Juvenile Residential Facility Census, 2022 [United States] (ICPSR 39418)
The Juvenile Residential Facility Census (JRFC), which is conducted biennially, collects basic information on juvenile residential facility characteristics, including security, capacity and crowding, injuries and deaths in custody, and facility ownership and operation. The JRFC also includes questions about facility type (such as detention center, training school, ranch, or group home) and residential services provided by the facility (such as independent living, foster care, or other arrangements), and detailed questions about mental health, substance abuse, and educational services provided to young persons.
In 2022, the JRFC was divided into seven sections:
- General facility information
- Mental health services
- Educational services
- Substance abuse services
- Events in the 30 days prior to the census reference date
- Deaths in the year prior to the census reference date
- Space shared with other facilities
Congress requires the Office of Juvenile Justice and Delinquency Prevention (OJJDP) to report annually on the number of deaths of juveniles in custody; the JRFC gathers this information and offers a portrait of the nation's juvenile facilities. The census reference date was the fourth Wednesday in October (October 26, 2022).
Assessing Washington State's Models to Increase Use of Medications for Opioid Use Disorder: Hub and Spoke and Low-Barrier Opioid Treatment Networks, 2015-2022 (ICPSR 39817)
MOUD remains an underutilized evidence-based practice with potential to reduce opioid use disorder (OUD) and save lives. Washington State expanded its Hub & Spoke (H&S) model for OUD medication treatment (MOUD: buprenorphine, methadone, naltrexone) by funding additional networks and developing a low-barrier, Opioid Treatment Network (OTN) model. The Washington H&S model was designed as a flexible approach incorporating primary care and substance use treatment programs, referral organizations, nurse care managers and care navigators. The OTN model offers MOUD induction in non-traditional settings (e.g., emergency departments, jails) with connections to community partners who will offer MOUD maintenance.
This study examined the implementation and effectiveness of the H&S and OTN models for increasing MOUD treatment and improving outcomes for people with OUD. Washington's H&S and OTN interventions were funded through federal STR/SOR opioid response grants, and the expanded H&S networks were funded through the state. The study, Hub and Spoke Opioid Treatment Networks: 2nd Generation Approaches to Improve Medication Treatment for Opioid Use Disorders (R01DA0561067), and its supplement (R01DA0561067S1) was part of NIH's HEAL initiative, and built on an earlier study (R33DA045851). All three studies were supported by the National Institute on Drug Abuse.
Expanding Access to Low-Barrier Opioid Use Disorder Treatment in Non-Traditional Settings: Washington's Opioid Treatment Network
- Increasing utilization of medications that treat opioid use disorders (MOUD) remains an essential strategy to curb the opioid crisis nationwide, especially among rural areas where access can present challenges. Washington State expanded access to MOUD through its opioid treatment networks (OTN), which provide low-barrier access to MOUD in non-traditional settings with an emphasis on buprenorphine and rural locations. The study examined changes in buprenorphine utilization between Medicaid beneficiaries who initiated treatment at OTNs compared to individuals outside OTN facilities and by rural-urban residence.
Differential Changes in Use of Medications for Opioid Use Disorder by Race-Ethnicity: Effects of a Hub-and-Spoke Model
- This study assessed whether delivery of opioid use disorder (OUD) treatment through a hub and spoke (HS) model is associated with better adherence to psychotropic medication treatment, compared to usual treatment. Washington State's HS model required each network to include at least one mental health program, so it was hypothesized that it would improve psychotropic medication adherence for people with both a mental health disorder (MHD) and an OUD.
Treatment for Comorbid Mental Health Disorders Among Patients Treated for Opioid Disorder: The Role of a Hub and Spoke Intervention
- The research team examined, separately for different racial-ethnic groups, whether use of medications for opioid use disorder (MOUD) increased more among people treated in a hub-and-spoke care model than among people treated in a non-hub-and-spoke model.

