The National Health Interview Survey, 1994: Second Supplement on Aging (SOA II), conducted approximately 10 years after the original SOA (see NATIONAL HEALTH INTERVIEW SURVEY, 1984 [ICPSR 8659], Parts 6 and 7), had four specific aims. The first was to provide a replication of the first SOA to determine whether changes had occurred in the level of disability among older persons between 1984 and the mid-1990s. The second aim was to elicit information on the causes and correlates of changes in health and functioning in older Americans, including background demographic characteristics, health behaviors, and attitudes, pre-existing illness, and social and environmental support. The third was to describe the sequence and consequences of health events, including utilization of health care and services for assisted community living, on the physiological consequences of disability such as pain and fatigue, on social consequences such as changes in social activities, living arrangements, social support, and use of community services, and on the deployment of assisted living strategies and accessibility of technological and environmental adaptations. The final aim was to serve as the baseline for another national longitudinal study focusing on older Americans, the SECOND LONGITUDINAL STUDY OF AGING (LSOA II) (for the first LSOA see NATIONAL HEALTH INTERVIEW SURVEY: LONGITUDINAL STUDY OF AGING, 70 YEARS AND OVER, 1984-1990 [ICPSR 8719]).

In preparing the data file for this collection, the National Center for Health Statistics (NCHS) has removed direct identifiers and characteristics that might lead to identification of data subjects. As an additional precaution NCHS requires, under Section 308(d) of the Public Health Service Act (42 U.S.C. 242m), that data collected by NCHS not be used for any purpose other than statistical analysis and reporting. NCHS further requires that analysts not use the data to learn the identity of any persons or establishments and that the director of NCHS be notified if any identities are inadvertently discovered. ICPSR member institutions and other users ordering data from ICPSR are expected to adhere to these restrictions.

All individuals 70 years of age and over, for whom data were collected as part of the 1994 National Health Interview Survey core interview. The NHIS core interview used a stratified multistage probability design.

1999-05-12 The codebook has been replaced with the codebook from the NCHS CD-ROM for this study (Version 2). It includes additional variable names that relate to the SAS and SPSS data definition statements that are also now available for this data collection.

1998-11-16 ICPSR data undergo a confidentiality review and are altered when necessary to limit the risk of disclosure. ICPSR also routinely creates ready-to-go data files along with setups in the major statistical software formats as well as standard codebooks to accompany the data. In addition to these procedures, ICPSR performed the following processing steps for this data collection:
