[DDI-SRG] 2 to 3 mapping

Mary,

Here is a zip file of the updated documents for mapping DDI 2 to DDI 3.0. 
You may post this set, but please post it as a zip file. Both the readme 
file and the document stress the need to read the document prior to 
working with the spreadsheet. I've incorporated the changes made in 
response to Sanda's review as well as some discussion of a couple of 
issues that were raised.