Governance of Learning Activities in Learning Healthcare Systems [Methods Study], United States, 2016-2021 (ICPSR 39711)
A learning health system, or LHS, is a health system that constantly looks for new ways to improve patient care. At an LHS, doctors and other hospital staff use learning activities to improve care, put what they've learned into practice, and share findings with other hospitals. Activities may include doing studies that compare treatments to see which one works better for which patients.
Governance refers to the way LHSs oversee learning activities. Governance includes people, committees, and policies that regulate learning activities. Including patients as partners in governance helps make sure learning activities address what's important to patients and protect patients' rights and interests. But LHSs don't always include patients in governance.
In this study, the research team wanted to learn how LHSs include patient partners in governance. The team interviewed patients and health system leaders to answer this question.
Randomize Everyone: Creating Valid Instrumental Variables for Learning Health Care Systems [Methods Study], New Hampshire, 2016-2022 (ICPSR 39717)
Comparative effectiveness research, or CER, compares two or more treatments. In some CER studies, researchers use patient data from electronic health records, or EHRs, to compare treatments. But patient traits like age may affect doctors' and patients' choice of treatments, which can bias results. Using EHR systems to identify eligible patients and assign them to treatments by chance could improve results of CER studies that use EHR data.
In this study, the research team explored the views of patients, clinic staff, and clinicians, such as doctors or nurses, on doing CER studies in clinics. The team also tested software with a widely used EHR system. The software finds patients who qualify for a study. During a clinic visit, the software prompts doctors to invite patients to take part in the study. If patients agree, the software assigns patients by chance to a treatment.
Evaluating the Comparative Effectiveness of Telemedicine in Primary Care: Learning from the COVID-19 Pandemic, New York, 2021 (ICPSR 39346)
During the COVID-19 pandemic, telemedicine emerged as the primary method of providing outpatient care in many regions with shelter-in-place and social distancing policies. This study aimed understand the impact of this rapid and widespread transition from in-person to remote visits on disparities in access to primary care, especially in chronic disease where ongoing communication between providers and patients is essential.
The newly developed or expanded telemedicine programs varied widely, raising questions about the effect of these differences on uptake of telemedicine among different patient populations and on patient-centered outcomes. Leveraging a natural experiment approach, this study examined rapidly changing telemedicine and in-person models of care during and after the COVID-19 crisis to determine whether certain patients could safely choose to continue telemedicine or telemedicine-supplemented care, rather than return to in-person care.

Feedback from Tribal communities were gathered about a future Tribally specific near real-time opioid overdose monitoring dashboard. A questionnaire about an example dashboard with questions about overdose information, Narcan usage, and feedback about the dashboard's uses were included.
Integrated Smoking Cessation Treatment for Smokers with Serious Mental Illnesses, Massachusetts, 2017-2020 (ICPSR 39152)
In the United States, tobacco smoking is associated with significant morbidity and premature mortality for individuals with serious mental illness (SMI) (e.g., schizophrenia, post-traumatic stress disorder, bipolar disorder, major depressive disorder). While many smokers with SMI wish to quit smoking, few are offered advice or treatments with demonstrated effectiveness in reducing tobacco dependence, primarily medication-assisted treatments. The overall aim of this randomized controlled trial was to test the effects of provider education (PE) (i.e. provider-level educational intervention focused on evidence-based smoking cessation treatment for those with SMI) and community health worker (CHW) support on the provision and utilization of smoking cessation treatment to those with SMI, and cessation rates for adults with SMI who smoke or use tobacco over a 2-year period. The objectives of this trial were to:
- Examine whether an intervention combining PE and CHW support would increase prescriber provision of advice and assistance to quit smoking, and improve tobacco cessation rates in smokers with SMI compared to usual care/treatment as usual (TAU) and compared to PE-only treatment
- Determine the effect of the combined PE+CHW intervention on patient-reported overall health compared to TAU and PE-only treatment
Eligible individuals were recruited from two outpatient psychiatric service providers in the Boston, Massachusetts metropolitan area. Clinics where individuals received services were randomized into either the TAU condition or into the PE condition, where health care providers would receive additional education on first-line medications used to treat tobacco use disorder. Within clinics in the PE arm, individuals were further randomized into the community health worker (CHW) support condition (PE+CHW), where CHWs would assist participants with smoking cessation care access and provide community outreach and education, or no CHW support (PE-only). Enrolled participants (n=1,010) completed surveys on smoking/tobacco use at 3 timepoints: study baseline, 1 year post-randomization, and 2 years post-randomization.
A mixed-methods evaluation of the trial was also conducted post-intervention, using an interactive convergent design. The aims of the evaluation were to identify barriers and facilitators to effective implementation; examine how primary care providers differed by performance and engagement level, and how experiences with the intervention compared across these groups; and identify anticipated barriers to implementing the intervention as discussed by stakeholders. Quantitative outcome and visit data from the trial were used in the evaluation. For the evaluation's qualitative component, interviews were conducted with purposively sampled community health workers, smoker participants, primary care providers, and other stakeholders in policy, payor, and clinical administration. Comparing Ways to Monitor Patients with COVID-19 at Home (COVID Watch), New Jersey, Pennsylvania, Delaware, 2020-2021 (ICPSR 38951)
The University of Pennsylvania Health System (Penn Medicine) developed COVID Watch, an automated text message-based, remote monitoring program with 24/7 clinical support. Remote outpatient monitoring of patients with COVID-19 became needed because patients with SARS-CoV-2 infection can decline rapidly and unpredictably, and because of their own limited capacity to manage acute symptoms and concerns about staff safety, office-based outpatient practices often redirect patients with confirmed or suspected COVID-19 to hospitals. As a result, emergency departments (EDs) and hospitals became overwhelmed during surge periods of high community incidence rates and prevalence. Remote monitoring has the potential to facilitate ED- and hospital-level care for patients who require it while supporting access to care for patients who can safely remain at home.
This study compared outcomes for patients enrolled in COVID Watch with those of patients who were eligible to enroll but received usual care, with the hypothesis that enrollment in COVID Watch was associated with reduced mortality. The present research examined whether patients with COVID-19 who were enrolled in COVID Watch experienced better health outcomes compared with usual care (Aim 1) and whether augmenting COVID Watch with at-home monitoring of SpO2 (blood-oxygen saturation) improves patient outcomes (Aim 2).

