Data are derived from the National Establishment Time Series (NETS) database and are available for four geographies: Census Tract 2010, Census Tract 2020, ZCTA 2010, and ZCTA 2020.

Integrating Patient-Centered Exercise Coaching into Primary Care to Reduce Fragility Fracture (WISE), Pennsylvania, 2016-2021 (ICPSR 38919)

Using a pragmatic trial design to limit exclusions, the investigators conducted a 36-month multi-center randomized effectiveness trial to compare the impact of an enhanced usual care (control) intervention, with exercise coaching (exercise), on fragility fractures and serious fall-related injuries (FF/SFRI) in patients with a previous fragility fracture. Specifically, the investigators examined the impact of the intervention on social loneliness, physical function, and bone strength. 1,139 individuals over 65 with a history of fragility fractures and/or osteoporosis were recruited over two years across three regions of Pennsylvania and randomized into either the enhanced usual care control group or exercise with coaching treatment group, where in-person exercise activities were led by trained volunteers.

Dataset (DS) 1 contains the following data used for analysis: participant characteristics at baseline by study group (referred to as Table 5 in the documentation), intervention participant characteristics at baseline based on exercise session type (referred to as Table 6), cumulative incidences of first serious fall-related injury compared by study group (referred to as Figure 3), cumulative incidence for first serious fall-related injury by age, gender, race, and osteoporosis medication (referred to as Table 8 and Figure 4), and cumulative incidence for first series fall-related injury by tertile of average intervention sessions per month (referred to as Figure 5). Other datasets used for analysis are fall injury data (DS2), monthly workout sessions data (DS3), secondary outcomes data (DS4, referred to as Table 7), and adverse events data (DS5, referred to as Table 9). DS6 includes markers designating before and after the start of the COVID-19 pandemic (March 11, 2020), allowing for analyses of participants who experienced fall-related injuries relative to COVID-19.

Datasets labeled "Miscellaneous" were not used in any analysis. These datasets contain extra measures from screening (DS7), baseline assessments (DS8), 4-month check-in visits (DS9), participant's distance to study site (DS10), coaching check-ins for weeks 1-12 (DS11), exercise sessions by month (DS12), adverse events (DS13), and end of study information (DS14).

These data were collected as part of the evaluation of the Healthy School Program (HSP), a program that provides support to elementary, middle, and high schools in the United States as they work to create healthy school environments that promote physical activity and healthy eating for students and staff. HSP was created in 2006 by the Alliance for a Healthier Generation with funding from the Robert Wood Johnson Foundation. The HSP evaluation addressed both process and impact outcomes:

  1. Is the HSP technical assistance and training model effective in increasing the implementation of policies and programs that promote and provide access to healthier foods and more physical activity before, during and after school?

  2. Are there distinctive or common school-level characteristics that hasten or hinder school-level implementation of policies and programs that promote and provide access to healthy foods and physical activity in the school setting in HSP schools?

  3. Does participation in HSP contribute to an increase in healthy eating behaviors and physical activity participation among students? Does participation in HSP contribute to a decrease in body mass index (BMI) among students?

The evaluation used a mixed-method design incorporating both quantitative and qualitative components. The quantitative component of the evaluation was a longitudinal design that measured student changes in eating and physical activity behaviors and BMI and schools' implementation of policies and practices promoted by HSP. For the qualitative component the evaluation team conducted site visits in a sample of HSP schools.

Dataset 1 contains data on school characteristics, HSP engagement indicators, baseline and follow-up responses to the Healthy Schools Inventory, and indices derived from the Inventory for all HSP schools as of August 2011. The Inventory collected information about each school's adherence to the Healthy Schools Program Framework, a set of best practice guidelines that promote physical activity and healthy eating among students and staff.

Datasets 2, 4 and 6 contain data from baseline and follow-up administrations of the Healthy Schools Youth Survey questionnaire in three samples of HSP schools: students in grades 5-12 in the initial pilot cohort of HSP schools; students in grades 5, 8 and 10 in the 2007-2008 cohort of HSP schools in Boston, Massachusetts and Miami-Dade County, Florida; and students in grades 5, 8 and 10 or 11 in HSP schools in Arizona, Nevada and Prince George's County, Maryland. Topics covered by the Healthy Schools Youth Survey questionnaire include eating and physical activity habits, attitudes about healthy eating and physical activity, health knowledge, and school food environments.

Datasets 3 and 7 contain baseline and follow-up height and weight measurements and derived BMIs, the former for students in grades 4-12 in schools sampled by the Pilot Student Survey and the latter for students in grades 5, 8, and 10 in Arizona and grades 1-12 in Prince George's County in schools sampled by the Arizona, Prince George's County and Nevada Healthy Schools Youth Survey.

Dataset 5 is an update to Dataset 1. Like Dataset 1 it contains data on HSP participation and engagement and school characteristics. Dataset 5 covers 8,500 schools that participated in HSP through fall 2014. It includes 4,028 of the 4,542 schools in Dataset 1.

Dataset 8 contains average math, reading and language scores for grades in HSP and comparable non-HSP schools in Arizona. Every record in the data file represents a grade (one or more of the grades 2-9) within a school (150 schools) for a given school year (up to seven years 2007-2008 to 2013-2014).

Dataset 9 contains data from a survey of HSP school coordinators in Prince Georges County. The coordinators were interviewed about the implementation of HSP in their schools.

ICPSR did not receive the site visit data.

This survey was conducted as part of the New Jersey Childhood Obesity Study, a project designed to provide vital information for planning, implementing, and evaluating interventions aimed at preventing childhood obesity in five New Jersey municipalities: Camden, Newark, New Brunswick, Trenton, and Vineland. Conducted among households with 3-18 year old children in the 5 cities, the survey interviewed the adult who made most of the decisions about food shopping in each household. The survey examined perceptions about food and physical activity environments in the five cities, investigated barriers related to access to healthy food and physical activity facilities, and collected information on the height and weight and food and physical activity behaviors of the cities' 3-18 year old children and the adult respondents. In addition, the survey collected demographic information about the household members.

Four linkable datasets contain the survey data: the Household File, Index Child File, Adult File, and All Child File. The Household File covers household and neighborhood characteristics, while the Index Child File describes the characteristics and behaviors of a randomly selected 3-18 year old child in the household, who is designated the "index child" and is the primary unit of analysis. The Adult File comprises characteristics and behaviors of the adult respondent, and, lastly, the All Child File covers a few characteristics of all children aged 3-18 in the household.

National Impact Study: Best Practice Physical Activity Programs for Older Adults, 2004-2006 (ICPSR 23240)

This study was one of three modules in a larger study designed to define, identify, and disseminate information about best practice physical activity programming in community-based organizations. The National Council on the Aging (NCOA), together with the Robert Wood Johnson Foundation (RWJF), conducted Module 1, a national competition for NCOA Awards for Excellence in Physical Activity Programming. The purpose of the competition was to bring national attention to best practice in physical activity programming operated by local public or non-profit organizations that serve older adults. NCOA was assisted in this endeavor by the University of Illinois at Chicago (UIC), the Healthy Aging Research Network of the Centers for Disease Control and Prevention's Prevention Research Centers, and a variety of nationally known physical activity experts. Module 2 was a census of physical activity programs for older adults that was conducted in seven locations by Healthy Aging Research Network. Module 3 was this National Impact Study, a study of best practice programming at three community-based organizations serving older adults. It was designed to answer the research question, "Do best practice physical activity programs provided by community-based organizations have a measurable impact on the health and well being of participants?" The three studied organizations were among the ten winners of the best practice competition.

Data were collected from previously sedentary, volunteer participants from each of the three organizations. The goal was to enroll 250 older adults at each site for a total of 750 subjects across the three programs. At each site, one half of all subjects (n = 125) were to be assigned to the treatment group and the other half to a control group. Subjects in the treatment group participated in the best practice program classes while the control group could enroll in other exercise classes but not the best practice program classes. The subjects were to be interviewed three times: baseline, three months, and nine months. Each interview contained two parts: (1) the administration of health and exercise related questions and (2) the administration of four physical fitness tests that assessed the respondent's strength, flexibility and aerobic endurance. Background information collected by the survey includes age, education, employment status, marital status, income, race, and Hispanic origin.

Active for Life: Translation of Physical Activity Programs for Mid-Life and Older Adults, 2003-2007 [United States] (ICPSR 24723)

Sponsored by the Robert Wood Johnson Foundation, the Active for Life (AFL) initiative investigated how two physical activity programs for adults aged 50 and older, Active Choices (AC) and Active Living Every Day (ALED), worked in community settings. Created by researchers at Stanford University, Active Choices used lifestyle counseling and personalized telephone support to encourage older adults to be physically active. In AFL, this was a 6-month program delivered through one face-to-face meeting followed by up to eight one-on-one telephone counseling calls. Active Living Every Day, which was created by the Cooper Institute and Human Kinetics Inc., also provided lifestyle counseling to promote physical activity, but in a classroom and workbook format. During the first three years of the four-year AFL initiative, ALED was delivered as a 20-week program where participants attended weekly small group meetings, but in the last year it was shortened to 12 weekly meetings. Nine organizations received AFL grants to implement the programs during 2003-2006. Four grantees implemented the one-on-one AC model, while five implemented the group-based ALED model.

