Consumer Expenditure Survey, 1991: Interview Survey, Detailed Expenditure Files (ICPSR 6262)
The ongoing Consumer Expenditure Survey (CE) provides a continuous flow of information on the buying habits of American consumers and also furnishes data to support periodic revisions of the Consumer Price Index. The survey consists of two separate components:
- A quarterly Interview Survey in which each consumer unit in the sample is interviewed every three months over a 15-month period, and
- A Diary Survey completed by the sample consumer units for two consecutive one-week periods.
The Interview Survey was designed to collect data on major items of expense, household characteristics, and income. The expenditures covered by the survey are those that respondents can recall fairly accurately for three months or longer. In general, these expenditures include relatively large purchases, such as those for property, or expenditures that occur on a fairly regular basis, such as rent, utilities, or insurance premiums. Excluded are nonprescription drugs, household supplies, and personal care items. Including global estimates on spending for food, it is estimated that about 90 to 95 percent of expenditures are covered in the Interview Survey.
The Detailed Expenditure Files that comprise this data collection were created from all the major expenditure sections of the Interview Survey questionnaires. These files contain more detailed expenditure records than those found in the Interview Survey data tapes. In addition, the Detailed Expenditure Files include family characteristics (FMLY) files and income and member characteristics (MEMB) files identical to those found in the Interview Survey.
CTDA 1010: Posttraumatic Stress in Children Age 6 to 16 Hospitalized for Accident-Related Injury and Their Parents, Australia, 2000-2004 (ICPSR 39198)
Trajectory modeling can identify patterns of posttraumatic stress symptoms in children and parents. This study aimed to describe trajectories of child and parent posttraumatic stress symptoms across 2 years post-injury, and to examine potential risk factors predicting problematic trajectories. The study enrolled children age 7 to 16 admitted to general or intensive care units for treatment of accidental injury, and one parent/caregiver per child. Within 2 weeks of injury, and at 4-6 weeks, 6 months, and (a subset) at 2 years post-injury, children and parents were assessed for posttraumatic stress symptoms. Parents also completed measures of parenting behavior and pre-injury child mental health.

Midlife in the United States (MIDUS 3): Neuroscience Project, 2017-2022 (ICPSR 38862)
From 2004-2009, an initial follow-up of the original Midlife Development in the United States samples (MIDUS 2) was conducted with expansion of the protocol to include Neuroscience Project data collection and a sample of Black Americans from Milwaukee, WI. The MIDUS Neuroscience Project performed a second follow-up from 2017-2022 of the MIDUS Main and Milwaukee samples (MIDUS 3) on a subsample of those who completed the MIDUS 3 Survey and Biomarker Projects.
The goal was to examine indices of brain aging, function, and structure with a focus on the brain circuitry associated with individual differences in affective style, and to characterize the peripheral consequences of these central profiles for biological systems that may be relevant to health. The primary aims were to: (1) characterize individual differences in emotional reactivity, recovery, and sustaining processes using corrugator and zygomatic electromyography and eyeblink startle magnitude, (2) characterize individual differences in brain morphology and connectivity using structural magnetic resonance imaging (MRI) and diffusion weighted imaging (DWI) (3) characterize individual differences in functional activity within the neural circuitry of emotion using task and resting state fMRI, (4) calculate brain age, and (5) test the ability of these indices to predict the comprehensive array of health, wellbeing, cognitive, psychological, social, and life challenge factors assessed in other MIDUS projects. To probe individual differences in emotional processes, psychophysiological and fMRI measures of emotional responses to the presentation of negative, positive, and neutral pictures, and these same measures during a post-picture period were examined.
Emotion-influenced memory was assessed at both the psychophysiological and imaging sessions: (1) Free recall of the presented affective pictures at the end of the psychophysiological session. (2) Memory and likeability ratings for neutral faces paired with the affective pictures in the imaging task. Finally, selected tasks from the CANTAB assessed affective biases and cognitive processes important for emotion regulation.

