Chronic low back pain is a leading cause of disability and reduced quality of life for nearly 100 million individuals in the United States. Approximately 5 to 10 percent of these individuals are prescribed opioids for treating their chronic low back pain. Limited data exists on the long-term efficacy of opioid treatment while much concern exists regarding the harm they can do to a person. Despite medication treatment patients still suffer from inadequate pain relief, impaired function, and quality of life.
This study sought to compare the long-term effectiveness of two types of alternative therapy approaches - mindfulness based therapy (MBT) and cognitive behavioral therapy (CBT) - for treating chronic low back pain especially those being treated by opioids.
National Comorbidity Survey: Reinterview (NCS-2), 2001-2002 [Restricted-Use] (ICPSR 30921)
The NCS-2 was a re-interview of 5,001 individuals who participated in the Baseline (NCS-1). The study was conducted a decade after the initial baseline survey. The aim was to collect information about changes in mental disorders, substance use disorders, and the predictors and consequences of these changes over the ten years between the two surveys. The collection contains four major sections: the main survey, demographic data, diagnostic data, and state, county, and tract FIPS data.
In the main survey, respondents were asked about general physical and mental health. Questions focused on a variety of health issues, including limitations caused by respondents' health issues, substance use, childhood health, life-threatening illnesses, chronic conditions, medications taken in the past 12 months, level of functioning and symptoms experienced in the past 30 days, and any services used by the respondents since the (NCS-1). Additional questions focused on mental disorders including depression, bipolar disorder, specific and social phobias, generalized anxiety, intermittent explosive disorder, suicidality, post-traumatic stress disorder, neurasthenia, pre-menstrual dysphoric disorder, attention deficit/hyperactivity disorder, oppositional defiant disorder, conduct disorder, and separation anxiety. Respondents were also asked about their lives in general, with topics including employment, finances, marriage, children, their social lives, and stressful life events experienced in the past 12 months. Additionally, two personality assessments were included consisting of respondents' opinions on whether various true/false statements accurately described their personalities. Another focus of the main survey dealt with substance use and abuse, nonmedical use of prescription drugs, and polysubstance use. Interview questions in the NCS-2 Main Survey were customized to each respondent based on previous responses in the Baseline (NCS-1).
The second part contains demographic and other background information including age, education, employment, household composition, household income, marital status, and region.
The third part focuses on whether respondents met diagnostic criteria for psychological disorders asked about in the main survey.
The fourth part contains respondents' state, county, and tract FIPS data.

Investigating the Neurobiologic Basis for Loss of Cortical Laterality in Chronic Stroke Patients, Charleston, South Carolina, 2014-2016 (ICPSR 37313)
The primary goal of this project was to determine the neurobiologic basis for elevated activity in the contralesional primary motor cortex (PMC). In healthy individuals, unimanual movement (with either the left or right hand) is associated with activity in a network of predominantly contralateral brain regions, including the primary motor cortex. This laterality is often compromised following a middle cerebral artery (MCA) stroke. Neuroimaging studies of these patients have shown that unimanual movements with the effected hand are associated with elevated blood oxygen level dependent (BOLD) signal in both the lesioned and the nonlesioned primary motor cortices. Elevated activity in the contralesional PMC is well-established in chronic stroke patients and is associated with poor motor rehabilitation outcomes. Yet the neurobiologic basis for this aberrant neural activity is equivocal.
One factor that may contribute to elevated activity in the contralesional PMC is increased cortical excitatory tone within the contralesional hemisphere. While approximately 80% of the descending corticospinal neurons that control the right hand originate in the left PMC, 20% originate in the right PMC. Elevated activity in the right PMC of left-sided stroke patients may reflect compensatory activity of these descending fibers. Neural activity in the PMC reflects the balance of local excitatory (glutamatergic) and inhibitory (GABAergic) processing. It can be measured in two manners: electrophysiologically, using single hemisphere paired pulse transcranial magnetic stimulation (TMS), and neurochemically, using magnetic resonance spectroscopy (MRS).
Another factor that may contribute to elevated activity in the contralesional PMC is a loss of transcallosal inhibition between the hemispheres. During right hand movement, the left PMC of healthy individuals actively inhibits the right PMC via inhibitory projections through the corpus callosum. In left MCA stroke patients, elevated activity in the contralesional (right) PMC when moving the right hand may reflect a loss of typical inhibition from the left PMC. The integrity of inter-hemispheric information transfer can be measured in two manners: using bi-hemispheric paired-pulse TMS, and using a multimodal brain stimulation/brain imaging approach, interleaved TMS/MRI. Through interleaved TMS/MRI, researchers can selectively stimulate the ipsilesional PMC and quantify the amount of TMS-induced activity in the contralesional PMC.
These two explanations were tested through a cross-sectional investigation of neural function in left MCA stroke patients with mild-moderate right upper extremity impairment and controls matched for age and cardiovascular risk factors. To assess the clinical relevance of these factors on motor dysfunction, the researchers performed a detailed kinematic assessment of movement efficiency, smoothness and compensation.
Language Development of Non-verbal Children Age 3 Years through 7 Years, 2007 to 2012 [Kansas City Metro Area] (ICPSR 36472)
The Language Development of Non-verbal Children Age 3 Years through 7 Years in the Kansas Metro Area is one of the three projects in the Communication of People with MR, 2006 to 2012 Series, which focuses on identifying participant variables that predict success in increasing communication skills of individual with intellectual disabilities. Data for Dataset 1 of this study were collected to illustrate how acquisition of symbolic communication using Voice Output Communication Aid (VOCA) affects the development of successful communication exchanges. For the data collection of Dataset 1, children were recruited by contacting school districts in and near the Kansas City metropolitan area, specifically, in Topeka, Kansas, and Wichita, Kansas. Teachers and speech-language pathologists were asked to nominate any children meeting specific criteria. The 93 children who were enrolled were administered the Mullen Scales of Early Learning and the Preschool Language Scale. A structured play assessment was also administered.
Subsequently, data for Dataset 2 was collected to analyze and compare 19 Spanish-speaking children to the original sample. Both data files contain the results of Complexity of Communication Scale, a measure developed by the Communication of People with MR project.
Requests for Assistance with Adaptive Switches from Individuals with Severe Communication Impairments, 2007 to 2012 [California, Kansas, and Washington] (ICPSR 36515)
The Requests for Assistance with Adaptive Switches from Individuals with Severe Communication Impairments study is one of the three projects in the Communication of People with MR, 2006 to 2012 Series, which focuses on identifying participant variables that predict success in increasing communication skills of individual with intellectual disabilities. The study data were collected to show how children and adults with profound multiple impairments can learn to request assistance with adaptive switches and to on examine the change from pre-intentional to intentional symbolic communication.
The study examined 77 respondents, ages 2 through 71, by administering three assessments: the Communication Complexity Scale (CCS), the Mullen Scales of Early Learning and a functional vision assessment. The CCS was administered to 68 respondents on up to three occasions. The first occasion of administration was to participants who were pre-intentional communicators. The CCS was administered on a second occasion to participants when they were identified as being on the cusp between pre-intentional and intentional communication. The third and final occasion of administration occurred when participants were identified as "learners" and could express preferences using switches and other means.
The Mullen Scales were administered to 22 of the participants. Of the 77 participants initially selected for the study, 7 did not complete either scale. Vision impairment was identified in 68 respondents, 28 who had no functional use and 41 who had some functional use of their vision.