Data were collected from the AC and ALED sites for both a process and outcomes evaluation. The primary aims of the process evaluation were to (1) monitor the extent to which the grantees demonstrated fidelity to the AC and ALED models in their program implementation, (2) assess staff experiences implementing the programs, and (3) assess participants' impressions of the programs. A quasi-experimental, pre-post study design was used to assess outcomes. Primary aims of the outcomes evaluation were to evaluate the impact of AC and ALED on self-reported physical activity, and to evaluate the impact of the programs on self-reported stress, depressive symptoms, and satisfaction with body function and appearance. Secondary aims of the outcome evaluation were to (1) evaluate the impact of the programs on measures of functional fitness, (2) examine whether changes in self-reported physical activity and functional fitness were moderated by participant characteristics, including age, gender, race, baseline physical activity self-efficacy, and baseline physical activity social support, and (3) examine whether changes in self-reported physical activity were consistent with a mediation model for physical activity self-efficacy and physical activity social support.

The collection has 14 data files (datasets). Datasets 1-7 constitute the process evaluation data, and Datasets 8-14 the outcomes evaluation data:

Dataset 1 (AC Initial Face-to-Face Sessions Data) contains information about the initial face-to-face AC session: the format, date, and length of the session, whether the 8 steps required in the face-to-face session were completed, what was discussed between the health educator and the participant related to physical activity plans, interests, benefits, and barriers, and the health educator's progress notes. The file contains one record for each AC participant.

Dataset 2 (AC Completed Calls Data) comprises information about the completed AC calls, but does not cover the topics discussed on the calls. Recorded information about each call includes the date and length of the call, the health educator's progress notes, and whether the participant was assessed for injury, light activity, moderate activity, exercise goals, or exercise intentions. Each call is represented by a separate record in the data file and, typically, there are multiple records per participant.

Dataset 3 (AC Topics Discussed on Completed Calls ) contains information about the topics discussed on each completed AC call, e.g., exercise barriers/benefits, previous exercise experiences, goal setting, long term goals, injury prevention, rewards/reinforcement, social support, progress tracking, and relapse prevention. Each record in the file represents one topic and there are often multiple records per call for each participant.

Dataset 4 (AC Aggregate Call Data) aggregates the call data across calls for each AC participant. For example, for a given participant, this dataset shows the total number of calls completed, the number of calls where injury/health problems were assessed, etc. The file contains one record per participant.

Dataset 5 (ALED Sessions Data) contains information about each class session for every ALED group, including the session date, start time, and end time, learning activities covered in the session, participant evaluations of the session and the facilitator, facilitator progress notes, the number of participants who were in various stages of readiness for moderate exercise, and the number of participants who tracked physical activity and thoughts about physical activity. This file has one record for each session of every ALED group.

Dataset 6 (ALED Attendance and Tracking Data (Years 2-4)) consists of participant-level attendance and tracking data for every ALED session during the second to fourth years of the evaluation, including the participant's attendance at the session, whether the participant's stage of readiness was assessed, and whether the participant tracked thoughts about physical activity or actual physical activity. There is no participant-level ALED data for the first year. Each participant has a separate record for each session. Thus, the file contains 20 records per participant in the years 2-3, and 12 records per participant in year 4.

Dataset 7 (ALED Aggregate Attendance and Tracking Data (Years 2-4)) contains ALED attendance and tracking data for each participant in years 2-4, aggregated across the sessions. The data file has one record for each participant.

Dataset 8 (Demographics) comprises program information (e.g., program status, start date, end date, site, etc.), demographic information (e.g., age, gender, race, Hispanic origin, employment status, income, and the participant's state and ZIP code of residence), and responses to the Physical Activity Readiness Questionnaire (PAR-Q), a screening tool that was used to assess possible risks of exercising based on answers to specific health history questions. The file contains one record for each AFL participant, except for those with a status of "nonstarter" or "repeater."

