person and is required to match the information on this demographic file to the rest of the NHANES 2003-2004 data). (2) Recoded Demographic Variables: these variables include age (age in months for persons through age 19 years, 11 months; age in years for 1- to 84-year-olds, and a top-coded age group of 85 years of age and older), gender, a race/ethnicity variable, current or highest grade of education completed, (less than high school, high school, and more than high school education), country of birth (United States, Mexico, or other foreign born), Poverty Income Ratio (PIR), income, and a pregnancy status variable (adjudicated from various pregnancy related variables). Some of the groupings were made due to limited sample sizes for the two-year data set. (3) Interview and Examination Sample Weight Variables: sample weights are available for analyzing NHANES 2003-2004 data. The Social Environment and Biomarkers of Aging Study (SEBAS) in Taiwan, 2000 and 2006, provides information regarding the health and well-being of older persons in Taiwan. Taiwan has undergone rapid demographic, social, and economic changes, becoming a highly urbanized and industrial society with a growing population of persons age 65 or older. SEBAS explores the relationship between life challenges and mental and physical health, the impact of social environment on the health and well-being of the elderly, as well as biological markers of health and stress. The study collected self-reports of physical, psychological, and social well-being, plus extensive clinical data based on medical examinations and laboratory analyses. Examination of health outcomes included chronic illnesses, functional status, psychological well-being, and cognitive function. Questions regarding life challenges focused on perceived stress, economic difficulties, security and safety, and the consequences of a major earthquake. Biological markers were used to identify cardiovascular risk factors, metabolic process measures, immune-system activity, the hypothalamic-pituitary adrenal axis, and sympathetic nervous system activity. Two rounds of biomarker data collected in 2000 and 2006 were complemented by face-to-face interviews with the participants. Demographic and background variables included age, sex, education, ethnicity, occupation, and residency.

The Study of Women's Health Across the Nation (SWAN) is a multisite longitudinal, epidemiologic study designed to examine the health of women during their middle years. The study examines the physical, biological, psychological and social changes during this transitional period. The goal of SWAN's research is to help scientists, health care providers, and women learn how mid-life experiences affect health and quality of life during aging. The study is co-sponsored by the National Institute on Aging (NIA) and the National Institute of Health (NIH), Office of Research on Women's Health. The study began in 1994. Between 1998 and 2001, 2,829 of the 3,302 women that joined SWAN participated in a collection of family history data. The research centers are located in the following communities: Ypsilanti and Inkster, MI (University of Michigan); Boston, MA (Massachusetts General Hospital); Chicago, IL (Rush Presbyterian-St. Luke's Medical Center); Almeda and Contra Costa County, CA (University of California, Davis and Kaiser Permanente); Los Angeles, CA (University of California, Los Angeles); Hackensack, NJ (Hackensack University Medical Center); and Pittsburgh, PA (University of Pittsburgh). SWAN participants represent five racial/ethnic groups and a variety of backgrounds and cultures.

Bruising as a Forensic Marker of Physical Elder Abuse in Orange County, California, 2006-2008 (ICPSR 28144)

The purpose of the study was to describe bruising as a marker of physical elder abuse. Consenting older adults were examined to document location and size of bruises and assess whether they were inflicted during physical abuse. An expert panel confirmed physical abuse. A research nurse conducted study assessments on 67 adults aged 65 and older reported to Adult Protective Services for suspected physical elder abuse in Orange County, California between July 2006 and May 2008. The study contains a total of 142 variables including age, sex, ethnicity, functional status, medical conditions, cognitive status, history of falls, bruise size, bruise location and color, recall of cause, and responses to the Revised Conflicts Tactics Scales (CTS2) and to the Elder Abuse Inventory (EAI).