Comparative Effectiveness of Anti-TNF in Combination with Low Dose Methotrexate vs Anti-TNF Monotherapy in Pediatrics Crohn's Disease (COMBINE), United States, 2015-2022 (ICPSR 38680)
The COMBINE study was a longitudinal examination of pediatric Crohn's Disease (CD) patients in the United States with data collected from 2015-2022. This study was a randomized, double blind, placebo controlled pragmatic trial to compare low dose oral methotrexate versus a placebo in children with Crohn's disease initiating anti-TNF (tumor necrosis factor) therapy with Infliximab or Adalimumab. Eligible participants were randomized with a 1:1 allocation and followed for a minimum of 12 months and maximum of 36 months in the context of routine clinical care. The primary outcome was a composite of indicators of treatment failure and/or toxicity. Secondary outcomes included patient reported outcomes of pain interference and fatigue.
Crohn's disease (CD) is a chronic inflammatory bowel disease (IBD) that affects approximately 600,000 Americans with estimated direct costs of $3.6 billion annually. Typical symptoms (e.g., abdominal pain, bloody diarrhea) result in substantial morbidity, including hospitalization and surgery, missed work and school, and diminished quality of life. The primary treatment goals for all CD patients are to induce remission by eradicating intestinal inflammation and related symptoms and maintain remission by preventing disease flares and progression. Additional treatment goals for pediatric CD include restoring physical and emotional development.

Decision Making About Hospital Arrival in Childbirth, United States, 2014-2015 (ICPSR 38772)
Childbirth is the most common reason for hospital admission in the United States (US) and the timing of admission influences the management and outcomes of labor, including rates of cesarean delivery. Although cesareans are life saving in emergency situations, the current prevalence and variability leads to excess risk for morbidity and mortality as well as higher health care costs in comparison to vaginal deliveries. Delaying hospital admission of women in latent labor is one of the most widely promoted strategies to reduce the likelihood of caesarean birth and its safety is established. Yet, trials of interventions that have aimed to reduce early admissions and the subsequent rates of medical intervention in labor have not succeeded. One proposed explanation is that the evaluated interventions exclusively focused on clinician assessment and diagnosis of active labor in hospital settings. The interventions did not fully account for women's recognition and response to the onset of labor, which is initially negotiated by the laboring women and members of her social network in settings outside the hospital. To develop efficacious strategies to reduce the likelihood of cesarean delivery, a qualitative understanding of why some women present early in labor and others later, and what can be done to promote timely hospital admission among medically low-risk nulliparous women is needed.
Specific Aim I: Determine the decision-making criteria and sequence of decision criteria used by women choosing either to go to the hospital or stay at home in early labor.
Specific Aim II: Determine the degree to which a symptom and labor management taxonomy accurately reflects women's experience with the recognition and response to early labor prior to hospital admission.
Comparison of Outcomes of Antibiotic Drugs and Appendectomy (CODA), United States, 2016-2020 (ICPSR 38541)
Antibiotics are considered a feasible treatment for appendicitis, yet appendectomy remains the treatment standard in the United States. Previous randomized trials comparing these treatments excluded important subgroups and recruited small sample sizes but questions remain about the applicability of these previous findings. This study conducted the Comparison of Outcomes of antibiotic Drugs and Appendectomy (CODA) randomized clinical trial to compare antibiotics with appendectomy among adults with appendicitis, including those with appendicolith. Those recruited comprised a diverse population, compared an overall measure of health status as the primary outcome, and included several secondary clinical and patient-reported outcomes, complications, and measures of healthcare utilization.
Comprehensive Post-Acute Stroke Services (COMPASS) Study, North Carolina, 2016-2018 (ICPSR 38185)
The Comprehensive Post-Acute Stroke Services (COMPASS) Study is a pragmatic cluster-randomized clinical trial that evaluated the real-world effectiveness of the COMPASS transitional care (COMPASS-TC) model compared to usual care among adult stroke and transient ischemic attack (TIA) patients discharged home between 2016 and 2018. In Phase 1, 40 North Carolina hospital units were randomized 1:1 to the COMPASS-TC intervention or usual care, stratified by stroke patient volume and stroke center certification. In Phase 2, hospitals randomized to usual care crossed over to implement COMPASS-TC, and hospitals randomized to the intervention sustained COMPASS-TC. The intervention was patient-centered and assessed social and functional determinates of health to inform individualized care plans for secondary prevention, recovery, and referrals to services and community-based resources. COMPASS-TC was consistent with Centers for Medicare and Medicaid Services (CMS) TC management reimbursement requirements.