Protecting the Mental and Physical Well-Being of Frontline Healthcare Workers During COVID-19, United States, 2021-2022 (ICPSR 38816)
Integrating Patient-Centered Exercise Coaching into Primary Care to Reduce Fragility Fracture (WISE), Pennsylvania, 2016-2021 (ICPSR 38919)
Using a pragmatic trial design to limit exclusions, the investigators conducted a 36-month multi-center randomized effectiveness trial to compare the impact of an enhanced usual care (control) intervention, with exercise coaching (exercise), on fragility fractures and serious fall-related injuries (FF/SFRI) in patients with a previous fragility fracture. Specifically, the investigators examined the impact of the intervention on social loneliness, physical function, and bone strength. 1,139 individuals over 65 with a history of fragility fractures and/or osteoporosis were recruited over two years across three regions of Pennsylvania and randomized into either the enhanced usual care control group or exercise with coaching treatment group, where in-person exercise activities were led by trained volunteers.
Dataset (DS) 1 contains the following data used for analysis: participant characteristics at baseline by study group (referred to as Table 5 in the documentation), intervention participant characteristics at baseline based on exercise session type (referred to as Table 6), cumulative incidences of first serious fall-related injury compared by study group (referred to as Figure 3), cumulative incidence for first serious fall-related injury by age, gender, race, and osteoporosis medication (referred to as Table 8 and Figure 4), and cumulative incidence for first series fall-related injury by tertile of average intervention sessions per month (referred to as Figure 5). Other datasets used for analysis are fall injury data (DS2), monthly workout sessions data (DS3), secondary outcomes data (DS4, referred to as Table 7), and adverse events data (DS5, referred to as Table 9). DS6 includes markers designating before and after the start of the COVID-19 pandemic (March 11, 2020), allowing for analyses of participants who experienced fall-related injuries relative to COVID-19.
Datasets labeled "Miscellaneous" were not used in any analysis. These datasets contain extra measures from screening (DS7), baseline assessments (DS8), 4-month check-in visits (DS9), participant's distance to study site (DS10), coaching check-ins for weeks 1-12 (DS11), exercise sessions by month (DS12), adverse events (DS13), and end of study information (DS14).

Produced by matching data from the last four sources to the NACCHO data, the data file contains one case for each of the 2,107 local health departments (LHD) that responded to the NACCHO survey. Each LHD's record in the file includes the ASTHO data for its state health department and the NALBOH data for its local board of health (LBH), if it had a LBH and the LBH responded to the NALBOH survey. (If a LHD had multiple LBHs, then the first one in the NALBOH data was matched to the LHD). In addition, county (or county equivalent)-level data from the County Health Rankings and Census Demographic Profile Summary File were matched to the records of the 1,535 LHDs represented in the data file with a jurisdiction covering a single county or county equivalent.

This is the second round of the Community Partnerships for Older Adults (CPFOA) Program Survey of Older Adults. Like the first round, which was fielded in 2002 and released as ICPSR 4301 (Community Partnerships for Older Adults (CPOA) Program Survey of Older Adults, 2002), the second round was conducted as part of the evaluation of the CPFOA Program, an initiative of the Robert Wood Johnson Foundation (RWJF) aimed at promoting improvements in the organization and delivery of long-term care and supportive services for older adults through local public-private partnerships. The 2002 survey was conducted in the 13 communities in which partnerships received development grants from RWJF, and, in 2008, the survey was repeated in the eight of them in which partnerships received implementation grants from the Foundation. The goal of the survey was to improve understanding of the characteristics of older adults, their knowledge and perceptions about issues related to long-term care, and their use of long-term care services and support. In addition, the data collected by the survey enabled the communities to target the partnership's activities in the most effective way.
The second round was based on the 2002 survey instrument. Changes to the instrument were minimized so that the data from the 2002 and 2008 rounds would be comparable. The instrument was modified to delete questions that had low item response in 2002, to add questions requested by the partnerships, to add questions for decision-makers, or to modify questions that were outdated. As in 2002, the 2008 survey interviewed respondents about supportive and long-term care services for older adults in their communities, including the availability, use of, and quality of the services and sources of information about them. Respondents were asked if they expected to stay in their community, if their homes needed repairs or modifications to improve their ability to live in them, how important it was to be able to live in their own home as they grew older, the age at which they thought they would need help to continue living in their own home, and the age at which they thought they could no longer live at home because of health problems. The survey also collected information on health status, problems with activities of everyday life, health insurance coverage and long-term care insurance, hospital stays, living arrangements, social activities, support from family and friends, access to transportation, and demographic characteristics.

National Survey of Small and Medium-Sized Physician Practices (NSSMPP), 2007-2009 (ICPSR 36113)
Funded by the Robert Wood Johnson Foundation, the National Study of Small and Medium-sized Physician Practices (NSSMPP) was designed to provide information about physician practices with 1-19 physicians. The survey focused on the use of information technology and care management processes for four major chronic illnesses: asthma, congestive heart failure, depression, and diabetes. Other topics covered by the survey include practice type, size, ownership and the breakdown of patients by race and Hispanic origin; clinical preventative services and health promotion; health insurance plan activities in preventative care and care for patients with chronic illness; performance reporting and incentives; revenue sources and compensation methods; and organizational culture.
NSSMPP was also designed to assist the RWJF-funded Aligning Forces for Quality (AF4Q) project by providing baseline data about small and medium sized practices in the AF4Q sites. AF4Q was a national program that aimed to lift the quality of health care in 16 targeted communities, reduce racial and ethnic disparities in those communities and provide models for national reform.
NSSMPP built on two previous studies -- the National Study of Physician Organizations and the Management of Chronic Illness (NSPO), 2000-2001 (ICPSR 4455) and the National Study of Physician Organizations and the Management of Chronic Illness II (NSPO2), 2006-2007 (ICPSR 29801) -- which collected information about medical groups and independent practice associations (IPAs) with 20 or more physicians.

This study comprises the second round of the physician survey component of the Community Tracking Study (CTS) sponsored by the Robert Wood Johnson Foundation. The CTS is a national study designed to track changes in the American health care system and the effects of the changes on care delivery and on individuals. Central to the design of the CTS is its community focus. Sixty sites (51 metropolitan areas and 9 nonmetropolitan areas) were randomly selected to form the core of the CTS and to be representative of the nation as a whole. As in the first round of the physician survey (COMMUNITY TRACKING STUDY PHYSICIAN SURVEY, 1996-1997: [UNITED STATES] (ICPSR 2597)), the second round was administered to physicians in the 60 CTS sites and to a supplemental national sample of physicians. The survey instrument collected information on physician supply and specialty distribution, practice arrangements and physician ownership of practices, physician time allocation, sources of practice revenue, level and determinants of physician compensation, provision of charity care, career satisfaction, physicians' perceptions of their ability to deliver care, views on care management strategies, and various other aspects of physicians' practice of medicine. In addition, primary care physicians (PCPs) were asked to recommend courses of action in response to some vignettes of clinical presentations for which there was no prescribed method of treatment.
Dataset 3, the Site and County Crosswalk Data File, identifies the counties that constitute each CTS site.
Dataset 4, the Physician Survey Summary File, contains site-level estimates and standard errors of the estimates for selected physician characteristics, e.g., the percentage of physicians who were foreign medical school graduates, the mean age of physicians, and the mean percentage of patient care practice revenue from Medicaid.

Early Intervention Colorado (EI-CO) Participant Characteristics, Service Use, and Patient-Reported Outcomes, Colorado, 2017-2021 (ICPSR 37320)
The Individuals with Disabilities Education Act authorizes states to create early intervention (EI) programming to provide developmental and therapeutic services to infants and toddlers with a developmental condition; EI programs are mandated to report on child and family outcomes for purposes of accountability and quality improvement. For both purposes, there is critical need for research evidence on the adequacy of EI services. The researchers for this study partnered with a large urban EI program in Colorado that had recently transitioned to electronic data capture and was implementing a statewide initiative to facilitate function-focused care with individual families and paired collected electronic patient-reported outcomes (e-PRO) data with EI administrative data on child and family characteristics and service use to more fully examine EI service adequacy relative to patient-important outcomes.
This study collected information from Young Children's Participation and Environment Measure (YC-PEM e-PRO) on home, environment, and community participation, involvement, and desired change. Measurements were also collected from the Pediatric Evaluation of Disability Inventory Computer Adaptive Test (PEDI-CAT e-PRO) on mobility, social cognitive, and daily activities domains. The Child Outcomes Summary (COS) was used to capture functional performance related to having positive social relationships, acquiring and using knowledge and skills, and taking appropriate action to meet needs. Early Intervention Service Use measured EI intensity, whether child received specific services including physical therapy (PT), occupational therapy (OT), speech therapy (ST), psychology, or developmental intervention (DI), and the total number of service hours received within each discipline. Demographic variables include reasons child received EI services, number of developmental delay conditions, age, race, and ethnicity; as well as caregiver's education, employment, number of children at home, income, and insurance type.

The Behavioral Risk Factor Surveillance System (BRFSS) is a system of health-related telephone surveys that collect state data about U.S. residents regarding their health-related risk behaviors, chronic health conditions, and use of preventive services. Established in 1984 with 15 states, BRFSS now collects data in all 50 states as well as the District of Columbia and three U.S. territories. BRFSS completes more than 400,000 adult interviews each year.
Tanzania Abortion Incidence Study, 2012-2014 (ICPSR 38812)
Unsafe abortions account for roughly 13 percent of maternal deaths worldwide. In addition, millions of women suffer nonfatal health consequences of unsafe abortion every year. Information on unsafe abortion can be instrumental in helping policy makers and program planners identify measures needed to improve maternal health. Insufficient information at the country level perpetuates the invisibility of the problem of unsafe abortion and results in governments giving it little priority in policy decisions, service delivery or program implementation.
This project was the first-ever national study of the incidence of induced abortion and health consequences of unsafe abortion in Tanzania. The three-year project aimed to:
- Provide evidence on the incidence of induced abortion in Tanzania, at the national and regional levels and by women's place of residence and economic status
- Document the incidence of abortion complications
- Document the methods and providers used to perform abortions in Tanzania

The National Drug Abuse Treatment System Survey (NDATSS) is a longitudinal program of research into organizational structures, operating characteristics, and treatment practices of outpatient drug treatment programs in the United States. This is done through interviews with program directors and clinical supervisors. In some publications, this research is referred to as the Outpatient Drug Abuse Treatment Studies (ODATS). Data in this collection include Wave V, Wave VI, Wave VII, Wave VIII, and Wave IX.
NDATSS includes four prior waves of data collection from substance abuse treatment programs surveyed in 1984, 1988, 1990, and 1995. Waves II through IV can be found at ICPSR here.
Wave I is not planned for public release because it had a significantly different sample design than the other waves.