This project examined the characteristics of sexual assault victimizations in Alaska, as observed and recorded by sexual assault nurse examiners in Anchorage, Kodiak, Bethel, Soldotna, Nome, Fairbanks, Homer, and Kotzebue. The sample utilized for this study included all sexual assault nurse examinations conducted in Anchorage from 1996 to 2004, in Bethel and Fairbanks in 2005 and 2006, and in Homer, Kodiak, Kotzebue, Nome, and Soldotna in 2005. A total of 1,699 examinations were collected. More specifically, the information contains demographic characteristics of patients, pre-assault patient characteristics, assault characteristics, post assault characteristics, exam characteristics and findings, and suspect characteristics. Demographic characteristics of patients include gender, race / ethnicity, and age, whether the patient was disabled, and whether the patient reported being homeless. Pre-assault characteristics included whether the patient reported engaging in consensual sexual activity within three days prior to the assault and information on the location of the initial contact with the suspect. Assault characteristics included information on the location of the assault, methods employed by the suspect, the patients' condition at the time of the assault, the patients' use of drugs and alcohol, and a detailed description of the assault itself. This detailed description included the patient's position during the assault, whether condoms and lubricants had been used, whether ejaculation had occurred, and an inventory of 17 different sexual acts. Post-assault characteristics included information on post-assault actions taken by the patient, whether the patient engaged in consensual sexual activity between the time of the assault to the examination, and the time elapsed from the assault to the examination. Exam characteristics and findings included information on whether the exam was completed, the type of exam that was conducted, the patients' behavioral and emotional state during the exam, whether the patient required emergency medical care, whether the presence of sperm was documented, whether patients tested positive for sexually transmitted infections or other genital infections, whether the patient was pregnant, and whether injuries were documented. Injury characteristics included descriptions of both non-genital and genital injury. A total of 108 indicators of non-genital injury were captured. These included nine possible injuries (i.e., bruising, redness, abrasions, lacerations, swelling, fractures, bite marks, pain, and other) to 12 possible sites (i.e., head/face, mouth, neck, shoulders, arms, hands, chest, abdomen, back, buttocks/hips, legs, and feet). A total of 60 indicators of genital injury were also captured. These included four possible injuries (i.e., bruising, abrasions, lacerations, and tenderness) to 15 possible sites (i.e., mons pubis, labia majora, labia minora, labia majora / minora junction, clitoral hood, clitoris, periurethra, hymen, fossa navicularis, posterior fourchette, perineum, vaginal walls, cervix, anus, and rectum). Suspect characteristics included the number of suspects, whether the identity of the suspect was known, demographic characteristics (gender, race/ethnicity, and age), whether the suspect had used alcohol or drugs, and the relationship between the patient and the suspect. In addition to providing detailed information from sexual assault nurse examinations, the data also include three indicators of legal resolutions - whether cases were referred for prosecution, whether cases were accepted for prosecution, and whether cases resulted in a conviction. Data on legal resolutions are only available for 1,229 cases examined from 1999 to 2005.

The National Health and Nutrition Examination Surveys (NHANES) is a program of studies designed to assess the health and nutritional status of adults and children in the United States. The NHANES combines personal interviews and physical examinations, which focus on different population groups or health topics. These surveys have been conducted by the National Center for Health Statistics (NCHS) on a periodic basis from 1971 to 1994. In 1999 the NHANES became a continuous program with a changing focus on a variety of health and nutrition measurements which were designed to meet current and emerging concerns. The surveys examine a nationally representative sample of approximately 5,000 persons each year. These persons are located in counties across the United States, 15 of which are visited each year. For NHANES 2005-2006, there were 10,348 persons selected for the sample, 10,122 of those were interviewed (79.3 percent) and 9,643 (75.6 percent) were examined in the mobile examination centers (MEC). Many of the NHANES 2005-2006 questions were also asked in NHANES II 1976-1980, Hispanic HANES 1982-1984, NHANES III 1988-1994, and NHANES 1999-2004. New questions were added to the survey based on recommendations from survey collaborators, NCHS staff, and other interagency work groups. As in past health examination surveys, data were collected on the prevalence of chronic conditions in the population. Estimates for previously undiagnosed conditions, as well as those known to and reported by survey respondents, are produced through the survey. Risk factors, those aspects of a person's lifestyle, constitution, heredity, or environment that may increase the chances of developing a certain disease or condition, were examined. Data on smoking, alcohol consumption, sexual practices, drug use, physical fitness and activity, weight, and dietary intake were collected. Information on certain aspects of reproductive health, such as use of oral contraceptives and breastfeeding practices, were also collected. The diseases, medical conditions, and health indicators that were studied include: anemia, cardiovascular disease, diabetes and lower extremity disease, environmental exposures, equilibrium, hearing loss, infectious diseases and immunization, kidney disease, mental health and cognitive functioning, nutrition, obesity, oral health, osteoporosis, physical fitness and physical functioning, reproductive history and sexual behavior, respiratory disease (asthma, chronic bronchitis, emphysema), sexually transmitted diseases, skin diseases, and vision. The sample for the survey was selected to represent the United States population of all ages. Special emphasis in the 2005-2006 NHANES was on adolescent health and the health of older Americans. To produce reliable statistics for these groups, adolescents aged 15-19 years and persons aged 60 years and older were over-sampled for the survey. African Americans and Mexican Americans were also over-sampled to enable accurate estimates for these groups. Several important areas in adolescent health, including nutrition and fitness and other aspects of growth and development, were addressed. Since the United States has experienced dramatic growth in the number of older people during the twentieth century, the aging population has major implications for health care needs, public policy, and research priorities. NCHS is working with public health agencies to increase the knowledge of the health status of older Americans. NHANES has a primary role in this endeavor. In the examination, all participants visit the physician who takes their pulse or blood pressure. Dietary interviews and body measurements are included for everyone. All but the very young have a blood sample taken and see the dentist. Depending upon the age of the participant, the rest of the examination includes tests and procedures to assess the various aspects of health listed above. Usually, the older the individual, the more extensive the examination. Some persons who are unable or unwilling to come to the examination center may be given a less extensive examination in their homes. Demographic data file variables are grouped into three broad categories: (1) Status Variables: provide core information on the survey participant. Examples of the core variables include interview status, examination status, and sequence number. (Sequence number is a unique ID assigned to each sample person and is required to match the information on this demographic file to the rest of the NHANES 2005-2006 data). (2) Recoded Demographic Variables: these variables include age (age in months for persons through age 19 years, 11 months; age in years for 1- to 84-year-olds, and a top-coded age group of 85 years of age and older), gender, a race/ethnicity variable, current or highest grade of education completed, (less than high school, high school, and more than high school education), country of birth (United States, Mexico, or other foreign born), Poverty Income Ratio (PIR), income, and a pregnancy status variable (adjudicated from various pregnancy related variables). Some of the groupings were made due to limited sample sizes for the two-year dataset. (3) Interview and Examination Sample Weight Variables: sample weights are available for analyzing NHANES 2005-2006 data. Beginning in 2007, some changes were made to the domains being oversampled. Sufficient numbers of MAs were retained in the sample design so that trends in the health of MAs can continue to be monitored. Persons 60 years of age and older, Blacks, and low income persons were also oversampled. In addition, for each of the race/ethnicity domains, the 12-15 and 16-19 year age domains were combined and the 40-59 year age minority domains were split into 10-year age domains of 40-49 and 50-59. This has led to an increase in the number of participants aged 40 and older and a decrease in 12- to 19-year-olds from previous cycles. The oversample of pregnant women and adolescents in the survey from 1999-2006 was discontinued to allow for the oversampling of the Hispanic population. The New York City Department of Health and Mental Hygiene, with support from the National Center for Health Statistics, conducted the New York City Health and Nutrition Examination Survey (NYC HANES) to improve disease surveillance and establish citywide estimates for several previously unmeasured health conditions from which reduction targets could be set and incorporated into health policy planning initiatives. NYC HANES also provides important new information about the prevalence and control of chronic disease precursors, such as undiagnosed hypertension, hypercholesterolemia, and impaired fasting glucose, which allow chronic disease programs to monitor more proximate health events and rapidly evaluate primary intervention efforts. Study findings are used by the public health community in New York City, as well as by researchers and clinicians, to better target resources to the health needs of the population. The NYC HANES data consist of the following six datasets: (1) Study Participant File (SPfile), (2) Computer-Assisted Personal Interview (CAPI), (3) Audio Computer-Assisted Self-Interview (ACASI), (4) Composite International Diagnostic Interview(CIDI), (5) Examination Component, and (6) Laboratory Component. The Study Participant File contains variables necessary for all analyses, therefore, when using the other datasets, they should be merged to this file. Variable P_ID is the unique identifier used to merge all datasets. Merging information from multiple NYC HANES datasets using SP_ID ensures that the appropriate information for each SP is linked correctly. (SAS datasets must be sorted by SP_ID prior to merging.) Please note that NYC HANES datasets may not have the same number of records for each component because some participants did not complete each component. Demographic variables include race/ethnicity, Hispanic origin, age, body weight, gender, education level, marital status, and country of birth.