The primary outcome was functional status (Stroke Impact Scale-16; SIS-16) at 90 days; secondary outcomes were mortality, disability, medication adherence, depression, cognition, self-rated health, fatigue, care satisfaction, home blood pressure monitoring, falls, and caregiver strain. Telephone interviewers, blinded to treatment assignment, assessed these outcomes at 90 days.

Sacramento Area Latino Study on Aging (SALSA Study), 1996-2008: Semi-Annual Phone Call Data (ICPSR 29321)
The Sacramento Area Latino Study on Aging (SALSA Study) was a longitudinal cohort study of 1,789 community-dwelling Mexican Americans residing in California's Sacramento Valley who were aged 60-101 years at baseline in 1998-1999. Participants were followed every 12-15 months via home visits that included clinical and cognitive assessments. A semiannual phone call was made to obtain updates on medications, health events, and some sociodemographic risk factors. This study, Semi-Annual Phone Call Data, contains data from the six-month follow-up calls of the SALSA project.
The SALSA project tracked the incidence of physical and cognitive impairment as well as dementia and cardiovascular diseases in elderly Latinos in the Sacramento, California region. The SALSA project aimed to assess cognitive, physical, and social functions, which include the ability to follow instructions, to perform certain movements, and to interact with others. The project explored the effects that cultural, nutritional, social, and cardiovascular risk factors have on overall health and dementia, and examined the association between diabetes and functional status.
Demographic information collected in these data includes language and age given at follow-up visits.
Epidemiologic Catchment Area Program Sites 1-4, 1979-1983 with National Death Index Data through 2007 (ICPSR 36621)
The Epidemiologic Catchment Area (ECA) program of research was initiated in response to the 1977 report of the President's Commission on Mental Health. The purpose was to collect data on the prevalence and incidence of mental disorders and on the use of and need for services by the mentally ill. Independent research teams at five universities (Yale University, Johns Hopkins University, Washington University, Duke University, and University of California at Los Angeles), in collaboration with the National Institute for Mental Health, conducted the studies with a core of common questions and sample characteristics. The sites were areas that had previously been designated as Community Mental Health Center catchment areas: New Haven, Connecticut, Baltimore, Maryland, St. Louis, Missouri, Durham, North Carolina, and Los Angeles, California. Each site sampled over 3,000 community residents and 500 residents of institutions, yielding 20,861 respondents overall. The longitudinal ECA design incorporated two waves of personal interviews administered one year apart and a brief telephone interview in between (for the household sample). The diagnostic interview used in the ECA was the NIMH Diagnostic Interview Schedule (DIS), Version III (with the exception of the Yale Wave I survey, which used Version II). Diagnoses were categorized according to the DIAGNOSTIC AND STATISTICAL MANUAL OF MENTAL DISORDERS, 3rd Edition (DSM-III). Diagnoses derived from the DIS include manic episode, dysthymia, bipolar disorder, single episode major depression, recurrent major depression, atypical bipolar disorder, alcohol abuse or dependence, drug abuse or dependence, schizophrenia, schizophreniform, obsessive compulsive disorder, phobia, somatization, panic, antisocial personality, and anorexia nervosa. The DIS uses the Mini-Mental State Examination (MMSE), which measures cognitive functioning, as an indirect measure of the DSM-III Organic Mental Disorders. In the ECA survey, this diagnosis is called cognitive impairment.
This collection features data from 17,327 participants across 2,005 variables. Data from the Los Angeles, California, Catchment (UCLA) are not included. Baseline data (Wave 1) and Wave 2 data were linked to the National Death Index through 2007, which includes primary and contributing causes of death, International Classification of Disease (ICD) codes, and nature of injury variables.
