2005-11-04 On 2005-03-14 new files were added to
one or more datasets. These files included additional setup files as
well as one or more of the following: SAS program, SAS transport, SPSS
portable, and Stata system files. The metadata record was revised

2004-07-30 Value labels were added to the data
definition statements. For Part 1 (Form 1 Questionnaire), the
following 22 variables were recoded to correct coding errors: V1227,
V1228, V1230, V1232 to V1239, and V1542 to V1552. For Part 3 (Form 3
Questionnaire), the following 14 variables were removed due to
undefined codes: V3319 to V3332. Minor changes were made to the