Datasets 9 (Pretest Survey Data) and 10 (Posttest Survey Data) contain data from the Pretest and Posttest Surveys. The Pretest Survey was administered at the beginning of the AC and ALED programs, while the Posttest Survey was administered at their end. Topics covered by the surveys include social and recreational activities, activities undertaken for exercise, perceived stress, depressive symptoms, satisfaction with body appearance and function, social support for physical activity, self-efficacy for physical activity, neighborhood environment, health conditions, health-related quality of life, caregiving, and self-reported height and weight. Both surveys included items from the Community Health Activities Model Program for Seniors Physical Activity Questionnaire (CHAMPS), the Center for Epidemiological Studies Depression Questionnaire (CES-D), the Behavioral Risk Factor Surveillance System Questionnaires (BRFSS), and the International Physical Activity Prevalence Study Environmental Module. These data files each have one record for each participant who submitted a questionnaire.

Dataset 11 (ALED Week 12 Survey Data (Year 4)) contains responses to the ALED Week 12 Posttest Survey, which was used to evaluate the 12-week adaptation of ALED in Year 4. (In Year 4, ALED participants completed both a 12- and 20-week posttest survey). There is one record for each participant who returned this survey.

Dataset 12 (Six-Month Posttest Follow-Up Survey Data (Years 3-4)) comprises data from a special 6-month follow-up survey which was administered in years 3-4 in six of the ALED sites and one of the AC sites. Participants were questioned about their current physical activities, weight, health-related quality of life, satisfaction with bodily function, and other topics. As with Datasets 9-11, the data file contains one record for each participant who returned a questionnaire.

Dataset 13 (Functional Fitness Tests Data) contains the results of pretest and posttest functional fitness tests which were administered by one ALED grantee. Four tests were adminstered: (1) the 30-Foot Walk Test, (2) the 30-Second Chair Stand, (3) 8-Foot Up and Go, and (4) the Chair Sit and Reach Test. This participant-level data file also includes pretest height measurements plus pretest and posttest weight measurements.

Dataset 14 (Participants' Impressions of the Programs (Years 1, 3, and 4)) contains data collected by the last sections of the Posttest Survey, ALED Week 12 survey, and 6-Month Follow-up Survey. The topics it covers include the participants' impressions of the programs, participation in physical activities, and changes (compared to before they started the AFL program) in motivation to be physically active, actual level of physical activity, medical and health conditions, overall pain, flexibility/limberness, level of stress, happiness, and enjoyment of life. The file has a separate record for each survey completed by the participants. Thus, there are 1-3 records per participant.

El papel de la motivación en la práctica sistemática de las actividades físicas (ICPSR 184424)

A través de las diferentes observaciones a clases se han constatado diversas manifestaciones de complejidad en el comportamiento del proceso y los resultados de la enseñanza-aprendizaje de habilidades en la clase de educación física de la educación superior, lo que conllevó a la verificación de la dinámica del proceso y el estudio de los productos de la actividad física, identificándose un insuficiente estado de motivación en la práctica de los ejercicios físicos. Fenómeno que provoca retardo en el cumplimiento de los objetivos de aprendizaje previstos en el programa de educación física. Revertir tal situación es el propósito de dicha investigación. En tal sentido el presente trabajo tiene como propósito realizar una valoración del estado actual de motivación hacia la práctica de las actividades físicas, lo que permitiría ofertar actividades motivadoras. Para ello se tomó una muestra de 110 estudiantes de la carrera de Lenguas extranjeras a los cuales se les aplicaron diversas encuestas, entrevistas y test psicológicos para diagnosticar su estado de motivación que nos permita elaborar actividades motivadoras que garanticen la práctica sistemática del ejercicio físico. Las metodologías utilizadas son la cualitativa y la cuantitativa, las cuales determinaron los resultados de esta pesquisa.

The “Meditation or Exercise to Prevent Respiratory Infection (MEPARI-2)” trial is a longitudinal study of community dwelling adults (30-69 years old) from the greater Madison, Wisconsin area in the United States.  The study was conducted from August 2012 through July 2017.  Participants were recruited in yearly cohorts and followed for about nine months.  MEPARI-2 combines longitudinal survey data on participant’s social, economic, psychological and physical well-being with contextual data on whether training in mindfulness meditation or exercise can lead to reductions in acute respiratory infections (ARI).

The Sacramento Area Latino Study on Aging (SALSA Study) project tracked the incidence of physical and cognitive impairment as well as dementia and cardiovascular diseases in elderly Latinos in the Sacramento, California, region. The SALSA project aimed to assess cognitive, physical, and social functions, which include the ability to follow instructions, to perform certain movements, and to interact with others. The study explored the effects that cultural, nutritional, social, and cardiovascular risk factors have on overall health and dementia, and examined the association between diabetes and functional status. This study contains the neuroclinical exam data from the SALSA project. Demographic information includes age given at follow-up visits, country of birth, language, religion, marital status, educational level, occupation, household income, and size of household.

High Intensity Interval- vs Moderate Intensity- Training for Improving Cardiometabolic Health in Overweight or Obese Males: A Randomized Controlled Trial (ICPSR 100179)

Purpose: To compare the effects of six weeks of high intensity interval training (HIIT) vs continuous moderate intensity training (MIT) for improving body composition, insulin sensitivity (SI), blood pressure, blood lipids, and cardiovascular fitness in a cohort of sedentary overweight or obese young men. We hypothesized that HIIT would result in similar improvements in body composition, cardiovascular fitness, blood lipids, and SI as compared to the MIT group, despite requiring only one hour of activity per week compared to five hours per week for the MIT group. Twenty-eight sedentary overweight or obese men (age, 20 ± 1.5 years, body mass index 29.5 ± 3.3 kg/m2) participated in a six week exercise treatment. Participants were randomly assigned to HIIT or MIT and evaluated at baseline and post-training. DXA was used to assess body composition, graded treadmill exercise test to measure cardiovascular fitness, oral glucose tolerance to measure SI, nuclear magnetic resonance spectroscopy to assess lipoprotein particles, and automatic auscultation to measure blood pressure.

The Survey of Consumer Attitudes and Behavior series (also known as the Surveys of Consumers) was undertaken to measure changes in consumer attitudes and expectations, to understand why such changes occur, and to evaluate how they relate to consumer decisions to save, borrow, or make discretionary purchases. The data regularly include the Index of Consumer Sentiment, the Index of Current Economic Conditions, and the Index of Consumer Expectations. Since the 1940s, these surveys have been produced quarterly through 1977 and monthly thereafter. The surveys conducted in 2010 focused on topics such as evaluations and expectations about personal finances, employment, price changes, and the national business situation. Opinions were collected regarding respondents' appraisals of present market conditions for purchasing houses, automobiles, and other durables. Explored in this survey were respondents' types of savings and financial investments, loan use, family income, and retirement planning. This survey also asked respondents about financial and health literacy; adult and online education; and technology use in health, finances, travel, and communication. Additional questions on independent living communities and general feelings were asked. Other topics in this series typically include ownership, lease, and use of automobiles, respondents' use of personal computers at home and in the office, and respondents' familiarity with and use of the Internet. Demographic information include ethnic origin, sex, age, marital status, and education.

The Korean General Social Survey (KGSS) is the South Korean version of the General Social Survey (GSS), closely replicating the original GSS of the National Opinion Research Center at the University of Chicago. Each round of the KGSS typically includes the topical module surveys of the International Social Survey Programme (ISSP), and/or the East Asian Social Survey (EASS), an international survey network of four GSS-type surveys from countries in East Asia (including China, Japan, Taiwan, and South Korea). Respondents were asked about their trust of people and institutions, government performance, financial status, and overall health. The Behavioral Risk Factor Surveillance System (BRFSS) is a state-based system of health surveys that collects information on health risk behaviors, preventive health practices, and health care access primarily related to chronic disease and injury. For many states, the BRFSS is the only available source of timely, accurate data on health-related behaviors. BRFSS was established in 1984 by the Centers for Disease Control and Prevention (CDC); currently data are collected monthly in all 50 states, the District of Columbia, Puerto Rico, the United States Virgin Islands, and Guam. More than 350,000 adults are interviewed each year, making the BRFSS the largest telephone health survey in the world. States use BRFSS data to identify emerging health problems, establish and track health objectives, and develop and evaluate public health policies and programs. The BRFSS is a cross-sectional telephone survey conducted by state health departments with technical and methodologic assistance provided by CDC. States conduct monthly telephone surveillance using a standardized questionnaire to determine the distribution of risk behaviors and health practices among adults. Responses are forwarded to CDC, where the monthly data are aggregated for each state, returned with standard tabulations, and published at the year's end by each state. The BRFSS questionnaire was developed jointly by CDC's Behavioral Surveillance Branch (BSB) and the states. When combined with mortality and morbidity statistics, these data enable public health officials to establish policies and priorities and to initiate and assess health promotion strategies.

The Eurobarometer series is a unique cross-national and cross-temporal survey program conducted on behalf of the European Commission. These surveys regularly monitor public opinion in the European Union (EU) member countries and consist of standard modules and special topic modules. The standard modules address attitudes towards European unification, institutions and policies, measurements for general socio-political orientations, as well as respondent and household demographics. The special topic modules address such topics as agriculture, education, natural environment and resources, public health, public safety and crime, and science and technology.

This round of Eurobarometer surveys covers the special topic of public health and includes the following major areas of focus: (1) check-up and prevention, (2) oral health, (3) alcohol habits, (4) smoking habits, (5) organ and blood donation, and (6) sport and physical activity. Questions pertain to medical tests and health exams received in the past 12 months, eating a healthy diet, exercise, visits and access to a dentist, and food and drink consumption. Other questions addressed alcohol consumption within the past 12 months, smoking status and behavior, as well as the knowledge of human organ donation and transplant, and safety of blood transfusions compared to 10 years ago.

Demographic and other background information collected includes age, gender, nationality, marital status, occupation, age when stopped full-time education, household composition, ownership of a fixed or a mobile telephone and other durable goods, difficulties in paying bills, level in society, and Internet use. In addition, country-specific data includes type and size of locality, region of residence, and language of interview (select countries).

The SALSA project aimed to assess cognitive, physical, and social functions, which include the ability to follow instructions, to perform certain movements, and to interact with others. The basic purpose of the National Health Interview Survey is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive. There are five types of records in the core survey, each in a separate data file. The variables in the Household File (Part 1) include type of living quarters, size of family, number of families in household, and geographic region. The variables in the Person File (Part 2) include sex, age, race, marital status, veteran status, education, income, industry and occupation codes, and limits on activity. These variables are found in the Condition, Doctor Visit, and Hospital Episode Files as well. The Person File also supplies data on height, weight, bed days, doctor visits, hospital stays, years at residence, and region variables. The Condition (Part 3), Doctor Visit (Part 4), and the Hospital Episode (Part 5) Files contain information on each reported condition, two-week doctor visit, or hospitalization (twelve-month recall), respectively. A sixth, seventh, and eighth file have been added along with the five core files. The Health Promotions and Disease Prevention Supplement is separated into three categories as follows: Child Safety/Infant Feeding (Part 6), Sample Person (Part 7), and Smoking (Part 8). These data files include questions on health and fitness awareness, general health habits, injury control, child safety and health, high blood pressure, stress, exercise, smoking, alcohol use, dental care, and occupational safety and health.

Prescription for Health Evaluation: Practice Information Form Data, 2005-2007 [United States] (ICPSR 27041)

Prescription for Health was an initiative funded by the Robert Wood Johnson Foundation in collaboration with the Agency for Healthcare Research and Quality. Under this initiative, primary care practice-based research networks (PBRNs) -- groups of ambulatory practices devoted principally to the primary care of patients -- developed, tested, and evaluated innovative strategies to improve the delivery and effectiveness of health behavior change services in primary care practice. The strategies targeted four health risk behaviors: tobacco use, unhealthy diet, lack of physical activity, and risky alcohol use. Prescription for Health was conducted in two rounds. Round one awarded grants to 17 PBRNs to test the feasibility of implementing the strategies, while round two awarded grants to ten PBRNs to measure the strategies' effectiveness and the expenses associated with them. More than a 100 primary care practices from the ten PBRNs participated in the evaluation.

This data collection comprises the data from one of the data collection efforts carried out by the second round: the responses to the Practice Information Form (PIF), a Web-based instrument which captured key organizational attributes of the participating practices. The PIF data were collected at two time points. Baseline data were collected from each practice before the intervention was implemented and follow-up data were collected approximately one year after the start of the intervention.

Information about the practices collected by the PIF include practice type and ownership; characteristics of each clinician and non-clinician staff person; number of vacancies for clinicians and non clinicians; number of exam rooms and volume of office visits; average number of new patients per month; percentages of patients in various age, race, Hispanic origin, and payer categories; and the predominant type of payment arrangement with health plans. In addition, the PIF asked whether a specific health plan controlled over half of the practice's total business; whether the practice had a pay-for-performance program; whether any payers or organizations publicly reported practice level performance information, such as patient satisfaction, chronic care/disease management, and/or preventive service delivery; and whether practices had a formal process for routinely measuring satisfaction among patients, clinicians, and other staff. The PIF also investigated how practices motivated their clinicians and staff; the level of competition among practices in local markets; the use of computers, electronic medical record systems, and patient registries; major changes that affected each practice's ability to make improvements in patient care; factors that prevented practices from translating the results of research into changes in medical practice; and the use of health risk assessment protocols or questionnaires to identify patients who may benefit from counseling or interventions. Questions about the four Prescription for Health behaviors -- physical activity, healthy eating, smoking cessation, and addressing risky drinking -- asked how practices linked patients to outside resources for each of the four behaviors; how practices used evidence-based guidelines and informed patients about recommendations for the behaviors; and which approaches practices used to support patients ready to engage in a behavior change for each of the behaviors.

This round of Eurobarometer surveys diverged from the standard Eurobarometer measures and queried respondents on (1) foreign languages, (2) biotechnology, (3) organized crime and corruption, (4) health consciousness, (5) smoking, (6) AIDS prevention, (7) medical errors, and (8) consumer rights. For the first topic, foreign languages, respondents were asked to identify their native language, and first, second, and third foreign languages spoken, including proficiency and frequency of use. In addition, respondents were asked to identify the main reasons to learn a new language, methods used in learning, and barriers preventing learning. Respondents' opinions were sought regarding the best age to start learning a first and second new language, language support, and whether there should be a common language used throughout the European Union (EU). For the second topic, respondents were asked about their understanding of biotechnology, including gene therapy, pharmacogenetics, genetically modified foods and plants, nanotechnology, stem cell research, and its application in industry. Respondents' opinions were sought regarding the use of these techniques, governing safety and regulatory processes, new technology development, and integration of biotechnology into society. Respondents were also queried about their knowledge of science and politics and discussion of these matters with others, their opinions regarding entity involvement, including the EU, in utilizing or advancing biotechnology, and their personal political involvement in this area. For the third topic, organized crime and corruption, respondents were asked to identify the degree of national corruption, sources where corruption exists, a regulatory force in reducing it, and any personal involvement with corruption, in addition to providing an opinion about whether information sharing or policy development may reduce corruption. For the fourth topic, health consciousness, respondents were asked about their current state of health, breastfeeding, dieting, views on eating, foods consumed, changes in eating or drinking patterns and associated reasons for these changes, ease of and barriers to eating healthily, and exercise. Respondents were asked about their knowledge of sports and physical activity, and their opinion about obesity among adults and children. For the fifth topic, smoking, respondents were asked about their smoking habits and use with other substances, sensitivity to smoke, knowledge about second-hand smoke, exposure to tobacco cessation campaigns, and the likelihood of quitting. In addition, respondents were asked to provide an opinion about smoking bans in public places and the consumption of alcohol and tobacco among pregnant women. For the sixth topic, AIDS prevention, respondents were asked about their knowledge of AIDS transmission, changes in personal behaviors influenced by AIDS, and their opinions regarding current national measures in managing the AIDS pandemic and the potential coordination with the EU. For the seventh topic, medical errors, respondents were asked about their awareness of incidents of medical errors in their country, the significance of those errors, personal experience of a medical error, the likelihood of avoiding an error, and their degree of concern about suffering a medical error. For the eighth and final topic covered by this survey, consumer rights, those respondents living in Poland were asked about where and how often they saw or heard information about consumer rights, how frequently the media talked about consumer rights, and who in the media was the source of this information. Respondents were also asked whether they had heard a particular message and to define the meaning of that message, to evaluate Poland's consumer rights in comparison to other EU countries, and to assess the effectiveness of the justice system in protecting consumer rights. In addition, respondents were queried about their knowledge of consumer rights in certain situations, which organizations they would trust to provide correct advice and information about consumer rights, and whether they would refer others to a specific organization that deals with consumer rights, Federacja Konsumentow. This poll is part of a continuing series of monthly surveys that solicit public opinion on the presidency and on a range of other political and social issues. Respondents -- all Blacks/African Americans -- were asked to give their opinions on the 2004 presidential campaign and the candidates, the way the Bush administration was handling certain issues, and the war in Iraq. Questions were asked regarding respondents' confidence that their votes would be accurately counted, whether there was a deliberate attempt to prevent African Americans from voting or having their votes properly counted, how the voting problems reported in Florida in the 2000 presidential election would affect voter turnout, and which candidate had more 'soul'. Respondents were also asked about various issues facing the country, such as how to provide African Americans with more jobs, the best way to help more African Americans go to college, and whether the United States should intervene when crises occur in Africa. Additional questions queried respondents' health behavior, exercise patterns, experiences with low carbohydrate diets, and attitudes toward reinstating the military draft. This special topic poll, conducted May 10-16, 2004, was undertaken to assess public opinion on the problem of obesity in the United States. Respondents were asked to rate their own personal health and the importance of a healthy diet and physical exercise in their lives. Questions were posed regarding how often respondents and their children watched television, used the Internet, e-mail, or computer games for personal use, and engaged in moderate or vigorous physical exercise, how much influence their children had on what food the family ate, whether they kept a supply of fresh fruits and vegetables in their home, and whether they or their children were overweight. Respondents were polled on how often they ate at fast-food restaurants, whether they tried to track the amount of calories, carbohydrates, and fat content in their daily diets, whether they wanted to lose or gain weight, whether they had followed any type of weight-loss program, and whether it had helped them. Several questions asked about the hardest part of losing weight, the biggest causes of obesity in the United States, whether respondents had ever felt that they were discriminated against because of their weight, whether they had any negative feelings about people who were overweight, and whether certain groups or institutions such as the fast-food industry and government policies and laws bore any responsibility for the nation's obesity problem. Respondents were also polled on whether they would support or oppose government policies such as a tax on high-fat or high-sugar foods, requiring labels on certain foods warning of the health risks of being overweight, and setting a legal limit on portion sizes in restaurants. Other topics addressed whether health insurance companies should be able to drop coverage or charge higher premiums to people who are overweight, whether schools should be allowed to raise money by allowing soda and candy vending machines in school, the seriousness of problems such as cancer, AIDS, cigarette smoking, obesity, and drug and alcohol abuse as public health issues, and whether the federal government was doing enough about these problems. Background information includes sex, ethnicity, education, marital status, household income, number and sex of children in household, weight and height of respondents and children, and subjective size of the community: rural, urban, or suburban.

The purpose of the National Health Interview Survey (NHIS) is to obtain information about the amount and distribution of illness, its effects in terms of disability and chronic impairments, and the kinds of health services people receive. In 1985, the NHIS questionnaire included a special section, the Health Promotions and Disease Prevention (HPDP) supplement, that queried female respondents aged 18 through 44 who had had live births in the past five years about smoking during pregnancy. Their responses to the supplement are recorded in this dataset, along with other information about them derived from the HPDP and the 1985 core questionnaire (see HEALTH INTERVIEW SURVEY, 1985 [ICPSR 8668]). Questions on smoking history during pregnancy included current pregnancy status, whether the respondent had ever smoked 100 cigarettes, whether she currently smoked, time elapsed since the respondent last smoked regularly, number of cigarettes the respondent smoked per day, whether the respondent smoked during pregnancy, number of cigarettes smoked before learning of pregnancy, number of cigarettes smoked after learning of pregnancy, whether the respondent smoked during most of last pregnancy, and whether the respondent was ever advised by a doctor to quit smoking. Other variables in the HPDP focus on health and fitness awareness, general health habits, injury control, child safety and health, high blood pressure, stress, exercise, smoking, alcohol use, dental care, and occupational safety and health. Variables from the core questionnaire include height, weight, age, race, sex, Hispanic origin, type of living quarters, region and metropolitan status of residence, marital status, veteran status, education, family income, health status, industry, occupation, activity limitation status, medical conditions, restricted activity days in the past two weeks, bed days in the past two weeks and past 12 months, time interval since the last doctor visit, and the number of doctor visits and short-stay hospital episodes in the past 12 months.

This survey focused on issues of nutrition and health. Respondents were asked whether they watched their diets to reduce their intake of salt, fat, sugar, and cholesterol. They also were asked a series of specific questions relating to cholesterol. In addition, respondents were questioned regarding the state of medical care in the country, their own health status, how often they exercised, and how they evaluated health care facilities, doctors, and medical personnel. Other topics covered include memory of people and events in the 1980s, such as Walter Mondale's running mate in 1984, the Iran-contra scandal, Gary Hart, professional sports figures, and the names of individuals or organizations that won the Nobel Peace Prize in the period. Background information on respondents includes political alignment, 1988 presidential vote choice, education, age, religion, social class, marital status, household composition, labor union membership, employment status, race, sex, income, and state/region of residence.

This data collection is a telephone survey of a national probability sample. It was designed to provide data for examining the relationships between health practices and physical health status. The survey was conducted in two waves of interviews among a panel of noninstitutionalized civilian adults aged 20-64 years in the coterminous United States. Wave I was conducted in the spring of 1979 and comprises data for 3,025 respondents. Wave II was conducted one year later and contains data for 2,436 respondents . The file contains data from both waves. Included are questions on diet, exercise, sleeping, smoking, drinking of alcoholic beverages, weight status, dental hygiene, use of seat belts, changes in preventive health behavior due to illness, perceived health status and energy level, use of preventive health services, use of medical and hospital care, behavior with respect to high blood pressure, work status, conditions of work with respect to hazards, stress, physical demands, and sick leave, participation in social groups and religious services, enjoyment of free time, number and closeness of friends and relatives, general psychological well-being, recent stressful life events, and socio-demographic information, such as age, sex, marital status, educational level, race and family income.
