[DDI-SRG] "representation" question

Sanda,
I discussed this at the TIC meeting with Ken, Pascal, and Achim. We have 
decided that for now the approach will be to create a categoryScheme with 
the anchor categories and a third category with an empty label and a 
description of "an unlabeled interceding value in a coded response range"

CodeScheme
   CatSchemeANCHOREDSCALE
    1 = base anchor category
    2 = empty label anchor
    3 = empty label anchor
    4 = empty label anchor
    5 = empty label anchor
    6 = empty label anchor
    7 = top anchor category

Describe the level providing the relationship type and itervals. All codes 
would be of this level.

We are also putting this in as a bug to review for 3.1. However Ken was 
comforable with this approach for now.