Johns Hopkins University Prevention Research Center - Risks for Transitions in Drug Use Among Urban Adults, Baltimore City, 2008-2011 (ICPSR 36622)
The Johns Hopkins University Prevention Research center - Risks for Transitions, Baltimore city, 2008-2011 study (JHU PRC Risks for Transition in Drug Use among Urban Adults) represents data collected for a 25 years follow-up from the original JHU PRC study. At the start of the data collection, a cohort of 2,311 youth were randomly assigned to two classroom-based universal preventive interventions implemented in 43 classrooms of 19 public schools located in 5 socio-demographically distinct areas in Eastern Baltimore. This collection includes data from 1,434 respondents from the original cohort collected in 2008-2012. The mean age of the respondents was 30-32 years old.
Psychopathology (major depressive episode, phobia and anxiety disorders) was assessed using modules from the Composite International Diagnostic Interview-University of Michigan Version, CIDI- UM (WHO, 1997). An antisocial symptom scale adapted from the National Epidemiologic Survey of Alcohol and Related Conditions (NESARC) was administered which consisted of 30 questions that assessed behaviors experienced since turning 18 years of age. The alcohol, tobacco and drug modules of the interview were modeled after NESARC Alcohol Use Disorder and Associated Disabilities Interview Schedule-IV. Types of information in these modules collected include age of onset, use in past year and prior past year, frequency and recency of use, and DSM-IV abuse and dependence criteria. Socioeconomic status indicators, variables on education, employment, marital status, parenthood, social supports, family history of mental health and drug problems, general health, treatment utilization, and life events are included in this dataset.
This dataset includes 3,140 variables.
Documentation of Resident to Resident Elder Mistreatment in Residential Care Facilities, New York City, 2009-2013 (ICPSR 35649)
These data are part of NACJD's Fast Track Release and are distributed as they were received from the data depositor. The files have been zipped by NACJD for release, but not checked or processed except for the removal of direct identifiers. Users should refer to the accompanying readme file for a brief description of the files available with this collection and consult the investigator(s) if further information is needed.
The purpose of this study was to investigate violence and aggression committed by nursing home residents that is directed toward other residents, referred to here as resident-to-resident elder mistreatment (R-REM). Resident-to-resident mistreatment (R-REM) was defined as: negative and aggressive physical, sexual, or verbal interactions between long term care residents, that in a community setting would likely be construed as unwelcome and have high potential to cause physical or psychological distress in the recipient.
The goals of this project were to: enhance institutional recognition of R-REM; examine the convergence of R-REM reports across different methodologies; identify the most accurate mechanism for detecting and reporting R-REM; develop profiles of persons involved with R-REM by reporting source; investigate existing R-REM policies, and; develop institutional guidelines for reporting R-REM episodes. Also, the project team sought to answer the following research questions: (1) Will the reporting of R-REM differ by source? (2) Which reporting methods will show the highest level of convergence and accuracy in reporting? (3) What resident characteristics or profiles will predict R-REM across the differing reporting sources? (4) What are the existing guidelines and/or institutional policies for reporting R-REM? To achieve these goals, the researcher conducted this study over a two week period in five urban and five suburban New York City facilities. Resident-to-resident abuse information was derived from five sources: (1) resident interviews (2) staff informants (3) observational data (behavior sheets) (4) resident chart reviews (5) incident and accident reports.
National Health Interview Survey, 2010 (ICPSR 36144)
These data are being released in BETA version to facilitate early access to the study for research purposes. This collection has not been fully processed by NACDA or ICPSR at this time; the original materials provided by the principal investigator were minimally processed and converted to other file types for ease of use. As the study is further processed and given enhanced features by ICPSR, users will be able to access the updated versions of the study. The National Health Interview Survey (NHIS) is conducted annually and sponsored by the National Center for Health Statistics (NCHS), which is part of the U.S. Public Health Service. The purpose of the NHIS is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive across the United States population through the collection and analysis of data on a broad range of health topics. The redesigned NHIS questionnaire introduced in 1997 (see National Health Interview Survey, 1997 [ICPSR 2954]) consists of a core that remains largely unchanged from year to year, plus an assortment of supplements varying from year to year.
