Description	The subject and young adult version of the Generalized Anxiety Disorder instrument was adapted from the Anxiety module of the Diagnostic Interview Schedule for Children (DISC 4) and obtained information regarding subjects' worrying and anxiety. WAVE 2: The subject version of the Generalized Anxiety Disorder instrument (PDF 2.3MB) was administered to subjects for Cohorts 9 to 15. WAVE 3: The subject and young adult version of the Generalized Anxiety Disorder instrument (PDF 2.5MB) was administered to subjects for Cohorts 6 to 15.