The National Hospital Ambulatory Medical Care Survey (NHAMCS) provides data from samples of patient records selected from emergency departments (EDs) and outpatient departments (OPDs) of a national sample of hospitals. The resulting national estimates describe the use of hospital ambulatory medical care services in the United States. For the 2003 survey, data were collected from 236 OPDs and 475 EDs. Among the variables included are age, race, and sex of the patient, reason for the visit, physician's diagnoses, cause of injury (EDs only), surgical procedures (OPDs only), medication therapy, and expected source of payment. For 2003, additional updates and revisions have been made to the drug characteristics data. The variables CSTRATM and CPSUM have been added to assist in the computation of variances.

Systems Change Analysis of Sexual Assault Nurse Examiner (SANE) Programs in One Midwestern County of the United States, 1994-2007 (ICPSR 25881)

The purpose of this study was to determine whether adult sexual assault cases in a Midwestern community were more likely to be investigated and prosecuted after the implementation of a Sexual Assault Nurse Examiner (SANE) program, and to identify the 'critical ingredients' that contributed to that increase.

Part 1 (Study 1: Case Records Quantitative Data) used a quasi-experimental, nonequivalent comparison group cohort design to compare criminal justice systems outcomes for adult sexual assault cases treated in county hospitals five years prior to the implementation of the Sexual Assault Nurse Examiner (SANE) program (January 1994 to August 1999) (the comparison group, n=156) to cases treated in the focal SANE program during its first seven years of operation (September 1999 to December 2005) (the intervention group, n=137). Variables include focus on case outcome, law enforcement agency that handled the case, DNA findings, and county-level factors, including prosecutor elections and the emergence of the focal SANE program.

Part 2 (Study 2: Case Characteristics Quantitative Data) used the adult sexual assault cases from the Study 1 intervention group (post-SANE) (n=137) to examine whether victim characteristics, assault characteristics, and the presence and type of medical forensic evidence predicted case progression outcomes.

Part 3 (Study 3: Police and Prosecutors Interview Qualitative Data) used in-depth interviews in April and May of 2007 with law enforcement supervisors (n=9) and prosecutors (n=6) in the focal county responsible for the prosecution of adult sexual assault crimes to explore if and how the SANEs affect the way in which police and prosecutors approach such cases. The interviews focused on four main topics: (1) whether they perceived a change in investigations and prosecution of adult sexual assault cases in post-SANE, (2) their assessment of the quality and utility of the forensic evidence provided by SANEs, (3) their perceptions regarding whether inter-agency training has improved the quality of police investigations and reports post-SANE, and (4) their perceptions regarding if and how the SANE program increased communication and collaboration among legal and medical personnel, and if such changes have influenced law enforcement investigational practices or prosecutor charging decisions.

Part 4 (Study 4: Police Reports Quantitative Data) examined police reports written before and after the implementation of the SANE program to determine whether there had been substantive changes in ways sexual assaults cases were investigated since the emergence of the SANE program. Variables include whether the police had referred the case to the prosecutor, indicators of SANE involvement, and indicators of law enforcement effort.

Part 5 (Study 5: Survivor Interview Qualitative Data) focused on understanding how victims characterized the care they received at the focal SANE program as well as their expriences with the criminal justices system. Using prospective sampling and community-based retrospective purposive sampling, twenty adult sexual assault vicitims were identified and interviewed between January 2006 and May 2007. Interviews covered four topics: (1) the rape itself and initial disclosures, (2) victims' experiences with SANE program staff including nurses and victim support advocates, (3) the specific role forensic evidence played in victims' decisions to participate in prosecution, and (4) victims' experiences with law enforcement, prosecutors, and judicial proceedings, and if/how the forensic nurses and advocates influenced those interactions.

