Description	The primary caregiver version of the Generalized Anxiety Disorder instrument was adapted from the Anxiety module of the Diagnostic Interview Schedule for Children (DISC 4) and obtained information regarding subjects' worrying and anxiety. WAVE 2: The primary caregiver version of the Generalized Anxiety Disorder instrument (PDF 2.3MB) was administered to subjects' primary caregivers for Cohorts 3, 6, and 9. WAVE 3: The primary caregiver version of the Generalized Anxiety Disorder instrument (PDF 2.9MB) was administered to subjects' primary caregivers for Cohorts 3, 6, and 9.