A Qualitative Assessment of Post-Partum Screening After Gestational Diabetes, St. Louis, 2017-2018 (ICPSR 38543)
This study is a qualitative examination via in-depth interviews and focus groups of barriers and facilitators to receiving postpartum diabetes screenings. The target population is women aged 18-40 who had a history of gestational diabetes within the prior 10 years and received Medicaid during pregnancy. Perspectives from patients, health care providers, and health care staff were solicited for the study. Overall, the goal was to learn about the patient, provider, and staff experience after a pregnancy complicated by gestational diabetes in order to understand how health centers may better support patients receiving recommended follow-up screenings and preventing postpartum type 2 diabetes.
All healthcare providers and staff were recruited from federally qualified health centers (FQHCs) in Missouri and provided care for women during and after pregnancies with gestational diabetes. Patients were recruited via health care centers or other community sites in the St. Louis metropolitan area. Interview and focus group questions assessed understanding and education provided around gestational diabetes diagnosis and postpartum screening/prevention, as well as prominent barriers and facilitators to gestational diabetes management, postpartum diabetes screening, and diabetes prevention.

Study to Promote Innovation in Rural Integrated Telepsychiatry (SPIRIT), Arkansas, Michigan, and Washington, 2016-2020 (ICPSR 38542)
This study addressed whether it is better to expand the scope of collaborative care programs to treat patients with more complex psychiatric disorders or to facilitate successful referrals to specialty mental health care. The primary objective of this study is to compare Telepsychiatry Collaborative Care (TCC) and Telepsychiatry Enhanced Referral (TER) from the patient and provider perspective. The secondary objective is to determine whether patients not engaging and responding to TER, improve with Phone-Psychiatry Enhanced Referral (PER). There are four specific aims.
Aim #1: To quantitatively compare the treatment experience, engagement, self-reported clinical outcomes, and recovery-oriented outcomes of patients initially randomized to TCC and TER.
Aim #2: For the subset of patients randomized to TER who do not engage in treatment and are still symptomatic at 6 months, quantitatively compare treatment experience, treatment engagement, self-reported clinical outcomes and recovery-oriented outcomes of patients randomized to continued-TER or PER.
Aim #3: To gain an in-depth understanding of patients' and providers' treatment experience, qualitatively compare those randomized to TCC, TER and PER.
Aim #4: To examine treatment heterogeneity among subgroups of patients randomized to TCC and TER based on race/ethnicity, age and clinical severity.
Reducing Barriers to Autism Care for Latino Children, Oregon, 2012-2013 (ICPSR 38525)
This collection includes qualitative data files from two parts of the same study that examined the barriers to autism care for Latino children in Oregon.
- Part 1: The research team sought to describe community, family, and health care system barriers to diagnosing autism spectrum disorders (ASD) in Latino children. Five focus groups and four individual interviews were conducted with 33 parents of Latino children previously diagnosed with an ASD. Participants described Latino community perceptions of autism and barriers they experienced during the diagnostic process.
- Part 2: The research team sought to assess the understanding and conceptualization of ASD in the Latino community to understand potential community barriers to early diagnosis. Five focus groups and four individual interviews were conducted with 30 parents of typically developing Latino children. Participants were asked structured questions concerning video vignettes that follow a Latina mother from the time she begins to worry about her 3-year-old son's behaviors to the time he receives an ASD diagnosis.

The 2012 ASTHO Profile Survey is a survey conducted by the Association of State and Territorial Health Officials (ASTHO) to gather information on state, territorial, and freely associated state public health agencies (S/THAs) and their activities, structure, and resources. The survey aims to define the scope of state and territorial public health services, identify variations in practice among public health agencies, and contribute to the development of best practices in governmental public health. The 121-question instrument was disseminated electronically in October 2012 and completed by senior deputies at each S/THA. The survey closed in May 2013; the response rate was 96 percent among the 50 states and D.C., and 92 percent among all states, territories, and freely associated states.
