Community Hospital Program (CHP) Access Impact Evaluation Surveys, 1978-1979, 1981 (ICPSR 8245)

This data collection evaluates group medical practices and the ways in which they affect both access to and use of medical services. Group practices, sponsored by the Robert Wood Johnson Foundation Community Hospital Program (CHP), were selected for use in this assessment. The data were collected by the Center for Health Administration Studies at the University of Chicago, with the assistance of Chilton Research Services. Two surveys were conducted for the study: a baseline survey in 1978-1979 and a follow-up in 1981. Community residents and CHP patients in 12 communities were interviewed. Demographic and medical care data were collected for selected individuals and families in the survey areas. Data on regular sources of medical care for individuals include the type of organization used, type of practice, accessibility, frequency of visits, types of health care professionals seen, cost, and satisfaction. Also in the collection are data on perceived health, episodes of illness (including symptoms, duration, disability days, and doctors consulted), use of preventive health care services, and insurance coverage. Demographic data for individuals and families include age, sex, race, educational attainment, employment, and income. Of the 198 files in this collection, 88 are "raw" data files and 110 are frequencies. The data files consist of four types. The first type are Sample Person files. These contain the responses of group practice patients and community members. The second type are Doctor Episode files, which record doctors and episodes of illness. Family files make up the third type of file, and consist of family members' responses to the survey. Analysis files, linking patient and doctor data, are the fourth type of file. The SPSS frequency files correspond to the data files: two per file for the Sample Person files, and one per file for the remaining three types of files.

Effects of Preferred Provider Organizations on Health Care Use and Costs: Pooled Cross-Sectional Time Series, First Quarter 1988 Through First Quarter 1990 (ICPSR 6373)

This research project studied the effects of Preferred Provider Organizations (PPOs) on health care use and costs in comparison with indemnity health care plans and indemnity plans with utilization review (UR). The data cover nine quarters of a single insurer's claims experience with these types of health care plans. The unit of observation is the employer group covered by a given plan. Variables describing claims experience include number of claims, reimbursed costs after copayments and deductibles, hospital expenditures, number of hospital admissions, percent of claims in different diagnostic categories (surgery, tumors, births, and mental health), and number of tonsillectomy/adenoidectomy and colonoscopy cases. Reported characteristics of these groups include coinsurance rates, plan type, industry of employer, group mean age, percent of covered lives with dental or prescription drug coverage, and percent of covered lives that were women or dependents. In addition, the data contain variables describing the market in which each group was located, such as number of hospital beds in the city or county, number of hospitals and health maintenance organizations in the metropolitan statistical area, median rental cost for housing units in the city or county, percent of county or city that was Black or age 65 or older, number of nonfederal physicians in the county, and number of PPOs in the state.

National Survey of Surgeons on Trauma Care Issues, March-July 1993 (ICPSR 6265)

This study investigated surgeons' practice patterns, experience and training in trauma care, and preferences and opinions about caring for trauma patients. Practice pattern variables include surgical specialty, type of surgical practice arrangement, type of appointment with a medical school or university, membership in the American Academy of Orthopedic Surgeons, the American College of Surgeons, or the American Association of Neurological Surgeons, and whether the respondents' patients paid their bills through private insurance, Medicaid, Medicare, or an HMO. In addition, respondents were queried about their primary hospital, including number of beds, types of physicians employed in the trauma or emergency department, whether the hospital was officially recognized as a trauma center, whether it had a separate clinical trauma service with oversight and responsibility for the care of trauma patients, whether surgical patients were covered 24 hours a day by a resident or in-house physician, and whether there was 24-hour coverage by a resident or in-house physician in the hospital's Intensive Care Unit. To assess experience and training in trauma care, respondents were asked how often they were inappropriately called to evaluate and treat trauma patients, if they had taken trauma call at any hospital during the last 12 months, how many trauma patients they treated during the last 12 months and for what percent of them they received compensation, whether they had taken the Advanced Trauma Life Support (ATLS) Course in the last four years, how much they had learned about trauma from residency training, post-residency fellowship, combat duty in the Armed Forces, journal articles, and colleagues, how confident they were in their ability to provide resuscitation, diagnosis, operative care, and critical care, if they had ever been named in a malpractice suit in a trauma case, non-trauma emergency case, or non-emergency case in certain disease categories, and whether this litigation made them reluctant to take on these types of cases. Preferences and opinions on the care of trauma patients were investigated through questions that asked respondents if they preferred to treat adult or pediatric trauma patients, if they preferred to treat blunt or penetrating trauma, and how taking care of trauma patients affected their image with their peers and community. Respondents were also queried about incentives and disincentives for treating trauma patients, reasons for not providing trauma care, opinions on how trauma cases compared with other emergency cases, and opinions on how various aspects of trauma care in their community were deficient. The data also include information on the age, gender, and geographic location (census region) of the respondents.