Part 6 (Study 6: Forensic Nurse Interview Qualitative Data) examined forensic nurses' perspectives on how the SANE program could affect survivor participation with prosecution indirectly and how the interactions between SANEs and law enforcement could be contributing to increased investigational effort. Between July and August of 2008, six Sexual Assault Nurse Examiners (SANEs) were interviewed. This data collection is an epidemiological study of chronic disease in the oldest old. It is based on information collected from Kaiser Permanente facilities in Northern California. Members of Kaiser Permanente Medical Care programs who were aged 65 or older at the time the data were being collected and who had taken a multiphasic health checkup examination within four to six years of the baseline date were included in the study. There are several components to the dataset. Part 1, Master Records, includes information from the morbidity review, in which over 35 chronic conditions or diagnoses were abstracted from the member charts, as well as detailed diagnostic criteria for the major conditions. A prevalence review was done, which included the four years prior to the baseline date for these same conditions. Recurrent disease is included for the following conditions: cancers, myocardial infarction, and various forms of strokes. A detailed account of outpatient health services use, and data from the multiphasic health checkup, which was administered to each participant during the nine yearly follow-ups, are also included in the Master Records file. Part 2, Hospitalization, contains records of causes and dates of hospitalizations and discharges and nursing home admissions. There is also a section on incomplete reviews and the reasons for them. Demographic information and some lifestyle information from the multiphasic health checkup (e.g., smoking, alcohol, and Body Mass Index) are also in this file.

This round of Eurobarometer surveys diverged from the Standard Eurobarometer measures and queried respondents on (1) nuclear energy and safety, and several public health issues including (2) electromagnetic fields, (3) alcohol and smoking habits, and passive smoking, (4) organ donation, and (5) personal state of health and prevention. For the first topic, the survey queried respondents about their knowledge of and opinions regarding nuclear power and safety, the regulation of its use, their relationship with nuclear power, and nuclear energy as an energy source. For the second topic, respondents were asked to identify sources of electromagnetic fields, potential health risks associated with electromagnetic fields, and the entities protecting them from these risks. For the third topic, respondents were asked about their alcohol consumption, the effects of price fluctuation on alcohol purchases, knowledge of blood alcohol content, and liquor control laws. In addition, respondents were queried about their smoking habits, their efforts to quit smoking, their use of tobacco cessation products, and about passive smoking and the ban of smoking in public places. For the fourth topic, respondents were asked their views about organ donation, whether they have discussed this topic with family, their willingness to donate an organ after death, and their support for and use of an organ donor card. For the final topic, personal state of health and prevention, the survey queried respondents about their general health and quality of life, current or past health problems, and the location of body pain. Respondents were also asked about treatment for chronic illness, medical tests or health checkups, and recent changes in health behavior. In addition, women responded about their opinion of and use of hormone replacement therapy. Demographic and other background information includes age, gender, origin of birth (personal and parental), marital status, left-right political self-placement, occupation, age at completion of full-time education, household composition, and ownership of a fixed or a mobile telephone and other durable goods. In addition, respondents were asked to identify persons in need of long term care, to provide their opinion and experiences in the planning and provision of long term care for the elderly, including the health care costs, and to evaluate the risk that dependent elders are being exposed to abuse and need for future personal care requirements. The second special topic, undeclared work, respondents were asked to identify their knowledge of persons who work without declaring income to tax or social security institutions, and the characteristics and reasons of those who would most likely do so. Respondents also evaluated the risk of being detected in not declaring income for which supplementary bills or fines may be issued, and sanctions expected to be implemented by authorities in response to a certain amount of income that is undeclared. The survey also queried respondents about services and goods acquired from an individual or group associated with undeclared work, and undeclared payment received from their employer and portion of gross yearly income this comprises, and their opinion about these practices. In addition, respondents identified the type and frequency of undeclared work in which they participated, amount of income received for this work, and the reasons this work was completed and for whom, and consequences in working undeclared. In addition, respondents assessed the legitimacy of certain behaviors pertaining to public and private economic transactions. As the next special topic, the survey examines respondents' knowledge of which countries currently plan to join the EU, which countries neighbor the EU, the European Neighborhood policy, and obtaining information about developmental aid. Pertaining to this policy, respondents were asked to assess the relationship between the EU and neighboring countries, and the importance of issues which would affect this relationship, including providing economic assistance. Respondents provided their opinion in regard to developmental aid the EU provides to the poor, the efficiency of providing aid through each member state or the European Commission, which donor provides the most aid to developing countries, and priorities for the EU in disbursing developmental aid. For the final special topic, respondents were asked about their knowledge of the sides of euro coins, to identify the genuineness and value of particular coins, to describe their experiences in accepting a fake, or a non-euro coin or coin-like object, and their opinion in regard to the national sides of the coins which differ among each country in the EU. Demographic and other background information includes respondent's age, estimate of life expectancy, gender, nationality, origin of birth (personal and parental), marital status, left-to-right political self-placement, occupation, age when stopped full-time education, household composition, ownership of a fixed or a mobile telephone and other durable goods, type and size of locality, region of residence, and language of interview (select countries). Respondents were also queried about their family size, including the number of children birthed, ages of their mother and father, and the housing situation for their child or parent, including distance from respondent. The survey also collected information such as the job sector in which the respondent currently works, number employed by respondent's employer, gross income, and hours worked per week at formal employment.

Positive Connections: Connecting HIV-Infected Patients to Care, 2004-2006 [United States] (ICPSR 22482)

The research study Positive Connections tested the Health Systems Navigation (HSN) model, an intervention linking near-peer interventionists with underserved HIV-infected individuals to assist them to become engaged and retained in HIV medical care through supportive services and facilitated referrals. Working with a core group of local AIDS service organizations to identify unstable and out-of-care HIV-positive individuals, the HSN will enroll and provide health system navigation to participants. The principal goal was to enhance the probability that individuals from historically underserved populations would become engaged and retained in high quality, culturally competent HIV care. The theoretical basis for this intervention included individual behavior change models, social and community networks, and provider cultural competence. This project also sought to improve the understanding and the measurement of health care access problems by seeking to determine which problems have indicators and which do not, and to identify steps that can be taken to develop a reliable access monitoring system. The concept of Health Systems Navigator has been developed by the Multicultural AIDS Coalition.

The National Survey of Medical Decisions (the DECISIONS study) was a random-digit dial telephone survey of 3,010 adults over the age of 40 in the United States conducted between November 2006 and May 2007. Participants were asked a series of screening questions to identify which of ten common medical decisions they may have discussed with their health care providers in the previous two years and then completed two to three question modules regarding specific decisions that were relevant to each individual.

Funded by a grant from the Foundation for Informed Medical Decision Making (Boston, MA), the DECISIONS study is a unique data source which enables consideration of a wide variety of research questions related to when and how older adults manage the medical decisions they face. The initial screening module gathered highly generalizeable data regarding the prevalence of different types of common medical decisions in the experience of older Americans. The dynamically-administered modules then requested detailed information regarding how and when patients discuss key medical decisions with their health care providers, and whether variations in decision-making processes may have influenced patients' medical care.

This round of Eurobarometer surveys queried respondents on standard Eurobarometer measures as well as lifelong learning, health, and partners and fertility. To start the interview, standard trend questions were asked regarding the euro and European Union (EU) policies. New questions were asked about EU expansion and priorities for local and national authorities. Next, questions were asked regarding lifelong learning situations within and outside of one's private life. Respondents were queried about places where learning occurred (e.g., school, work, social situations, and home), whether the study/training was for work or personal reasons, whether it was mandatory, what the benefits were, reasons for future study/training, best ways to improve job skills, obstacles to study/training, most useful information sources for study/training, and how much respondents would be willing to pay toward study/training. Respondents were also asked whether they possessed certain skills and could prove it, whether these skills were important within and outside of their private lives, and which study/training opportunities in the past five years they considered most important. The third portion of the interview collected information on respondent health problems and health-related issues. Questions were asked pertaining to current illnesses, visits to various medical professionals, long-term treatment of a variety of conditions (e.g., diabetes, cancer, and AIDS/HIV), dental issues, health tests, and check-ups (e.g., X-rays, blood pressure, and hearing), diet, alcohol consumption, and opinions of and things done to ensure child safety. Women in the study were asked about their knowledge and use of hormone replacement therapy, gynecological and other female-specific types of exams, and breastfeeding of children. The fourth portion of the survey dealt with issues of family and partners. Respondents were asked about the importance of having a spouse/partner and children, whether the father or mother should carry out certain childcare and household tasks, whether they had children and if so who was responsible for various household and childcare tasks, and with which of these task divisions they were most dissatisfied. They were also queried on plans for more children and desire for and timing of the births of current children. Background variables include age, gender, nationality, marital status, left-right political self-placement, occupation, age at completion of education, type and size of locality, and household income.

The National Ambulatory Medical Care Surveys (NAMCS) supply data on ambulatory medical care provided in physicians' offices. The 1985 survey, the tenth in a series of annual surveys, contains information from approximately 2,879 physicians. Data are available on the number of office visits by the patient, the nature and length of the visits, the patient's problem, the physician's diagnosis, and the kinds of diagnostic and therapeutic services rendered. Information is also included on the physician's specialization, type of practice, and geographical location. Demographic information on patients was also collected.
