[DDI-SRG] code value type location

Ken,

The zip file contains 3 files.

r_codevaluetyp.txt which lists all the locations where CodeValueType is 
available including restrictions and extentions.

internalcodetypes.txt lists all the internal codesets used and their 
contents READ THESE there are some errors.

att_xs_string.txt  a list of elements and attributes currently xs:string 
which might make use of CodeValueType. Many of these are really marginal. 
I left off the ones where it was obvious that you wouldn't want a 
controlled vocabulary.