Practice Patterns of Young Physicians, 1987: [United States] (ICPSR 9277)

This study investigated the factors that influenced the career decisions of young physicians and the characteristics of their practices. The collection has five datasets: Public-Use Version of the Young Physicians Survey (Dataset 1), Socioeconomic Monitoring System Study (Dataset 2), ZIP Code Data (Dataset 3), Verbatim Responses to the Open-Ended Questions (Dataset 4), and Restricted-Use Version of the Young Physicians Survey (Dataset 5).

The Public-Use Version of the Young Physicians Survey comprises responses from the Young Physicians Survey (YPS), plus merged data from the American Medical Association (AMA) Masterfile and the Association of American Medical Colleges' Student and Applicant Information Management System (SAIMS) database. The YPS interviewed physicians below 40 years of age who recently completed graduate medical training and were in their early years of practice. These physicians were queried about their graduate medical training, perceptions of the medical profession, current practice arrangements, career decisions, family background, patient care activities, and current income and expenses. To obtain information on current practice arrangements, respondents were questioned about the practices they worked in, including who owned the practices, the number of physicians in each practice, specialties or subspecialties practiced, usual fees for selected services, percentages of revenues from HMOs, PPOs, and IPAs, and percentages of patients who were Medicare patients, had no health insurance coverage, or were poor, Black, Hispanic, severely physically disabled, or chronically mentally ill. Questions on career decisions asked respondents about factors that influenced their career choices, such as reasons for working in multiple practices, reasons for leaving past practices, and reasons for deciding in favor of or against self-employment. Information on family background elicited by the survey includes the respondent's race, marital status, and educational debt, parents' income class and education, number of children living in the respondent's home, and whether the respondent's spouse or parents were physicians. Questions on patient care activities included questions on the number of hours spent providing uncompensated health care to the poor, and the number of hours spent with patients in a variety of settings, such as the office, emergency rooms, hospital outpatient clinics, and operating rooms. Information from the AMA Masterfile and the SAIMS database includes board certification status, AMA membership, school and year of graduation, Medical College Admission Test scores, primary undergraduate institution, most recent grade point averages, place of birth, number of acceptances to United States medical schools, parents' occupations, preferred medical specialty, and preferred practice setting.

Dataset 2 comprises responses from the AMA's Socioeconomic Monitoring System (SMS), a semiannual survey of nonfederal physicians that collected data on topics similar to those in the YPS, such as practice ownership, hours spent seeing patients in various settings, income, expenses, and opinions on practice procedures. The SMS data can be used for comparative analyses of young, prime, and senior physicians.

The ZIP Code Data contain estimates for the composition of the population residing in the ZIP code areas of the YPS respondents' main practices. This includes estimates of the size of each ZIP code area population, as well as its components with respect to gender, age, race, Hispanic ethnicity, and income. Also included are estimates of the number of physicians and their composition with respect to age, sex, practice type, and specialty.

Dataset 4 contains verbatim responses to open-ended questions asked in the YPS.

The Restricted-Use Version of the Young Physicians Survey is the same as the Public-Use Version of the Young Physicians Survey, except for some variables that were restricted from general dissemination for reasons of confidentiality. The restricted-use version includes the restricted variables, but the public-use version does not.

