[DDI-SRG] Mixing code and value ranges

Pascal...I didn't mean that you'd make a code list for the number of cats.
Ideally, the number of cats would be listed as 1-6 numeric range with a
missingValue attribute listing the missing value 55. What this does not
support is when you have say 3 missing values 55, 56, 57.
55 = refused to answer
56 = don't know
57 = unable to count
We can't link the missing value labels to the missing values as listed in
the missingValue attribute.