The 2010 NHIS Core consists of three modules: Family, Sample Adult, and Sample Child. The datasets derived from these modules include Household Level, Family Level, Person Level, Injury/Poison Episode Level, Injury/Poison Verbatim Level, Sample Adult Level, and Sample Child level.
The 2010 NHIS supplements consist of stand alone datasets for Cancer Level and Quality of Life data derived from the Sample Adult core and Disability Questions Tests 2010 Level derived from the Family core questionnaire. Additional supplementary questions can be found in the Sample Child dataset on the topics of cancer, immunization, mental health, and mental health services and in the Sample Adult dataset on the topics of epilepsy, immunization, and occupational health.
Part 1, Household Level, contains data on type of living quarters, number of families in the household responding and not responding, and the month and year of the interview for each sampling unit. Parts 2-5 are based on the Family Core questionnaire. Part 2, Family Level, provides information on all family members with respect to family size, family structure, health status, limitation of daily activities, cognitive impairment, health conditions, doctor visits, hospital stays, health care access and utilization, employment, income, participation in government assistance programs, and basic demographic information. Part 3, Person Level, includes information on sex, age, race, marital status, education, family income, major activities, health status, health care costs, activity limits, and employment status. Parts 4 and 5, Injury/Poisoning Episode Level and Injury/Poisoning Verbatim Level, consist of questions about injuries and poisonings that resulted in medical consultations for any family members and contains information about the external cause and nature of the injury or poisoning episode and what the person was doing at the time of the injury or poisoning episode, in addition to the date and place of occurrence.
A randomly-selected adult in each family was interviewed for Part 6, Sample Adult Level, regarding specific health issues, the relation between employment and health, health status, health care and doctor visits, limitation of daily activities, immunizations, and behaviors such as smoking, alcohol consumption, and physical activity. Demographic information, including occupation and industry, also was collected. The respondents to Part 6 also completed Part 7, Cancer Level, which consists of a set of supplemental questions about diet and nutrition, physical activity, tobacco, cancer screening, genetic testing, family history, and survivorship. Part 8, Sample Child Level, provides information from an adult in the household on medical conditions of one child in the household, such as developmental or intellectual disabilities, respiratory problems, seizures, allergies, and use of special equipment like hearing aids, braces, or wheelchairs.
Parts 9 through 13 comprise the additional Supplements and Paradata for the 2010 NHIS. Part 9, Disability Questions Tests 2010 Level, is a supplemental set of six questions asked at the end of the Family Core questionnaire about sensory, mobility, self-care, cognition, and independent living issues. Part 10, Paradata Level, does not contain health related information, but rather data which are related to the interview process, including measures of time, contact-ability, and cooperation. Please see the User Guide for additional information and details. Part 11, Quality of Life Level, was asked to a randomly selected subsection of the Sample Adult questionnaire. Respondents were asked about participation in society, degree of difficulty and functioning in activity domains including vision, hearing, mobility, upper body, learning, cognition, affect, pain, fatigue, and communication. Part 12, Special Sample Adult Disability Weights Level, contains weights for use with an analysis of the merged data from the Sample Adult Level and Disability Questions Tests 2010 Level. The 2011 NHIS Core components contain Household, Family, Person, Sample Adult, and Sample Child files. Each record in Part 1, Household Level, contains data on type of living quarters, number of families in the household responding and not responding, and the month and year of the interview for each sampling unit. Part 2, Family Level, is made up of reconstructed variables from the person-level data of the basic module and includes information on sex, age, race, marital status, education, veteran status, family income, family size, major activities, health status, health care costs, activity limits, and employment status, along with industry and occupation. As part of the basic module, Part 3, Person Level, provides information on all family members with respect to health status, limitation of daily activities, cognitive impairment, and health conditions. Also included are variables related to doctor visits, hospital stays, and health care access and utilization. Basic demographic information is provided as well.
A randomly-selected adult in each family was interviewed for Part 4, Sample Adult Level, regarding respiratory conditions, renal conditions, AIDS, joint symptoms, health status, health care and doctor visits, limitation of daily activities, and behaviors such as smoking, alcohol consumption, and physical activity. Part 5, Sample Child Level, provides information from an adult in the household on medical conditions of one child in the household, such as developmental or intellectual disabilities, respiratory problems, seizures, allergies, and use of special equipment like hearing aids, braces, or wheelchairs.
Parts 6 through 11 comprise the additional Supplements and Paradata for the 2011 NHIS. Part 6, Injury/Poison Episode, is an episode-based file that contains information about the external cause and nature of the injury or poisoning episode and what the person was doing at the time of the injury or poisoning episode, in addition to the date and place of occurrence. Part 7, Adult Disability Level and Part 8, Child Disability Level, are a supplemental set of six questions asked at the end of the Sample Adult and Sample Child Questionnaires for half of families that did not receive the Family Disability Supplement. These specific disability questions were only asked of the Sample Adult and the Sample Child. Part 9, Family Disability Level, seeks to identify the subpopulation that is at a greater risk than the general population of experiencing restrictions in social participation, for example, restrictions in employment, education, or civic life. Specific questions ask about respondent difficulty performing daily activities, such as dressing, bathing, or walking. Approximately one half of sample adults were selected to receive the Part 10, Adult Functioning and Disability Level Supplement. Questions were asked about a respondent's functioning in various basic and complex activity domains: vision, hearing, mobility, communication, cognition, upper body, affect, pain, and fatigue. This supplement also included questions designed to capture an individual's ability to participate in society. Follow-up questions on the degree of difficulty, use of assistive devices, and functioning with assistance were included for most domains. Part 11, Paradata Level, does not contain health related information, but rather data which are related to the interview process, including measures of time, contact-ability, and cooperation. National Health Interview Survey, 2012 (ICPSR 36146)
The National Health Interview Survey (NHIS) is conducted annually by the National Center for Health Statistics (NCHS), Centers for Disease Control and Prevention (CDC). The main objective of the NHIS is to monitor the health of the United States population through the collection and analysis of data on a broad range of health topics.
The NHIS contains many similar questions every year. The repeated items are called "core questions." Beginning with a new NHIS design in 1997, these core questions were divided into three components -- Family, Sample Adult, and Sample Child.
The 2012 NHIS data release consists of six core data files, a paradata file, the three Disability Questions Tests files, a Functioning and Disability file, and two Complementary and Alternative Medicine files. Users may see the Survey Description document for more details.
The 2012 NHIS contains the core questions, as well as enhanced questions on health care access and utilization. Supplemental topics are covered in the following questionnaires: the Family questionnaire covers subjects of food security; the Sample Adult questionnaire covers subjects of immunization, complementary and alternative medicine, non-cigarette tobacco use, voice, speech, and language; and the Sample Child questionnaire covers subjects of mental health, mental health services, immunization, complementary and alternative medicine, balance, voice, speech, and language. Along with the 2012 NHIS core data files are the Disability Questions Tests 2012 files which contain person-level data collected via a field test of six disability questions. These supplemental questions appeared on the NHIS, at the end of the Family, Sample Adult, and Sample Child Cores.
The Disability Questions Tests 2012 files are released as three separate files. A fourth disability supplement was also fielded in 2012 as part of the Sample Adult Core and is called "Adult Functioning and Disability Level."
The Adult and Child Alternative Health Supplement files were intended to expand on knowledge of alternative medical services. Questions focus on how often various types of alternative therapies are used, the associated costs, and the reasons they are used.
Lastly, the Paradata Level file contains information about the survey and data collection processes; included are data on response rates, keystrokes, interview times, and number of contact attempts.