Practice Patterns of Young Physicians, 1991: [United States] (ICPSR 6145)

The purpose of this survey was to obtain information on the characteristics and practice patterns of early career physicians in order to analyze trends in physicians' activities and the supply of physicians. To that end, the survey interviewed early career physicians and re-interviewed physicians who participated in the previous survey of early career physicians PRACTICE PATTERNS OF YOUNG PHYSICIANS, 1987 (ICPSR 9277). With separate samples drawn for allopathic and osteopathic physicians, the respondents were interviewed about their medical training, medical education financing, career choices and satisfaction, practice arrangements and compensation, and patient care activities. They were also questioned about medical care management in their practice(s), perceptions of their freedom to deliver care, medical malpractice claims, and the composition of their patients, such as the percent who were poor, black, Hispanic, uninsured, covered by Medicaid or Medicare, or had severe physical disabilities, chronic mental illness, or problems with substance abuse. Demographic characteristics covered by the survey include race, Hispanic origin, year of birth, marital status, number and ages of children, and parents' education.

In addition to the variables collected by survey, the allopath sample data also comprise variables obtained from the American Medical Association (AMA) and the Student and Applicant Information Management System (SAIMS) of the Association of American Medical Colleges (AAMC). The AMA variables include gender, name of medical school, board certification status, physician's specialty, and AMA membership, while the SAIMS variables include dates of application to medical school, graduation dates, Medical College Admission Test (MCAT) scores, undergraduate grade-point averages, religious preference, career preference, preferred practice setting, educational debt, scholarship information, and participation in courses/clerkships in different subject areas.

The study comprises five data files. Dataset 1 contains the public-use version of the data for the allopath sample and Dataset 2 the public-use version of the data data for the osteopath sample. Both of these files were generated by ICPSR from the original restricted-use allopath and osteopath data files provided by the principal investigator, which are stored as Datasets 3 and 4 respectively. As noted in the ICPSR Processing Note in the codebook, Dataset 5 contains 26 restricted variables which the principal investigator omitted from the original allopath data (Dataset 3) for reasons of confidentiality. ICPSR received the omitted variables in 2009, 14 years after its initial release of the data.

Practice Patterns of Young Physicians, 1997: [75 Largest Metropolitan Statistical Areas in the United States] (ICPSR 2829)

This survey reinterviewed a subsample of physicians who responded to the survey PRACTICE PATTERNS OF YOUNG PHYSICIANS, 1991: [UNITED STATES] (ICPSR 6145). Respondents answered questions about their practice arrangements, such as the number of different medical practices that they worked in during the past month, the number of hours spent providing patient care, and the number of patients seen in the past week. They also described the characteristics of their main practice in terms of type of practice setting, practice ownership, number of physicians, percentage of revenues from patients covered by Medicaid and Medicare, share of Medicaid and Medicare revenues from managed care organizations, percentage of patients with no health insurance coverage, and percentage of Black and Hispanic patients. Other information covered whether the practice had a formal mechanism for reviewing clinical practice decisions, whether it had contracted to provide care through a Preferred Provider Organization (PPO), Health Maintenance Organization (HMO), or Independent Practice Arrangement (IPA), whether it was joined with one or more physician practices or was purchased by an insurance company in the past two years, percentage of revenues from PPO, HMO, and IPA arrangements, and whether there were personal financial incentives that favored reducing or expanding services to patients. Additionally, respondents were asked whether contracts or other communications received from insurance plans implied that continued participation in the plans depended upon costs associated with their clinical decisions, or implied that they should not tell patients about restrictions on coverage for medically accepted testing, treatment, or referral options. Other questions probed respondents' career satisfaction, ethical beliefs regarding the practice of medicine, and freedom to practice medicine as they saw fit. The survey also gathered information on determinants of physician compensation, medical specialty, income, marital status, spouse's occupation, and the number of children living with the respondent.

Robert Wood Johnson Foundation Employer Health Insurance Survey [Community Tracking Study and State Initiatives in Health Care Reform Program], 1997 (ICPSR 2935)

The objectives of this study were to describe and understand employers' and employees' behavior with respect to employment-based health insurance, to track trends in health insurance provided by employers, and to evaluate selected policies to regulate or expand employment-based health insurance coverage. Sampling was designed to permit estimates for selected communities that are part of the Robert Wood Johnson Foundation (RWJF) Community Tracking Study (CTS) and for selected states of interest to the RWJF State Initiatives in Health Care Reform Program. Data were collected on employers' offers of health insurance coverage, employees' eligibility and enrollment in health plans, and, for each plan offered, the plan type (HMO, POS, PPO, conventional), premiums (employer and employee contributions), benefits, cost-sharing, and employer self-insurance status. The study also collected information on the characteristics of employers and workers, including the number of employees at the establishment, the number of employees statewide and nationwide, and the distribution of workers by hours worked, age, sex, and earnings.

This survey was conducted as part of an evaluation of the Robert Wood Johnson Foundation's Health Care for the Uninsured Program (HCUP), a program whose primary focus was the development and marketing of affordable health insurance products for small businesses. The survey investigated the number and types of small businesses that offered and did not offer insurance, the number and types of employees of small businesses who received and did not receive insurance, and whether the employers and employees participating in HCUP were different from those with other types of insurance or from those with no insurance. In addition, the survey was designed to test several hypotheses: whether employers facing an inelastic demand for their product or a tight labor market would be more likely to offer health insurance to their employees, and whether higher wages substitute for health insurance for certain groups of highly skilled or unionized workers. Firm-level data collected by the survey include number of permanent and temporary employees, employee turnover, fringe benefits offered to full- and part-time employees (e.g., paid vacation, paid sick leave, long-term disability insurance, life insurance, retirement plan, group health insurance), type of business, number of years owner had owned the company, age and legal form of the company, and gross revenue. Extensive information on health insurance was obtained from firms offering this benefit: total monthly premium paid for health insurance, percent of premium paid by the company, reasons that influenced the decision to provide health insurance, whether a Health Maintenance Organization (HMO) insurance plan was offered, whether a deductible or co-payment was required for hospital inpatient services, and whether hospital room and board, physician office visits, maternity care, prescription drugs, inpatient mental health treatment, or substance abuse treatment were covered. These firms were also queried about recent changes in the number of health plan enrollees, deductibles, co-insurance rates, benefits offered, employer premium share, recent changes in health insurance carriers and reasons for changing, and recent increases in premiums and their effects on the firm's prices, profits, wages, and number of employees. Companies not offering health insurance were asked why they did not offer this benefit and were queried about factors that might influence them to offer a health plan. Individual-level data on employees include sex, age, marital status, length of employment, number of hours worked during the last week, salary or wage, health plan participation, amount of health premium paid by the employee, and whether the employee had health coverage from another source.

This is the second wave of a split-panel study first conducted in 1990 (SMALL BUSINESS BENEFITS STUDY (SBBS), 1990 [DENVER, FLINT, TAMPA, AND TUCSON] [ICPSR 6002]). Both waves examined the characteristics of small businesses and their health insurance markets. SBBS 2 also collected follow-up information on the respondents to the first wave. Firm-level data collected by SBBS 2 include type of business, age of the firm, number of years under the current owner, gross receipts, number of employees, and whether the firm offered health insurance. For firms that offered health insurance benefits, respondents were queried on the continuity of benefits, why firms changed insurers (where applicable), characteristics of the insurance plan, and how the firm would respond to a 25-percent increase in premiums. For firms that did not offer health insurance benefits, respondents were asked whether insurance was available in the last five years, why the firm decided to stop offering insurance if it had offered health insurance previously, whether the firm was interested in offering insurance, and factors that might influence the firm to offer insurance. Individual-level data on employees include gender, age, marital status, salary and wages, hours worked, and length of employment
