[DDI-SRG] Following note added to Bug 153

I have reviewed the contents of the schemas and all instances where 
CodeValueType is used are optional with the following exception:

Its use in describing the code of a geographic level or geography value in 
which case the use of the parent level is optional. In short, you use it 
if it has a code you want to describe and if you decide to describe the 
code, you're forced to put in the code. In this case there is no logical 
case where it would be empty.

In other words you would be solving a problem that really doesn't exist. 
If I'm not using it, it just don't put it in, or put in its parent in the 
geography case noted above.