National Health and Nutrition Examination Survey (NHANES), 2003-2004 (ICPSR 25503)
The National Health and Nutrition Examination Surveys (NHANES) is a program of studies designed to assess the health and nutritional status of adults and children in the United States. The NHANES combines personal interviews and physical examinations, which focus on different population groups or health topics. These surveys have been conducted by the National Center for Health Statistics (NCHS) on a periodic basis from 1971 to 1994. In 1999 the NHANES became a continuous program with a changing focus on a variety of health and nutrition measurements which were designed to meet current and emerging concerns. The surveys examine a nationally representative sample of approximately 5,000 persons each year. These persons are located in counties across the United States, 15 of which are visited each year.
For NHANES 2003-2004, there were 12,761 persons selected for the sample, 10,122 of those were interviewed (79.3 percent) and 9,643 (75.6 percent) were examined in the mobile examination centers (MEC). Many of the NHANES 2003-2004 questions were also asked in NHANES II 1976-1980, Hispanic HANES 1982-1984, NHANES III 1988-1994, and NHANES 1999-2002. New questions were added to the survey based on recommendations from survey collaborators, NCHS staff, and other interagency work groups. As in past health examination surveys, data were collected on the prevalence of chronic conditions in the population. Estimates for previously undiagnosed conditions, as well as those known to and reported by survey respondents, are produced through the survey. Risk factors, those aspects of a person's lifestyle, constitution, heredity, or environment that may increase the chances of developing a certain disease or condition, were examined. Data on smoking, alcohol consumption, sexual practices, drug use, physical fitness and activity, weight, and dietary intake were collected. Information on certain aspects of reproductive health, such as use of oral contraceptives and breastfeeding practices, were also collected. The diseases, medical conditions, and health indicators that were studied include: anemia, cardiovascular disease, diabetes and lower extremity disease, environmental exposures, equilibrium, hearing loss, infectious diseases and immunization, kidney disease, mental health and cognitive functioning, nutrition, obesity, oral health, osteoporosis, physical fitness and physical functioning, reproductive history and sexual behavior, respiratory disease (asthma, chronic bronchitis, emphysema), sexually transmitted diseases, skin diseases, and vision. The sample for the survey was selected to represent the United States population of all ages. Special emphasis in the 2003-2004 NHANES was on adolescent health and the health of older Americans. To produce reliable statistics for these groups, adolescents aged 15-19 years and persons aged 60 years and older were over-sampled for the survey. African Americans and Mexican Americans were also over-sampled to enable accurate estimates for these groups. Several important areas in adolescent health, including nutrition and fitness and other aspects of growth and development, were addressed. Since the United States has experienced dramatic growth in the number of older people during the twentieth century, the aging population has major implications for health care needs, public policy, and research priorities. NCHS is working with public health agencies to increase the knowledge of the health status of older Americans. NHANES has a primary role in this endeavor. In the examination, all participants visit the physician who takes their pulse or blood pressure. Dietary interviews and body measurements are included for everyone. All but the very young have a blood sample taken and see the dentist. Depending upon the age of the participant, the rest of the examination includes tests and procedures to assess the various aspects of health listed above. Usually, the older the individual, the more extensive the examination. Some persons who are unable or unwilling to come to the examination center may be given a less extensive examination in their homes.
Demographic data file variables are grouped into three broad categories: (1) Status Variables: provide core information on the survey participant. Examples of the core variables include interview status, examination status, and sequence number. (Sequence number is a unique ID assigned to each sample person and is required to match the information on this demographic file to the rest of the NHANES 2003-2004 data). (2) Recoded Demographic Variables: these variables include age (age in months for persons through age 19 years, 11 months; age in years for 1- to 84-year-olds, and a top-coded age group of 85 years of age and older), gender, a race/ethnicity variable, current or highest grade of education completed, (less than high school, high school, and more than high school education), country of birth (United States, Mexico, or other foreign born), Poverty Income Ratio (PIR), income, and a pregnancy status variable (adjudicated from various pregnancy related variables). Some of the groupings were made due to limited sample sizes for the two-year data set. (3) Interview and Examination Sample Weight Variables: sample weights are available for analyzing NHANES 2003-2004 data.
